Bilateral consecutive blindness due to sphenoid sinus mucocele with unilateral partial recovery.

BACKGROUND: We present an interventional case report of a rare occurrence of sphenoid sinus mucocele causing bilateral consecutive blindness with unilateral partial recovery after endoscopic surgery. METHODS: A 64-year-old woman with known sphenoid sinus mucocele and right blindness was referred because of an acute drop of vision to hand motion in her only-seeing left eye over 5 days. Imaging revealed a large mucocele enlarging the sphenoid sinus, eroding the base of the skull, protruding into the cranial cavity, and compressing the left optic nerve. RESULTS: Urgent endoscopic sphenoidotomy …

Risk factors for recurrence of postoperative trichiasis: implications for trachoma blindness prevention.

BACKGROUND: Trachoma is the leading worldwide cause of preventable blindness. Surgery can alleviate trachomatous trichiasis (TT), the blinding sequelae of trachoma, but recurrence rates are high. OBJECTIVE: To investigate risk factors for TT recurrence. METHODS: We conducted a prospective case-control cohort study in Nepal that included patients with TT who were undergoing bilamellar tarsal-rotation surgery and control subjects who have scarring but no TT. Conjunctivae were graded for trachoma and swabbed to detect Chlamydia trachomatis by in-house and commercial (Roche Amplicor; Roche Diagn…

Sociodemographic characteristics associated with blindness in a Nile Delta governorate of Egypt.

BACKGROUND: Globally, blindness is associated with old age and being female. Other sociodemographic and socioeconomic status characteristics associated with blindness have included educational attainment, and occupation. These factors reflect exposure to specific risk factors for blinding eye diseases and utilisation of preventive and curative services by specific sectors of the population. METHODS: A population based survey of blindness and trachoma was conducted in Menofiya governorate in Egypt. 3322 adults 50 years of age and over were sampled from throughout the governorate (population 2.…

Incidence of blindness in southern Germany due to glaucoma and degenerative conditions.

PURPOSE: To estimate population-based incidence rates of registered blindness separately, to determine its main causes. METHODS: The files of all newly registered blindness-allowance recipients in Württemberg-Hohenzollern, Germany (population: approximately 5 million), between 1994 and 1998 were reviewed. From ophthalmological reports on file the fulfillment of the German criteria for blindness (visual acuity of 1/50 or less or equivalent reduction of visual function) was ascertained, and the causes of blindness were obtained. Incidence rates of blindness due to macular degeneration, glaucoma…

Childhood blindness in the context of VISION 2020--the right to sight.

The major causes of blindness in children vary widely from region to region, being largely determined by socioeconomic development, and the availability of primary health care and eye care services. In high-income countries, lesions of the optic nerve and higher visual pathways predominate as the cause of blindness, while corneal scarring from measles, vitamin A deficiency, the use of harmful traditional eye remedies, and ophthalmia neonatorum are the major causes in low-income countries. Retinopathy of prematurity is an important cause in middle-income countries. Other significant causes in a…

Does interhemispheric competition mediate motion-induced blindness? A transcranial magnetic stimulation study.

Motion-induced blindness (MIB) is a phenomenon, perhaps related to perceptual rivalry, where stationary targets disappear and reappear in a cyclic mode when viewed against a background (mask) of coherent, apparent 3-D motion. Since MIB has recently been shown to share similar temporal properties with binocular rivalry, we probed the appearance-disappearance cycle of MIB using unilateral, single-pulse transcranial magnetic stimulation (TMS)--a manipulation that has previously been shown to influence binocular rivalry. Effects were seen for both hemispheres when the timing of TMS was determined …

The role of endosymbiotic Wolbachia bacteria in the pathogenesis of river blindness.

Parasitic filarial nematodes infect more than 200 million individuals worldwide, causing debilitating inflammatory diseases such as river blindness and lymphatic filariasis. Using a murine model for river blindness in which soluble extracts of filarial nematodes were injected into the corneal stroma, we demonstrated that the predominant inflammatory response in the cornea was due to species of endosymbiotic Wolbachia bacteria. In addition, the inflammatory response induced by these bacteria was dependent on expression of functional Toll-like receptor 4 (TLR4) on host cells.

