Atrial functional mitral regurgitation- A review with emphasis on therapeutic strategies.

Atrial functional mitral regurgitation (AFMR) may account for a substantial proportion of functional MR, although its prevalence varies by population and diagnostic definition. AFMR increases mortality risk regardless of left ventricular (LV) function. The mechanism involves mitral annular dilation and "atriogenic leaflet tethering." Key echocardiographic features include preserved left ventricular ejection fraction (LVEF; typically >50%), significant left atrial (LA) enlargement, mitral annular dilation, and minimal leaflet tethering. Medical therapy, including renin-angiotensin system inhibi

Psoriasis and atrial fibrillation: Exploring the intersection.

This review explores the emerging connection between psoriasis and atrial fibrillation (AF), focusing on shared inflammatory mechanisms, clinical implications, and research gaps. Psoriasis, characterized by chronic systemic inflammation, has been associated with increased AF risk, driven by elevated pro-inflammatory cytokines such as interleukin (IL)-6, IL-17, and tumor necrosis factor-alpha. These inflammatory mediators contribute to atrial remodeling, fibrosis, and conduction abnormalities, evidenced by prolonged P-wave dispersion and atrial electromechanical delay in psoriasis patients. Sev…

Atrial TRPM2 Channel-Mediated Ca2+ Influx Regulates ANP Secretion and Protects Against Isoproterenol-Induced Cardiac Hypertrophy and Fibrosis.

Transient receptor potential melastatin 2 (TRPM2) channel is a Ca2+-permeable, redox-activated cardiac ion channel protective in ischemia-reperfusion, but whether it regulates atrial endocrine output under stress is unclear. Here, we investigated whether TRPM2 contributes to the atrial natriuretic peptide (ANP) response during β-adrenergic stimulation. We compared how male C57BL/6J wild-type (WT) and TRPM2 knockout (TRPM2-/-) mice (8-12 weeks old) respond to β-adrenergic stress induced by isoproterenol (ISO) using echocardiography, histology, RT-PCR, electrophysiology, Ca2+ imaging, ELISA, and

A comparison of early versus delayed elective electrical cardioversion for recurrent episodes of persistent atrial fibrillation: A multi-center study.

BACKGROUND: Due to barriers to accessing timely elective electrical cardioversion (CV) for persistent AF (PeAF), we adopted a policy of instructing patients to present directly to the Emergency Department (ED) for CV. OBJECTIVE: We compare a strategy of Emergency CV (ED-CV) versus Elective CV (EL-CV) for treatment of symptomatic PeAF. METHODS: Between 2014 and 7, we evaluated 150 patients undergoing CV for PeAF. ED-CV patients were provided an AF action plan for recurrent symptoms and advised to present to ED within 36 h. EL-CV patients followed standard care, including cardiologist referral

Alamandine Attenuates Hypoxia-Induced Atrial Fibrillation in Rats: Involvement of Endoplasmic Reticulum Stress and Pyroptosis Pathways.

Chronic intermittent hypoxia (CIH) constitutes the primary pathophysiological disturbance in obstructive sleep apnoea (OSA), recognised as an independent risk factor for cardiovascular disease. The protective peptide alamandine (ALA), emerging as a novel component of the renin-angiotensin system, holds potential for elucidating molecular mechanisms underlying OSA-associated atrial fibrillation (AF). This study demonstrates that ALA treatment downregulates the expression of molecules associated with the GRP78/ATF6/CHOP pathway and pyroptosis-related proteins, while simultaneously mitigating atr

Congestive Heart Failure Leads to Prolongation of the PR Interval and Atrioventricular Junction Enlargement and Ion Channel Remodelling in the Rabbit.

Heart failure is a major killer worldwide. Atrioventricular conduction block is common in heart failure; it is associated with worse outcomes and can lead to syncope and bradycardic death. We examine the effect of heart failure on anatomical and ion channel remodelling in the rabbit atrioventricular junction (AVJ). Heart failure was induced in New Zealand rabbits by disruption of the aortic valve and banding of the abdominal aorta resulting in volume and pressure overload. Laser micro-dissection and real-time polymerase chain reaction (RT-PCR) were employed to investigate the effects of heart

Assessment of persistent atrial fibrillation arrhythmogenesis in the right atrium using digital twins.

BACKGROUND: Pulmonary vein isolation (PVI) is the cornerstone of atrial fibrillation (AF) therapies; however, in persistent AF (PsAF), characterized with fibrosis proliferation, its success is limited. Various extra-PVI strategies have also failed to improve outcome. Although PsAF entails inflammation-driven remodeling throughout the atria, the left atrium (LA) has been the main focus of ablative therapies. We hypothesized that the suboptimal outcomes of PsAF treatment are attributable to the arrhythmogenic role of the right atrium (RA). OBJECTIVE: Using personalized digital twins (DTs), an e

Temporal gene expression profiling reveals the protective mechanism of APIP in atrial fibrillation following myocardial infarction.

BACKGROUND: Atrial fibrillation (AF), the most prevalent form of cardiac arrhythmia, frequently develops as a complication of acute myocardi … us time points following MI. Cardiomyocyte-specific overexpression of APIP significantly attenuated atrial remodeling and fibrillation following MI. These beneficial effects were accompanied by elevated Mfn1/Mfn2 and p-Drp1(S637) levels, reduced Drp1 expression levels, and enhanced mitochondrial function. Finally, APIP upregulated citrate synthase (CS), enhanced re …

Preprocedural M2-muscarinic acetylcholine receptor autoantibody levels predict left atrial low-voltage area severity in atrial fibrillation undergoing catheter ablation.

