Use of HIV Phylogenetic Analysis in Public Health in Victoria, Australia: Regulatory Considerations.

Human immunodeficiency virus (HIV) phylogenetic analysis has been introduced in a select number of jurisdictions globally for the purposes of HIV public health surveillance. Using this epidemiological tool in this way can enhance public health efforts aimed at managing HIV transmission. This application, however, presents many legal and ethical concerns that may counter such efforts, particularly in regulatory environments where HIV transmission can be subject to criminal prosecution. This article uses Victoria, Australia, as a case study and analyses the current regulatory frameworks relevant

Building Moral Community in an Age of Polarization: A Model for Reconciling Rights of Conscience in Healthcare Organizations.

Responding to conscientious objection and participation declarations to various morally contentious medical services requires a careful, inclusive, and comprehensive approach in reconciling the right of conscience of involved parties. In this paper, we make a case that the traditional discourse of whether or not to accommodate conscience-based requests does not adequately support complex, team-based healthcare systems in addressing emerging areas of moral controversy in medicine, particularly in this era of polarization. We describe how key parties in our organization developed an inclusive an

Expanding Clinical Deliberation: Affective Embodiment in Cosmetic Gynaecology.

Cosmetic gynaecological procedures such as labiaplasty are commonly evaluated through frameworks of voluntary informed consent and adaptive preference. These approaches assess whether a patient's preference for a procedure has developed under conditions of adequate understanding and voluntariness. While necessary, this focus on preference formation does not exhaust what is ethically at stake in a given case. To account for this further dimension, this paper develops an account of affective embodiment as a way of describing lived bodily orientation, interpreting how social norms surrounding vul

Primary Care Physician Perspectives on Artificial Intelligence for Dementia Screening: A Qualitative Study.

BACKGROUND: Automated approaches to cognitive impairment screening may soon achieve sufficient levels of accuracy for clinical implementation but they present potentially serious ethical challenges. Overcoming such challenges for successful implementation of automated screening may depend on the perspective of the clinicians who are its intended users. METHODS: We conducted a qualitative study of internal medicine, geriatrics and family medicine physicians recruited from ambulatory practices in New York City, NY to identify ethical challenges of implementing automated CI screening (n = 22). Imperatives in Social Media and Beyond: A Lingisian Analysis.

It has become a commonplace that our phones and other devices have a strange and unhealthy hold on us. Social media seemed to promise us a world where we are all linked into a borderless community where each could be seen, recognized, and valued in our individuality. But in fact, things have turned out very differently. Our devices, social media, and A.I. grab and manipulate our attention with the peculiar force of moral and psychological imperatives. In this paper, I turn to the philosopher Alphonso Lingis, who has written with uncommon clarity and depth about the nature of imperatives and of

Impact of Mandatory Clinical Ethics Consultation for Neurocritically Ill Patients.

This study aimed to investigate and compare the longitudinal impacts of mandatory clinical ethics consultation (CEC) on resource use and ethical conflicts among critically ill neurologic and non-neurologic patients. It also sought to identify relevant predictive factors and determine family member satisfaction with CEC. This combined retrospective and prospective cohort study was conducted at a tertiary academic medical centre. During the eight-year study period, 1,150 adult patients participated in this study (437 neurological and 713 non-neurological). In both groups, the ICU length of stay

Deliberate infection in research: ethical perspective of controlled human infection studies.

A controlled human infection study is the deliberate exposure of a healthy volunteer to an infectious pathogen under a strict controlled environment. Although there has been noticeable contribution of such studies to the biomedical research involving understanding of disease pathogen interaction, vaccine development, etc, they are in direct contradiction of core ethical principles such as non-maleficence. The major ethical dilemma lies in the deliberate exposure without certain direct benefits. This narrative review discusses the ethical challenges and need for robust ethical framework taking

Navigating Ambivalence: What It Means to Share Your Health Story Online.

With increasing frequency, personal medical journeys are shared online. While online platforms can provide life-saving community, emotional validation, and practical "lived expertise," they may also expose users to harassment, misinformation, and emotional exhaustion. This commentary analyzes the experiences of 12 people who have shared their personal health journeys on social media. Overall, the narrative authors show deep ambivalence toward sharing. I observe three core tensions in the narratives: (a) community versus critics, where the authors find comfort, support, and solidarity online, b

The Moral Principles of Gamete Donor Screening.

Gamete Donation is a widely accepted method which allows for individuals who may be limited by medical, genetic, or social reasons to conceive a child. While the ethics of gamete donation have been widely discussed, no discussion of the ethics of gamete donor screening and the different levels at which it can be understood has previously taken place. This paper discusses gamete donor screening in the context of Australia and proposes that there are three distinct levels to screening: legislation, donor banks, and individual selection. Through an examination of what has been deemed permissible

Ethical Issues Conducting Research During War and Violent Conflict: A Review.