Blindness and severe visual impairment in facial fractures: an 11 year review.

We present an 11 year review of facial fractures treated in the Australian Craniofacial Unit and the Department of Plastic and Reconstructive Surgery at the Royal Adelaide Hospital, specifically looking at those fractures in adults that resulted in blindness or severe visual impairment. During the period 1989-2000, a total of 2516 patients with facial fractures were treated operatively. From our facial-fracture database, 317 patients were identified as having an ophthalmological complication on presentation. Of these, 19 had severe visual impairment or blindness. The exact fracture pattern and…

Is V1 necessary for conscious vision in areas of relative cortical blindness?

Visual field defects result from postgeniculate lesions. It is generally assumed that absolute defects are caused by total destruction or denervation of primary visual cortex (V1) and that the degraded but conscious vision that remains or returns in relative or partial defects is mediated by compromised V1 cortex that retains a sufficiently large population of functional neurons. We here report the results of three patients with long-standing postgeniculate lesions who underwent functional magnetic resonance imaging while their partial defect was stimulated with high-contrast reversing checker

Regional variation in blindness in children due to microphthalmos, anophthalmos and coloboma.

BACKGROUND: The prevalence and causes of blindness in children vary widely between regions. Few epidemiological data are available on the relative importance of the major congenital anomalies of the globe (i.e., microphthalmos, anophthalmos, coloboma) as causes of blindness in children. The aim of this study was to determine the re-gional variation in the proportion of severe visual impairment and blindness due to congenital abnormalities of the globe in children in schools for the blind and in those identified through Community Based Rehabilitation programs. Other objectives were to estimate …

Complete congenital stationary night blindness maps on Xp11.4 in a Sardinian family.

X-linked congenital stationary night blindness (CSNBX) is a hereditary non-progressive retinal disorder, which can appear in two different clinical forms, complete and incomplete, associated with CSNB1 and CSNB2 loci on Xp. We describe a Sardinian family with complete CSNBX and define better the limits of the CSNB1 genetic locus on Xp11.4 through linkage analysis. Haplotype analysis showed two key recombinants, which restrict the CSNB1 locus to a region of about 3 cM limited by markers DSX1068 and DSX6810 respectively. The locus that we describe is included in the CSNB1 locus defined by previo…

Childhood MELAS syndrome presenting with seizure and cortical blindness: a case report.

An 11-year-old boy presented with seizure and cortical blindness. A T1 weighted magnetic resonance image of the brain showed high signal intensity in the bilateral corpus striatum and long T1 and T2 changes in the bilateral occipital and cerebellar hemispheric regions. Increased cerebrospinal fluid lactate concentration of 56.7 mg/dl and blood lactate concentration of 34.2 mg/dl were also noted. A muscle biopsy obtained from the quadriceps femoris muscle showed the presence of ragged red fibers and mitochondrial DNA (mtDNA) analysis showed an A-->G mutation at nucleotide position 3243. MtDNA a…

[Acute reversible night blindness caused by vitamin A deficiency in pancreatic carcinoma].

BACKGROUND: Vitamin A deficiency is a leading cause of blindness in childhood in the developing countries. In developed countries xerophthalmia occurs mainly as secondary hypovitaminosis due to malabsorption in chronic alcoholism or intestinal disorders. PATIENT AND METHODS: A 24-year-old female patient complained of night blindness since one week. There was a history of pancreas tail resection combined with radiation due to a papillary low grade cystic carcinoma at the age of 15. During a review period of 9 years of follow-up there was a slow disseminated metastatic spread including the live…

Bilateral striate cortex contusion presenting with cerebral asthenopia rather than cortical blindness.

We report a 22-year-old female with a history of contusion of bilateral occipital lobes at the age of 1 year. Magnetic resonance imaging confirmed severe lesion in bilateral striate cortices which should have resulted in cortical blindness. However, she displayed a fairly good visual function presenting only cerebral asthenopia and the restriction of the visual field. A positron emission tomography revealed that no areas other than the primary visual cortices responded to visual stimuli. Thus, her residual vision may be attributable to plasticity of the visual cortex in humans.