BACKGROUND: Autoantibodies against the M2-muscarinic acetylcholine receptor (anti-M2-R) have been implicated in atrial remodeling and atrial fibrillation (AF) pathogenesis. However, their relationship with atrial fibrosis severity, as assessed by low-voltage areas (LVAs), remains unclear. METHODS: This prospective study included 164 patients (89 with paroxysmal AF [PAF], 75 with non-paroxysmal AF [NPAF]) undergoing first-time catheter ablation. Preprocedural plasma anti-M2-R levels were measured using ELISA. Left atrial LVAs were assessed via electroanatomic voltage mapping and categorized in…

Role of Adjunctive Cryoballoon Left Atrial Posterior Wall Isolation in Non-paroxysmal Atrial Fibrillation: A Meta-analysis.

Progressive remodeling of left atrial posterior wall (LAPW) plays an important role in the pathology of persistent/ long-standing persistent atrial fibrillation. The role of pulmonary veins isolation (PVI) and adjunctive LAPW isolation using cryothermal energy in non-paroxysmal atrial fibrillation is yet to be established. The databases of Pubmed, EMBASE and Cochrane Central databases were searched from inception to April 2022. Relevant randomized trials and observational studies comparing de novo cryoballoon PVI only versus PVI+LAPWI in patients with non paroxysmal AF were identified and a me

Expression of opioid receptor subtypes and their ligands in fibrillating human atria.

The delta- and kappa-receptor subtypes are both abundantly expressed in the human heart and participate in age- and stress-related alterations of cardiac function. Opioid receptor agonists mediate cardioprotection in response to ischemic preconditioning via increased intracellular Ca(2) (+) levels, opening mitochondrial K(ATP) channels, and PKC activation. We studied the expression of opioid receptor subtypes kappa and delta, and of their ligand precursors, proopiomelanocortin (POMC) and preproenkephalin A (PENKA), in human atrial tissue of patients in sinus rhythm (SR), or persistent atrial f

Right Heart Structure and Function in Lifelong Recreational Endurance Athletes with and without Paroxysmal Atrial Fibrillation.

BACKGROUND: Healthy young athletes adapt to the increased demands of endurance exercise with symmetric cardiac remodeling. Male veteran endurance athletes have an increased risk for atrial fibrillation (AF), and some athletes seem susceptible to changes mimicking arrhythmogenic cardiomyopathy. Intense exercise puts a disproportionate hemodynamic load on the right-sided heart chambers. Despite this, data describing right heart structure and function in older veteran athletes are scarce. The aim of this study was to investigate structural and functional characteristics of the right heart in vete

Anabolic steroid excess and myocardial infarction: From ischemia to reperfusion injury.

Anabolic steroids (AS) are synthetic testosterone-derivatives developed by the pharmaceutical industry to mimic testosterone biological effects. So far, AS have been implicated in the treatment of pathological conditions, such as hypogonadism, anemia, and cachexia. Since their discovery, though, AS have been illicitly used by elite and recreational athletes, bodybuilders and weightlifters in order to enhance athletic and aesthetic performance. This practice is characterized by cycles of administration and withdrawal, the combination of different AS compounds, and administration of doses 50 - 1

Transient left bundle branch block and intraventricular dyssynchrony as a cause of reversible left ventricular dysfunction: The "in vivo" documentation of spontaneous electrical remodeling.

Spontaneous resolution of non-rate-dependent left bundle branch block (LBBB) has been rarely reported. We present the case of a 74-year-old woman admitted with pulmonary edema, a newly diagnosed LBBB and severe left ventricular (LV) dysfunction. Five months later, the patient was asymptomatic, the ECG recording showed complete regression of the LBBB to narrow QRS and LV function completely recovered. However, at one-year follow-up LBBB reappeared together with mild LV dysfunction. Spontaneous resolution of LBBB may be responsible for LV electrical and mechanical reverse remodeling in dyssynchr

Atrial natriuretic peptide in cardiovascular biology and disease (NPPA).

Atrial natriuretic peptide (ANP) is a cardiac hormone that regulates salt-water balance and blood pressure by promoting renal sodium and water excretion and stimulating vasodilation. ANP also has an anti-hypertrophic function in the heart, which is independent of its systemic blood pressure-lowering effect. In mice, ANP deficiency causes salt-sensitive hypertension and cardiac hypertrophy. Recent studies have shown that ANP plays an important role in regulating vascular remodeling and energy metabolism. Variants in the human NPPA gene, encoding the ANP precursor, are associated with hypertensi

Does left atrial enlargement contribute to mitral leaflet tethering in patients with functional mitral regurgitation? Proposed role of atriogenic leaflet tethering.

Papillary muscle repositioning due to left ventricular remodeling is thought to be the primary mechanism responsible for functional mitral regurgitation. This article proposes that remodeling (enlargement) of the left atrium also plays a role in the pathogenesis of functional mitral regurgitation, through a novel mechanism-atriogenic leaflet tethering-described herein.

Proprotein Convertase Subtilisin/Kexin 6 in Cardiovascular Biology and Disease.

Proprotein convertase subtilisin/kexin 6 (PCSK6) is a secreted serine protease expressed in most major organs, where it cleaves a wide range of growth factors, signaling molecules, peptide hormones, proteolytic enzymes, and adhesion proteins. Studies in Pcsk6-deficient mice have demonstrated the importance of Pcsk6 in embryonic development, body axis specification, ovarian function, and extracellular matrix remodeling in articular cartilage. In the cardiovascular system, PCSK6 acts as a key modulator in heart formation, lipoprotein metabolism, body fluid homeostasis, cardiac repair, and vascul
