Fixation of proximal humeral fractures with the Polarus nail.

Thirty patients with fractures of the proximal humerus were treated by internal fixation with an intramedullary locked nail. Fourteen of these patients were more than 60 years of age; 23 patients had 2-part fractures (surgical neck fractures) and were operated on within an average of 3 days after injury. At follow-up, 20 (80%) of 25 patients showed satisfactory to excellent functional outcomes, as measured by Constant score. All but 1 fracture progressed to clinical and radiologic union. The good functional results indicate that the Polarus nail is a satisfactory mechanical device that can be

Lengthening of the tibia over an intramedullary nail, using the Ilizarov external fixator. Major complications and slow consolidation in 9 lengthenings.

We lengthened 9 tibial segments over a nail to reduce the time in the external fixator in 5 patients with constitutional shortness. The median lengthening was 7 (5.5-7.3) cm and the external Ilizarov frame was removed after median 99 (63-125) days. In spite of a short time in the external fixator, consolidation was slow, with a median lengthening index of 4.4 (2.4-6.1) months/cm. The procedure resulted in 3 fatigue fractures of the intramedullary nail or interlocking screws that needed revision and bone grafting. In 1 patient, a deep intramedullary infection occurred. After the experience of t

Finite element analysis of the modified intramedullary nail-II for managing reverse obliquity trochanteric fractures.

Cephalomedullary nails are widely used to fix unstable femoral trochanteric fractures nowadays. However, for reverse obliquity trochanteric (ROT) fractures, the fixation failure rate of existing cephalomedullary nails is high, resulting in many complications. Our team aimed to propose the modified intramedullary nail-II (MIN-II) to improve the fixation effects of ROT fractures and make biomechanical comparisons between MIN-II and three other cephalomedullary nails by finite element method. AO/OTA 31-A3.1 and 31-A3.3 ROT fracture models were established via a series of femoral CT data. Four cep

Minocycline-induced black bone disease encountered during total knee arthroplasty.

Finding discolored bone intraoperatively can be confusing and concerning to orthopedic surgeons. Multiple causes of pigmented bone exist, including ochronosis, metabolic bone diseases, metal deposits, sequestrum, metastatic disease, and minocycline use. Bone quality is an important consideration in intraoperative decision making with respect to components and fixation options in total joint arthroplasty. Abnormal bone encountered in routine arthroplasty can raise concerns over the integrity and healing potential of the bone when the etiology is uncertain.Minocycline is a drug routinely used fo

Bone Diaphysis Metastases, the Ways and Results of Surgical Treatment Saving the Joints.

BACKGROUND: A consequence of the progress in oncological treatment is an increasing number of bone complications asso-ciated with metastases. With appropriate choice of oncological treatment and appropriate surgical management, patients may recover their physical function and maintain the previous level of quality of life. Available surgical techniques include intrame-dullary nailing, stabilisation with plates and screws and the use of modular prostheses. AIM OF THE STUDY: to retrospectively assess the techniques and outcomes of surgical treatment of long bone metastases at the Oncology Ortho

Effects of different reduction patterns on stress distribution in patients with intertrochanteric fractures with intramedullary nail fixation: a finite element analysis.

OBJECTIVE: Positive medial cortical support is a reliable fracture reduction pattern, but existing research on its use is mainly qualitative. We conducted this finite element analysis study to quantitatively evaluate the usefulness of positive cortical support for intertrochanteric fracture reduction. METHODS: Twenty-five models of intramedullary nail fixation for AO type 31-A1.2 intertrochanteric fractures subjected to different reduction patterns were established. The peak von Mises stress at the femoral fracture surface, proximal intersection of the intramedullary nail, and distal intersec

Risk factors for poorly controlled and recurrence-prone ingrown toenails treated with nail braces: A retrospective observational study of 238 cases.

Background Nail braces are reportedly effective for treating both acute inflamed and chronic dystrophic type ingrown toenails. Aims In this study, risk factors for poorly controlled and recurrence-prone ingrown toenails treated with nail braces were identified. Methods We performed a retrospective study on patients with ingrown toenails between June 1, 2015, and May 31, 2018. The last follow-up date was January 31, 2019. Multivariate logistic regression was performed to evaluate the possible factors associated with poorly controlled status (ongoing paronychia during treatment) and recurrence.

Outcomes of closed interlocking intramedullary nailing of the tibia without fluoroscopy in resource-limited settings: experience from two hospitals in Cameroon.

INTRODUCTION: closed static interlocking nailing with c-arm guidance is the standard procedure for the treatment of closed diaphyseal leg fractures. It is still very difficult to carry out such procedures in a low-income setting because of few or absent image intensifiers (c-arm) despite the necessity. The aim of this study was to describe the outcomes of patients with tibial fractures treated with closed interlocking intramedullary nails without c-arm guidance in a Cameroonian population. METHODS: this was a prospective study including adult patients treated for tibial fractures without a c-

Surgical management of angulated femoral intramedullary nails associated with closed fractures: A systematic review of the literature.

