Atrophic Anterior Mandible Treated With Sandwich Osteotomy Without Mini-Screws and Mini-Plates: A Case Report With 7 Years of Follow-Up.

Restoring the anterior mandible may be challenging due to both insufficient height and width of the edentulous alveolar ridge; thus, this case report aimed to treat anterior mandibular atrophy by using the inlay technique without the use of mini-screws or mini-plates to stabilize the augmented bone fragments. A 19-year-old patient who lost his anterior teeth in an accident was treated with a horizontal osteotomy performed 4 mm from the alveolar ridge, with two oblique cuts made using an ultrasonic instrument, and the final phase of the osteotomy was performed with a lever for dental extraction

Reasons for Removal of Mandibular Reconstruction Plates.

BACKGROUND: Mandibular reconstruction plates (MRPs), used alone or alongside bone reconstruction, are now the standard approach for treating mandibular continuity defects. Advancements in plate design and materials have improved the success rate of mandibular reconstruction. Nonetheless, plate-related complications remain prevalent and may, in some cases, result in significant morbidity for patients. OBJECTIVE: To determine complications associated with MRPs and analyse parameters related to the complications. METHODS: A descriptive cross-sectional retrospective study was undertaken over a 5

The Interobserver Reliability of Grading of Distal Radius Volar Plate Prominence.

PURPOSE: The 3-category rating of volar plate prominence in relation to the most volar edge of the distal radius (the watershed line) on lateral radiographs was reliable among a small group of surgeons and associated with the probability of flexor tendon irritation and potential rupture. Classifications are often less reliable when tested among a large group of practicing surgeons in different environments. METHODS: In this survey-based experiment, an international group of 115 fracture and upper extremity surgeons viewed 1 of 4 sets of 24 lateral radiographs (96 unique lateral radiographs) o

Increased rate of pseudarthrosis in the anterior intersegmental gap after mandibular reconstruction with fibula free flaps: a volumetric analysis.

OBJECTIVES: Pseudarthrosis after mandibular reconstruction leads to chronic overload of the osteosynthesis and impedes dental rehabilitation. This study evaluates the impact of gap site on osseous union in mandible reconstruction using a new volumetric analysis method with repeated cone-beam computed tomography (CBCT). METHODS: The degree of bone regeneration was evaluated in 16 patients after mandible reconstruction with a fibula free flap and patient-specific reconstruction plates. Percentual bone volume and width changes in intersegmental gaps were retrospectively analyzed using a baseline

Construct stability in anterior open wedge osteotomy: Assessing 3D-printed polylactic acid wedges against cortical bone.

BACKGROUND: This in-vitro biomechanical study aimed to assess whether using a wedge as a gap filler in anterior open wedge osteotomy of the proximal tibia reduces load on the screw-plate construct and to evaluate if a 3D-printed polylactic acid wedge offers similar biomechanical stability like a bone wedge. METHODS: Twenty-four cadaveric proximal tibiae were randomly assigned to one of three groups: TomoFix plate alone as control group, TomoFix plate with a bone wedge, and TomoFix plate with a 3D-printed polylactic acid wedge. The specimens were subjected to axial loads of 400 N, 800 N, and 1

Diaphyseal long bone nonunions - types, aetiology, economics, and treatment recommendations.

The intention of the current article is to review the epidemiology with related socioeconomic costs, pathophysiology, and treatment options for diaphyseal long bone delayed unions and nonunions. Diaphyseal nonunions in the tibia and in the femur are estimated to occur 4.6-8% after modern intramedullary nailing of closed fractures with an even much higher risk in open fractures. There is a high socioeconomic burden for long bone nonunions mainly driven by indirect costs, such as productivity losses due to long treatment duration. The classic classification of Weber and Cech of the 1970s is base

Post-traumatic caspase-3 expression in the adjacent areas of growth plate injury site: a morphological study.

The epiphyseal plate is a hyaline cartilage plate that sits between the diaphysis and the epiphysis. The objective of this study was to determine the impact of an injury in the growth plate chondrocytes through the study of histological morphology, immunohistochemistry, histomorphometry and Western Blot analyses of the caspase-3 and cleaved PARP-1, and levels of the inflammatory cytokines, Interleukin-6 (IL-6) and Tumor Necrosis Factor alpha (TNF-α), in order to acquire more information about post-injury reactions of physeal cell turnover. In our results, morphological analysis showed that in

Postoperative radiotherapy delineation after mandibular bone flap reconstruction in oral cavity and oropharyngeal cancers.

BACKGROUND: Postoperative radiotherapy is a cornerstone of management for locally advanced oral cavity and oropharyngeal cancers. Increasing use of vascularised mandibular bone flaps and patient-specific fixation systems has substantially modified postoperative anatomy, thereby complicating target volume delineation. While consensus guidelines exist for soft tissue flaps, no practical framework specifically addresses mandibular bone flap characterisation and delineation in poRT planning. We aimed to provide multidisciplinary, imaging-based recommendations for bone flap delineation on planning

Thoracolumbar spine stabilization in fracture--dislocations.

