OBJECTIVES: AA amyloidosis (AAA) is a complication of chronic inflammation whose burden is expected to decrease in the era of biological therapies. We assessed the prevalence, clinical and laboratory manifestations, management and evolution of AAA in patients with inflammatory joint diseases (IJDs).

In 1986 and 1987 nine patients with a raised antibody titre to Yersinia pseudotuberculosis were found. Two of these results were almost certainly due to the cross reactivity between some Y pseudotuberculosis serogroups and some salmonella serotypes. Of the other seven patients with otherwise unexplained Y pseudotuberculosis serology, three suffered from severe arthritis and two had symptoms suggestive of sacroiliitis. Our data and published reports suggest that reactive arthritis might follow both Y enterocolitica and Y pseudotuberculosis infections.

A 43-year-old Japanese woman with adult T-cell leukemia (ATL) developed rheumatoid arthritis-like polyarthritis with dermatitis and skin erosion. Her rheumatoid factor and C-reactive protein results were positive. Musculoskeletal ultrasonography showed intra-articular and peritendinous power Doppler signal-positive synovitis. Plain magnetic resonance imaging showed synovitis of the above lesion and remarkable bone marrow edema/osteitis. She was diagnosed as having ATL-associated arthritis based on the invasion of ATL cells by skin biopsy at the arthritis lesion.

In contrast to rheumatoid arthritis (RA), the triggering antigens are known in reactive arthritis (ReA) and Lyme arthritis. Thus, in these arthritides the antigen-specific T-cell response can be investigated in much detail and lessons possibly learned for other spondyloarthropathies (SpA) such as ankylosing spondylitis (AS) where T cells may well also play an important role in the pathogenesis. Antibodies to Borrelia spirochetes were determined by an enzyme-linked immunosorbent assay (ELISA) in sera from 298 patients with postinfectious arthritis. Sera from healthy individuals, patients with acute infectious meningitis of proven etiology and patients with multiple sclerosis served as controls. Although arthralgia is a common complaint in Mediterranean spotted fever, frank arthritis is very unusual. We report two cases of this condition during the course of Mediterranean spotted fever. Synovial fluid was inflammatory in both patients and the disease resolved after doxycycline therapy. Immunofluorescence test for Rickettsia conorii in synovial fluid was negative, suggesting a reactive arthritis.

Salmonella dublin is a rare pathogen in man usually causing an enteric fever syndrome. We report a 32-year-old black male who developed septic polyarthritis with Salmonella dublin. The clinical features are similar to those seen with other Salmonella serotypes reported to cause arthritis. The initial presentation is suggestive of a reactive arthritis that is immunologically mediated.
