Modified Angulated Innominate Osteotomy Using Cortical Screw Fixation for Developmental Dysplasia of the Hip: A Technical Note with a Case Series.

INTRODUCTION: Salter innominate osteotomy (SIO) is a well-established surgical procedure for the treatment of developmental dysplasia of the hip (DDH). Despite generally favorable outcomes, loss of correction before bone union remains a clinically relevant concern, particularly when fixation strength is insufficient. Angulated innominate osteotomy without bone grafting has been introduced to improve stability and reduce surgical invasiveness; however, fixation strategies for this technique have largely remained unchanged and continue to rely on Kirschner wires (K-wires), which may provide limi

What Is the Optimal Method of Fixation for Scaphoid Nonunion Relative to the Location of the Lesion?

INTRODUCTION: Scaphoid nonunion is a typical complication of scaphoid fractures, with the nonunion rate varying by the location of the scaphoid. The current widely used method for treating scaphoid nonunion, which is challenging for surgeons, is the headless compression screw (HCS). Various surgical approaches, such as the scaphoid plate, have been proposed to address the problem of screw fixation; however, no consensus exists regarding the optimal treatment method. This study focused on analyzing appropriate treatment methods based on the anatomical location of the scaphoid nonunion. METHODS

Strain reduction screws for nonunions following fixation around the elbow - A case series and review of the literature.

BACKGROUND: Nonunions following fracture fixation result in significant patient morbidity and financial burden. Traditional operative management around the elbow consists of removal of metalwork, debridement of the nonunion and re-fixation with compression, often with bone grafting. Recently, some authors in the lower limb literature have described a minimally invasive technique used for select nonunions where simply placing screws across the nonunion facilitates healing by reducing inter-fragmentary strain. To our knowledge, this has not been described around the elbow, where traditional more

What is the best position for coracoid fixation in the Latarjet procedure?

HYPOTHESIS: Multiple problems and complications associated with Latarjet fixation have been described; thus, this is the first study in the literature to identify the maximum allowed screw clamping force and best fixation screw position for Latarjet surgery. METHODS: A variation of distal and proximal coracoid screw positions with and without a flat washer was evaluated through finite element analysis, at a minimum distance of 3 mm from the edge. A loading progression test was performed until the maximum stress reached a limit imposed by the bone yield. We identified the maximum allowed screw

Biomechanical design optimization of distal femur locked plates: A review.

Clinical findings, manufacturer instructions, and surgeon's preferences often dictate the implantation of distal femur locked plates (DFLPs), but healing problems and implant failures still persist. Also, most biomechanical researchers compare a particular DFLP configuration to implants like plates and nails. However, this begs the question: Is this specific DFLP configuration biomechanically optimal to encourage early callus formation, reduce bone and implant failure, and minimize bone "stress shielding"? Consequently, it is crucial to optimize, or characterize, the biomechanical performance

The Vascular Anatomy of the Scaphoid: New Discoveries Using Micro-Computed Tomography Imaging.

PURPOSE: The purpose of this study was to investigate the intraosseous vascular anatomy of the scaphoid using recent advances in micro-computed tomography (micro-CT) imaging and 3-dimensional reconstruction. We also studied the effect of scaphoid shape and screw position on the intraosseous vascular structure. METHODS: Thirteen upper extremities were injected with a contrast agent. The scaphoid bones were extracted and scanned using a micro-CT scanner. The vascular impact of screw insertion at various axes through the scaphoid was calculated and compared using the generated 3-dimensional mode

Clinical and Radiographic Outcomes of Customized Titanium Mesh vs. Screw Tent-Pole Grafting: A Retrospective Study.

BACKGROUND: Guided bone regeneration (GBR) is a predictable approach for managing severe alveolar ridge deficiencies prior to implant placement. Resorbable collagen membranes supported by tenting screws are widely used, although space maintenance in non-contained defects may be challenging. Customized CAD/CAM titanium meshes have been introduced to enhance graft stability and surgical workflow, but comparative clinical evidence remains limited. OBJECTIVE: To compare clinical, radiographic, procedural, and peri-implant outcomes of customized CAD/CAM titanium meshes versus resorbable collagen m

Patellar Fracture After ACL Reconstruction: Open Reduction and Internal Fixation with a Maxillofacial Plate: A Case Report.