Global health engagement increasingly occurs in the context of war and violent conflict. Many health-related decisions are made which should be guided by evidence. Health research is needed to provide evidence, yet conducting research in conflict settings raises multiple ethical issues. However, the ethical issues involved in conducting research during war and violent conflict have received insufficient attention. The literature suggests that these ethical issues, while similar to those of research conducted in humanitarian crises, are also distinct. This article provides a review of the ethic

Clinician Mistrust in Patients: A Survey of Primary and Urgent Care Clinicians in Colorado.

BACKGROUND: Few studies have examined clinicians' mistrust in their patients. We explored this concept in greater detail. METHODS: We performed a survey (62.8% RR) of clinicians in primary and urgent care clinics at a university health system and a safety net health system in Denver, Colorado. The survey measured the frequency of clinician mistrust in patients, attitudes about mistrusting patients, perceived benefits and harms of mistrusting patients, and factors associated with mistrusting patients. RESULTS: On the survey virtually all clinicians (99.5%) indicated they mistrusted some patie

Ethics and Inequities in the Context of Urgency : A Qualitative Study With Stakeholders on COVID-19 Decision-Making in Germany and Israel.

Urgent decision-making during the early COVID-19 pandemic raised complex ethical challenges, in particular concerning how to reach and include marginalized populations. To examine these issues, we conducted a qualitative study based on twenty-three semi-structured interviews with pandemic decision-makers and representatives of marginalized populations in Germany and Israel, focusing on decisions made in 2020 before vaccines were available. Key ethical themes emerged, including decision-making with ethical relevance under intense time pressure, the dominance of biomedical framings often at the

The last low whispers revisited: a reply to Sulmasy on palliative sedation.

Many ethicists who have opposed euthanasia have nonetheless approved of various practices which have been believed to hasten, or risk hastening, the death of terminally ill and suffering patients. For instance, they have approved of a practice which I shall call 'end-of-life sedation,' or 'EOL sedation.' This practice involves administering drugs with sedating effects to relieve suffering at the end of life. Some have believed that EOL sedation risks hastening death by suppressing respiration in certain cases. The main difference, according to these opponents of euthanasia, is that whereas eut

The Community of Those Who Have Nothing But Cancer in Common.

In The Community of Those Who Have Nothing is Common, Alfonso Lingis begins his exploration of the "other community" with the story of those dying in hospital. The spectre of mortality is constricting and limiting; yet, within the milieu of finality, individuals are freed from many constraints, opening doors to new possibilities. In facing our own mortality, we recognize "others" grappling, like ourselves, with insoluble questions, while trying to work out how to act. In the darkness, chaos and terror of diagnosis and treatment, communities are thereby formed of individuals who have nothing bu

Clinical Partnering - When Should Host Nation Patients Not be Directed to Coalition Military Hospitals?

This case commentary explores whether and when it is justifiable NOT to direct injured or ill host nation patients to international coalition military hospitals that are better equipped than host nation hospitals to meet patients' health needs in order to ensure sufficient patients for success in clinical partnering. The case discusses Medical Rules of Eligibility as a policy to determine the destination of patients who present for medical care to international military medical services. It also discusses the ethical challenges for international military healthcare workers when working as clin

Establishing Clinical Trial Priorities for Military Global Health Research.

Military leaders must support health care and research to protect the health and readiness of their forces. Part of this effort entails conducting human subjects research to address current and emerging international health threats. The military is also attentive to the national security interests associated with global health, such as protection from widespread disruption due to disasters and epidemics. While military leaders' global health research objectives relate to the military's strategic goals, including force health protection and national security, it is only through longstanding par

"I Have Someone's Life in my Hands": How Cancer Care Partners Experience and Navigate Moral Challenges.

BACKGROUND: Cancer care partners (also known as caregivers or caretakers) are individuals without formal training or experience in cancer care who provide health or social support to others with cancer. Care partners are oftentimes family or friends of people with cancer. While the experiences of cancer care partners are well documented, little is known about the moral challenges they face. The purpose of this analysis was to explore the moral landscape of cancer care partners' experiences across disease and treatment trajectories. METHODS: We used the HEXI methodology and dimensional analysi

A Need for Societal Involvement in Promoting a Culture of Care in Animal Research.

Evidence shows that people doing biomedical research with non-human animals suffer psychological consequences. A culture of care in animal research demands ensuring the well-being of personnel and research animals, and proposals for ethical oversight, social support, and external interventions to address the psychological burden on personnel are growing. With biomedical research being a public good, and the people involved doing so to advance scientific knowledge for societal health benefits, society bears some responsibility for animal research and could be involved in ways that promote the w

Clinical reasoning as a hermeneutic arc: teleology, narrative, and practical wisdom in patient care.

Clinical reasoning is often described through partially independent frameworks, including causal explanation, probabilistic inference, ethical deliberation, and narrative understanding. What remains insufficiently clarified is how these dimensions are structurally integrated in the care of a particular patient. Drawing on Paul Ricoeur, this paper argues that clinical reasoning is best understood as a hermeneutic and practical movement structured by three interrelated arcs: an epistemological arc (from naive understanding, through explanation, to informed understanding), an ethical arc (from te