This study analyzed a series of 35 patients with unstable fractures, with and without neurologic deficit including complete paraplegia. The patients underwent spinal stabilisation using plates and screws after sustaining fracture dislocations. Very good results were obtained in 19 patients with non-complicated fractures operated on by dorsal stabilization using Meurig-Williams plates. Complete recovery of the neurologic deficit was achieved in five out of 13 patients with neurologic deficit two of whom were paraplegic. Use of stabilization operation accompanied by dorsal vertebrodesis and post

Sleeper Plate for Guided Growth: Does the Plate Material Change the Risk of Tethering?

BACKGROUND: Pediatric coronal plane deformities are commonly managed by guiding growth by placing an extraperiosteal 2-hole plate across the growth plate with 1 epiphyseal and 1 metaphyseal screw. Once the correction is achieved, removing the metaphyseal screw only (sleeper plate) has been described to facilitate easier future corrections if deformity rebounds. A complication commonly described with this approach is tethering, where the patient continues to overcorrect despite the elimination of tension across the physis. This study aims to evaluate the safety and efficacy of sleeper plates an

Juvenile kyphosis: histological and histochemical studies.

UNLABELLED: Histological and histochemical studies of the spine of a sixteen-year-old boy with juvenile kyphosis who was killed in an automobile accident showed abnormal cartilage in extensive areas of the vertebral and growth plates of the involved vertebrae. In these areas the cartilage matrix was loose, strongly positive when stained with alcian blue and weakly positive to periodic acid-Schiff, and it contained numerous chondrocytes. Some chondrocytes were irregularly shaped and others were in clusters. Vertebral bone growth was stunted under the areas of abnormal vertebral and growth plate

ER Stress During the Pubertal Growth Spurt Results in Impaired Long-Bone Growth in Chondrocyte-Specific ERp57 Knockout Mice.

Long-bone growth by endochondral ossification is cooperatively accomplished by chondrocyte proliferation, hypertrophic differentiation, and appropriate secretion of collagens, glycoproteins, and proteoglycans into the extracellular matrix (ECM). Before folding and entering the secretory pathway, ECM macromolecules in general are subject to extensive posttranslational modification, orchestrated by chaperone complexes in the endoplasmic reticulum (ER). ERp57 is a member of the protein disulfide isomerase (PDI) family and facilitates correct folding of newly synthesized glycoproteins by rearrange

Guided ultrasonic waves in long bones: modelling, experiment and in vivo application.

Existing ultrasound devices for assessing the human tibia are based on detecting the first arriving signal, corresponding to a wave propagating at, or close to, the bulk longitudinal velocity in bone. However, human long bones are effectively irregular hollow tubes and should theoretically support the propagation of more complex guided modes similar to Lamb waves in plates. Guided waves are attractive because they propagate throughout the bone thickness and can potentially yield more information on bone material properties and architecture. In this study, Lamb wave theory and numerical simulat

Clinical outcomes of treatment with locking compression plates for distal femoral fractures in a retrospective cohort.

BACKGROUND: Plate fixation is one of the standard surgical treatments for distal femoral fractures. There are few reports on the relationship between the screw position and bone union when fixing by the bridging plate (relative stability) method. METHODS: This retrospective study included 71 distal femoral fractures of 70 patients who were treated with the locking compression plate for distal femur (DePuy Synthes Co., Ltd, New Brunswick, CA, USA). The following measurements were evaluated and analyzed: (1) bone union rate, (2) bridge span length (distance between screws across the fracture),

3D full-field strain in bone-implant and bone-tooth constructs and their morphological influential factors.

The biomechanics of bone-tooth and bone-implant interfaces affects the outcomes of several dental treatments, such as implant placement, because bone, tooth and periodontal ligament are living tissues that adapt to the changes in mechanical stimulations. In this work, mechanical testing coupled with micro-CT was performed on human cadaveric mandibular bone-tooth and bone-implant constructs. Using digital volume correlation, the 3D full-field strain in bone under implant loading and tooth loading was measured. Concurrently, bone morphology and bone-implant and bone-tooth contact were also measu

Age-related histological changes in calcified cartilage and subchondral bone in femoral heads from healthy humans.

OBJECTIVE: Age is the most important risk factor for osteoarthritis (OA). It is suggested that changes in subchondral bone and calcified cartilage may occur in early OA. Therefore, the aim was to investigate age-related changes in the femoral head composition. We hypothesise that the thickness of the subchondral bone plate decreases with age, while the thickness of the calcified cartilage increases with age as seen in early-stage OA. METHODS: Femoral heads from 29 women (20-74 years) and 32 men (23-78 years), who had died suddenly and unexpectedly, were obtained at autopsy. Individuals with b
