Preclinical vCJD after blood transfusion in a PRNP codon 129 heterozygous patient.

We report a case of preclinical variant Creutzfeldt-Jakob disease (vCJD) in a patient who died from a non-neurological disorder 5 years after receiving a blood transfusion from a donor who subsequently developed vCJD. Protease-resistant prion protein (PrP(res)) was detected by western blot, paraffin-embedded tissue blot, and immunohistochemistry in the spleen, but not in the brain. Immunohistochemistry for prion protein was also positive in a cervical lymph node. The patient was a heterozygote at codon 129 of PRNP, suggesting that susceptibility to vCJD infection is not confined to the methion…

New-variant Creutzfeldt-Jakob disease: the risk of transmission by blood transfusion.

New-variant Creutzfeldt-Jakob disease (nvCJD) was first described in the UK in 1996 and is thought to be related to the consumption of cattle suffering from bovine spongiform encephalopathy. Although only 29 cases have been confirmed to date, it is too early to predict the number of people who may currently be incubating the disease. Past experience suggests that sporadic CJD is rarely, if ever, spread by blood transfusion. However, it is unclear whether nvCJD may be transmissible by this route and if so, how easily. Assessing the potential risk of transmission of nvCJD by blood transfusion an…

Elective surgery in children with sickle cell disease without preoperative blood transfusion.

It is generally recommended that patients with sickle cell disease receive red blood cell (RBC) transfusions before undergoing general anesthesia and surgery. Since RBC transfusions are costly, inconvenient, and may cause serious complications, it might be useful to identify groups of patients for whom they are not absolutely necessary. We report our experience with 54 pediatric patients undergoing 66 elective surgical procedures without preoperative transfusion preparation. All patients were felt to be clinically and hematologically stable in the immediate preoperative period. For the majorit

Blood Transfusion Induced Opportunistic Adult T Cell Leukaemia/Lymphoma after Hodgkin's Disease.

A patient previously treated for Hodgkin's disease (HD) developed secondary adult T cell leukaemia/lymphoma (ATL) after blood transfusion. Immunohistochemical analysis and polymerase chain reaction support the diagnosis. To the best of our knowledge this is the first occurrence of transfusion induced ATL occurring as a second malignancy after treatment for HD. The leukaemia/lymphoma probably developed on the basis of underlying immunosuppression.

[Autologous blood transfusion in the complex surgical treatment of chronic hematogenic osteomyelitis].

The article analyzes results of complex surgical treatment of 109 patients with chronic hematogenic osteomyelitis of long tubular bones and pelvis, 53 of them were subjected to correction of intraoperative blood loss with autogenic blood. Autohemotransfusion with procurement of 400-600 ml of blood immediately before operation in combination with normovolemic hemodilution allows to correct intraoperative blood loss up to 20% of the circulating blood volume. Autohemotransfusion with the preliminary taking of blood as much as 800 ml allows to correct intraoperative blood loss more than for 20% of

Successful ultrasound-guided intrauterine blood transfusion in severe non-immune hydrops fetalis.

Non-immune hydrops fetalis has varied etiology, high mortality and few known successful treatment modalities. A rare case of non-immune fetal hydrops with severe anemia diagnosed prenatally and treated successfully by repeated ultrasound-guided intrauterine blood transfusions is presented.

Plasma atrial natriuretic peptide in human fetus: response to intravascular blood transfusion.

To investigate whether atrial natriuretic peptide is present in the human fetal circulation and changes in response to fetal blood volume expansion, the concentration of plasma atrial natriuretic peptide was measured by radioimmunoassay in samples obtained by cordocentesis. Twenty-four patients referred for intravascular blood transfusion because of red cell isoimmunization at 21 to 35 weeks' gestation were studied. Plasma atrial natriuretic peptide concentrations in fetal blood (median, 8.0 pmol/L; range, 1 to 27.3) were significantly higher than those of 25 young 18- to 32-year-old adult con…

Human T-cell leukemia virus type I (HTLV-I) and blood transfusion.

Human T-cell leukemia (or T-lymphotropic) virus type I (HTLV-I) is a human exogenous infectious retrovirus of the family Retroviridae. This virus has been associated with adult T-cell leukemia and endemic myelopathies (tropical spastic paraparesis and HTLV-I associated myelopathy). HTLV-I is transmitted by sexual contact, from mother to child, by intravenous drug abuse, and by blood transfusion. The estimated lifetime risk of developing disease in antibody-positive patients is 1 in 80, and a latency period as long as 20 years can intervene. No case of transfusion-transmitted disease has been r…

Oxygen dissociation after transfusion of blood stored in ACD or CPD solution.

In 20 patients undergoing open-heart surgery, 2,3-diphosphoglycerate (2,3-DPG) concentrations, oxygen affinity of hemoglobin (Po2 at half saturation of hemoglobin with oxygen [P50]), hemoglobin concentration, and pH were measured repeatedly. Measurements were made before and at various times after open-heart surgery and replacement of blood loss with blood stored in acid-citrate-dextrose (ACD) or citrate-phosphate-dextrose (CPD) solutions for less than 72 hours (10 cases per group). Infusion of ACD blood caused P50 and 2,3-DPG concentration to decrease significantly after the operation. The in
