Pediatric organ donation and transplantation.

BACKGROUND AND OBJECTIVES: There is increasing unmet need for solid organ donation. Alternative donor sources, such as donation after circulatory determination of death (DCDD), are needed. The objective of this study was to examine the impact of DCDD on trends in pediatric organ donation and transplantation. METHODS: Data were obtained from the Organ Procurement and Transplantation Network for US organ recipients and donors from 2001 to 2010 stratified according to age, organ, and deceased donor type (DCDD or donation after neurologic determination of death). Additional data included transpla

[Premature craniosynostosis--cause of sudden death in children and young adults].

Primary premature craniosynostosis ordinarily leads to characteristic skull abnormalities, if the ossification of the sutures happens in early childhood and involves all sutures, the elevation of intracranial pressure may not cause severe clinical features and the head morphology may be normal. A disproportion between skull capacity and brain volume will be the result. Three cases are reported, where these preconditions combined with exterior effects like minimal trauma, physical activity or consumption of alcohol or medicaments caused sudden death. Important autoptic findings are ossified cra

Astrocytic Multimodal Programmed Cell Death in Ischemic Stroke: Regulatory Mechanisms and Translational Prospects.

INTRODUCTION: Ischemic stroke remains a leading cause of disability and mortality worldwide. Current neuroprotective treatments predominantly target neurons and fail to address the complex post-ischemic cell death cascades in astrocytes, the most abundant glial cells in the central nervous system, whose multimodal Programmed Cell Death (PCD) directly determines blood-brain barrier integrity, neuroinflammation, and secondary brain injury. METHODS: The literature on astrocytic PCD in ischemic stroke was systematically reviewed before April 2026, encompassing spatiotemporal dynamics, molecular m

Neuroprotective actions of pterostilbene on hypoxic-ischemic brain damage in neonatal rats through upregulation of heme oxygenase-1.

Neonatal hypoxic-ischemic (HI) brain damage causes acute mortality and morbidity in newborns and long-term neurological disorders in the survivors. Pterostilbene (PTE) is a natural compound possessing various biological and pharmacological activities. In the present study, we aimed to investigate the effect of PTE on neonatal HI brain damagein P7 rat model and to explore the possible mechanisms. Neonatal HI brain damage was induced in rat pups (P7). Prior to the induction of HI injury, PTE was injected with or without zinc protoporphyrin IX (ZnPP), an inhibitor of heme oxygenase-1 (HO-1). ZnPP

Pediatric heart transplant in donation after circulatory death using normothermic regional perfusion.

OBJECTIVE: Orthotopic heart transplant is the definitive option for pediatric patients with end-stage heart failure. Unfortunately, the greatest contributor to waitlist mortality has been a shortage of available hearts for transplant. Donation after circulatory death with normothermic regional perfusion may mitigate this supply-demand mismatch. METHODS: Donation after circulatory death with normothermic regional perfusion recipients were matched to similar donation after brain death recipients. Primary end points included 1-year survival and episodes of primary graft dysfunction at 1 year. Se…

Functional characterization of acid-sensing ion channels in the cerebellum-originating medulloblastoma cell line DAOY and in cerebellar granule neurons.

Acid-sensing ion channels (ASICs) are Na+ channels that are almost ubiquitously expressed in neurons of the brain. Functional ASIC1a is also expressed in glioblastoma stem cells, where it might sense the acidic tumor microenvironment. Prolonged acidosis induces cell death in neurons and reduces tumor sphere formation in glioblastoma via activation of ASIC1a. It is currently unknown whether ASICs are expressed and involved in acid-induced cell death in other types of brain tumors. In this study, we investigated ASICs in medulloblastoma, using two established cell lines, DAOY and UW228, as in vi

Nerve cell death types in the edematous human cerebral cortex.

Cortical biopsies of 18 patients with clinical diagnosis of congenital hydrocephalus, brain trauma, and vascular anomaly were examined with the transmission electron microscope to study the distinct and overlapped morphological cell types of nerve cell death in the human edematous cerebral cortex. The nerve cells showed lobulated and shrunken nucleus, irregular enlargement and fragmentation of perinuclear cistern, with areas of apparently intact nuclear pore complexes alternating with regions of nuclear pore complex disassembly. The nucleolus appears unaltered in moderate edema and with distor

Changes in renal perfusion with brainstem death - revisiting the hypothesis of sympathetically mediated organ hypoperfusion.

Changes in sympathetic outflow, and the associated cardiovascular effects, are well known to occur during refractory intracranial hypertension, uncal herniation and brainstem death. However, sympathetically mediated changes in renal perfusion and the cortical microcirculation, although hypothesised to occur, are not well documented. Using contrast enhanced ultrasound, the presented case demonstrates a dramatic improvement in, previously abnormal, renal blood flow following uncal herniation. These findings potentially illustrate the effect of sympathetic tone on organ perfusion and the microcir

Impact of ex vivo lung perfusion on brain-dead donor lung utilization: The French experience.