BACKGROUND: Femoral nail bending is a rare complication of intramedullary (IM) fixation of femoral diaphyseal fractures. Published literature regarding this injury pattern has thus far been limited to case reports or case series, thus no universally accepted surgical treatment strategy has been developed. METHODS: A systematic review was conducted using the Pubmed/MEDLINE and Scopus/EMBASE databases. A standardized template was used to extract data including author, year of publication, patient demographics, degree of angulation, mechanism of injury, time since initial procedure to reinjury,

Retrograde locked nailing of pubic ramus fractures: a technical note.

Percutaneous screwing of the pubic ramus presents several limitations, such as the infeasibility within a bone corridor that is too narrow or too curved, or the lack of proximal locking, which may result in the mobilization of implants. This technical note presents an original method for percutaneous fixation of pubic ramus fractures using retrograde locked nailing. The implant used is the fibula rod system (Acumed), a proximally locking fibula nail. Four patients underwent surgery for pelvic ring fractures with anterior fixation using this technique, allowing immediate weight-bearing. All the

Anterior clamp-assisted reduction for retrograde nailing of infra-isthmic femoral fractures: technique and preliminary results on a short case series.

Infra-isthmal femoral fractures in elderly patients are difficult to stabilize due to the broad metaphyseal anatomy. In the absence of consensus on optimal management, this study assesses a minimally invasive technique combining anterior clamp-assisted reduction with retrograde intramedullary nailing. A retrospective case series was conducted on ten patients treated between January 2023 and April 2024. All underwent percutaneous fracture reduction via a limited anterior approach, followed by retrograde nailing. The mean patient age was 84.9 ± 6.2 years. All fractures were caused by low-energy

Fat embolism: the reaming controversy.

Intramedullary nailing is the preferred treatment method for stabilizing femoral diaphyseal fractures. Despite its superior biomechanical advantages over other implants, its use, particularly in selected groups of patients, has been questioned because of the possible harmful systemic effects of intramedullary reaming. The increase in intramedullary canal pressure during intramedullary nailing can result in intravasation of bone marrow and fat into the venous blood system. The subsequent consequences can be fat embolism syndrome (FES), adult respiratory distress syndrome (ARDS), and multiple or

The graft-reposition-on-flap technique for fingertip amputations: a patient-reported outcomes study of 111 patients.

PURPOSE: Graft-reposition-on-flap (GRF) is a reconstruction method described for non-replantable fingertip amputations. We hypothesised that this technique provides acceptable outcomes for such an injury, as measured by a patient-reported survey. METHODS: This retrospective study evaluated 111 patients [118 digits], operated between January 2019 and March 2023 for Allen 3 or 4 fingertip amputations. All patients had crush avulsion amputations not fit for replantation and had undergone flap cover with grafting of the amputated nail bed and bone. 80 digits had undergone a flap with bone and nai

Interlocking Nailing Versus Plating in Tibial Shaft Fractures in Adults: A Comparative Study.

INTRODUCTION: Tibial diaphyseal fractures are the commonest long bone fractures in adults, most commonly managed by intramedullary interlocking nailing. However, several meta-analysis show that locking plate osteosynthesis is equally effective in managing tibial diaphyseal fractures and are associated with less number of complications. AIM: To compare the results of fixation of tibial fractures following plating and nailing in terms of union, patient satisfaction and complications. MATERIALS AND METHODS: A hospital based non randomized clinical trial was performed from September 2013 to Augu

Management outcome of closed femoral shaft fractures by open Surgical Implant Generation Network (SIGN) interlocking nails.

BACKGROUND: Many hospitals in our country lack surgical expertise and operating room facilities like image intensifier and fractures table to carry out closed interlocking nails (ILN) in femoral shaft fractures. But availability of Surgical Implant Generation Network (SIGN) interlocking nails & nailing instrumentation have made open ILN of these fractures possible at very basic level of our health care system. We have carried out open SIGN nailing in patients with closed fractures of shaft femur without the use of image intensifier. Result for fracture union was evaluated both clinically and r

The role of Poller screws in intramedullary nailing for lower limb extra-isthmic fractures.

INTRODUCTION: Intramedullary nailing is the preferred treatment for lower limb long bone fractures, but stability is reduced in extra-isthmic fractures due to nail-metaphysis mismatch. Poller screws may help, but their use is controversial. This study evaluates whether Poller screws are associated with improved prognosis in intramedullary nailing for lower limb extra-isthmic fractures and explores their mechanical mechanisms. METHODS: PubMed, EMBASE, Cochrane Library, and Web of Science were searched up to December 2025 using keywords like 'Fracture,' 'Intramedullary nail,' 'Poller screw,' an

Physeal-sparing unreamed locked intramedullary nailing for adolescent tibial fractures.

INTRODUCTION: The first-line treatment of paediatric tibial fractures is non-operative but the number of operatively treated patients is rising. Elastic intramedullary nailing and external fixation are widely used in children while solid intramedullary nailing, the standard procedure in adults, is usually not recommended due to the open physes. MATERIALS AND METHODS: Between January 2007 and October 2017, unreamed locked solid intramedullary nailing was used in 16 cases of adolescent (mean age 13.7 ± 1.25 years) tibial diaphyseal fractures with open physes. An atypical, physeal-sparing method