CASE: Patellar fractures after anterior cruciate ligament (ACL) reconstruction with a bone-patellar-tendon-bone (BPTB) autograft are a rare complication with a reported incidence of 0.2% to 2.3%. Treatment has previously been nonoperative splinting, lag screws, or a tension-band construct. We present the case of a 14-year-old adolescent girl who suffered a comminuted patella fracture 4 weeks after an ACL reconstruction using a BPTB autograft who subsequently underwent successful operative fixation through a novel technique with the use of a maxillofacial plate and screw system. CONCLUSION: A

A hybrid treatment modality of a subtrochanteric femoral fracture in a patient with osteoporosis due to a renal Fanconi syndrome: a case report.

A 24-year-old male with an idiopathic renal Fanconi syndrome presented to our ER after a low-energetic fall. Conventional imaging revealed a right subtrochanteric femoral fracture, severely decreased bone quality and cannulated collum femoris screws on the contralateral side. Regular plate-screw osteosynthesis or cephalomedullary implantation was deemed insufficient, due to a high iatrogenic and periprosthetic fracture probability. The decision was made to perform a plate-screw osteosynthesis combined with an intramedullary polymer bone enhancement (IlluminOss), to minimize this risk. No compl

Dorsal Plating of Unstable Scaphoid Fractures and Nonunions.

Achieving stable fixation of displaced acute and chronic nonunited scaphoid fractures continues to be a challenge for the treating surgeon. The threaded compression screw has been the mainstay of treatment of these fractures for the last 3 decades; however, persistent nonunion after screw fixation has prompted development of new techniques. Recent results of volar buttress plating have been promising. We describe a novel technique of dorsal scaphoid plating. In contrast to volar plating, the dorsal plate is biomechanically more favorable as it utilizes the tension side of the scaphoid bone for

Bilateral Scaphocapitate Fracture Syndrome: A Case Report with Long-Term Follow up.

Carpal bone fracture-dislocation is an uncommon wrist injury. Regarding limited prevalence of such injury, most of orthopedic surgeons have low experience in treatment and handling of these fractures and dislocations. A 25-year-old male worker with an uncommon carpal bone fracture-dislocation, namely bilateral scaphocapitate fracture syndrome, was described. Volar and dorsal approaches were used for reduction and fixation; complete stabilization was achieved after open reduction and fixation using Herbert screws into scaphoid and capitate. A 5-year follow-up showed satisfactory functional and

Cement Augmentation in Sacroiliac Screw Fixation Offers Modest Biomechanical Advantages in a Cadaver Model.

BACKGROUND: Sacroiliac screw fixation in elderly patients with pelvic fractures is prone to failure owing to impaired bone quality. Cement augmentation has been proposed as a possible solution, because in other anatomic areas this has been shown to reduce screw loosening. However, to our knowledge, this has not been evaluated for sacroiliac screws. QUESTIONS/PURPOSES: We investigated the potential biomechanical benefit of cement augmentation of sacroiliac screw fixation in a cadaver model of osteoporotic bone, specifically with respect to screw loosening, construct survival, and fracture-site

A Systematic Review of Surgical Techniques for the Repair of Capitellar Fractures.

The rarity of capitellum fractures makes them a thorny problem in orthopedic practice, as fracture reduction and repair to restore joint function within a complex elbow joint are difficult. Properly treated, these fractures can avoid complications such as stiffness, instability, and posttraumatic arthritis. Several surgical techniques optimize patient outcomes, including open reduction with internal fixation using Herbert screws, buttress plating, and headless compression screws (HCS). The choice of technique, however, is determined by many factors, including fracture type, patient characteris

Biocompatibility and functionality of the degradable polymer alkylene bis(dilactoyl)-methacrylate for screw augmentation in vivo.

Recently, a new degradable polymer has been developed on the basis of alkylene bis(dilactoyl)-methacrylate as an alternative material for screw augmentation. The polymer has been investigated in vitro and in a short-term experiment in rabbits exhibiting promising results. The aim of the present study was to investigate its long-term biocompatibility and mechanical functionality in a large animal model. The polymer was used for screw augmentation in the cancellous bone of the femoral condyle and tibia epiphysis of 12 sheep and was compared to polymethylmethacrylate (PMMA) augmented and nonaugme

Review and evolution of occipital fixation devices and techniques.

INTRODUCTION: Occipital-cervical (OC) fixation poses distinctive challenges due to unique craniocervical junction (CCJ) anatomy and a complex range of pathology leading to CCJ instability. There has been substantial evolution in modern OC fixation techniques since their introduction in the 1990s, each with implications on rate of fusion, range of motion, and ease of use. AREAS COVERED: We performed a manual review of OC fixation techniques via a query of the National Library of Medicine MEDLINE database between 10 September and 1 October 2025. Categories of OC fixation included sublaminar wir