Ex vivo lung perfusion (EVLP) is a valuable method for expanding the lung donor pool. Its indications currently differ across centers. This national retrospective cohort study aimed to describe the profile of donors with lungs transplanted after EVLP and determine the effectiveness of EVLP on lung utilization. We included brain-dead donors with at least one lung offered between 2012 and 2019 in France. Lungs transplanted without or after EVLP were compared with those that were rejected. Donor group phenotypes were determined with multiple correspondence analysis (MCA). The association between

Ferroptosis: A Promising Therapeutic Target for Neonatal Hypoxic-Ischemic Brain Injury.

Ferroptosis is a type of programmed cell death caused by phospholipid peroxidation that has been implicated as a mechanism in several diseases resulting from ischemic-reperfusion injury. Most recently, ferroptosis has been identified as a possible key injury mechanism in neonatal hypoxic-ischemic brain injury (HIBI). This review summarizes the current literature regarding the different ferroptotic pathways, how they may be activated after neonatal HIBI, and which current or investigative interventions may attenuate ferroptotic cell death associated with neonatal HIBI.

Cerebral Paragonimiasis Presenting with Sudden Death.

A 58-year-old Korean-born woman with a history of seizures and psychiatric issues was found dead at home. Autopsy was notable for large, calcified nodules that had nearly replaced her right temporal lobe. Histologic examination revealed the presence of Paragonimus eggs. This case demonstrates a rare manifestation of an aberrantly migrated lung fluke that resulted in epilepsy and sudden death years after the initial infection.

[A case of brain dead patient with a spinal pathology of preserved marginal white matter and pencil-shaped softening--a theory for pathogenesis of pencil-shaped softening].

We reported a case of brain death which had been caused by massive cerebral hemorrhage. The spinal pathology showed preserved marginal parts of spinal white matter in the segments of C7 to T5 and S1 to S3, and the other parts showed necrosis. We found pencil-shaped softening (PS)-like lesion in the segment C8 to T2, but the lesion was more preserved than the surrounding tissue. The intraspinal structure of C5 was distorted by the necrotic cerebellar tissue in the subarachnoid space of the segment and the posterior column area was decreased. The posterior column in C5 and PS showed the same sev…

[Cerebral lesions observed in a twin after the in utero death of the other twin. Fetal anoxia-ischemia can be the possible mechanism (3 cases)].

BACKGROUND: The death of one twin in utero may result in visceral lesions, of possible vascular origin, in the surviving twin when the pregnancy is monochorionic and diamniotic. CASES REPORT: Case n. 1: The death of one twin and enlargement of the ventricular system in the other were seen by ultrasonography at 24 weeks of pregnancy, 8 weeks after the mother had a fall on the stairs. The heterogeneous imaging in the inferior part of placenta suggested a clot in this area. The pregnancy was terminated at 28 weeks because the ventricular dilatation continued to increase. The newborn died a few m

Hydrogen alleviates hypoxic-ischaemic brain damage in neonatal rats by inhibiting injury of brain pericytes.

Hypoxia-ischaemia (HI) can induce the death of cerebrovascular constituent cells through oxidative stress. Hydrogen is a powerful antioxidant which can activate the antioxidant system. A hypoxia-ischaemia brain damage (HIBD) model was established in 7-day-old SD rats. Rats were treated with different doses of hydrogen-rich water (HRW), and brain pericyte oxidative stress damage, cerebrovascular function and brain tissue damage were assessed. Meanwhile, in vitro-cultured pericytes were subjected to oxygen-glucose deprivation and treated with different concentrations of HRW. Oxidative injury was

Leucine-rich repeat kinase 2 aggravates secondary brain injury induced by intracerebral hemorrhage in rats by regulating the P38 MAPK/Drosha pathway.

Leucine-rich repeat kinase 2 (LRRK2) is the genetic cause of both familial and idiopathic Parkinson's disease (PD), and it is associated with neuronal death, vesicle trafficking, mitochondrial dysfunction, and inflammation. However, its role in secondary brain injury (SBI) induced by intracerebral hemorrhage (ICH) has not been evaluated. In this study, an ICH model was induced by injecting autologous whole blood into the right basal ganglia of adult rats. Meanwhile, primary rat cortical neurons treated with Oxyhemoglobin (OxyHb) were used as an in vitro ICH model. Protein levels of LRRK2 incre

Cardiogenic oscillation and ventilator autotriggering in brain-dead patients: a case series.

Brain death is manifested by a flaccid, areflexic patient on assessment of brain function with fixed and dilated pupils at midpoint, loss of consciousness, no response to stimulation, loss of brainstem reflexes, and apnea. A lesion or clinical state responsible for the loss of consciousness must be found. An integral part of clinical evaluation of brain death is apnea testing, which indicates complete loss of brainstem function and respiratory drive. Ventilator triggering or overbreathing the ventilator's set rate may be considered consistent with intrinsic respiratory drive consequent to resi…
