Bronchial manifestation of acute febrile neutrophilic dermatosis (Sweet's syndrome).

We describe a 62 yr old female with acute febrile neutrophilic dermatosis (AFND) or Sweet's syndrome, involving the bronchial mucosa. The patient presented with classical skin manifestations of AFND accompanied by bilateral pneumonia, pleural effusions and lesions of the oral mucosa. The diagnosis was confirmed by skin and mucosal biopsies. Bronchoscopy revealed raised pustules, similar to the lesions of the skin, suggesting bronchial involvement of AFND. The patient responded to systemic corticosteroids only for a short period and died as a result of multiorgan failure. Postmortem findings co…

Oncocytic variant of choroid plexus papilloma.

Two cases of oncocytic plexus papilloma are presented which were in the fourth ventricle and cerebello-pontine angle in women aged 70 and 63 years, respectively. The cytoplasm of the transformed tumor cells was enlarged, eosinophilic, and granular; and it was characterized ultrastructurally by innumerable mitochondria. Most of them had lamellar cristae, a clear matrix, and contained dense granules. These 'oncocytes' are identical with oncocytes of the human thyroid, parathyroid, bronchi, kidneys, adrenals, salivary gland and myocardium, and non-neoplastic choroid plexus epithelium. As far as w…

Tracheobronchopathia osteochondroplastica.

6 cases of tracheobronchopathia osteochondroplastica are discussed. Diagnoses were established ante mortem. Endoscopic examination was performed in 3 patients because of hematic expectoration, in the other 3 cases the induction was given by chronic bronchorhea, slow-resolving pneumonia and suspected neoplasia, respectively. Calcification and cartilaginous areas were restricted to a small portion of the tracheobronchial tree in 1 of the cases; in 3 cases the disease affected the trachea and both main bronchi, whereas it was found markedly spread throughout the trachea in the last 2 patients. Tr…

Probable familial congenital bronchiectasis due to cartilage deficiency (Williams-Campbell syndrome).

Two siblings in whom respiratory symptoms developed immediately after birth subsequently were found to have bronchiectasis with strikingly similar distribution of lesions (mainly lower lobes). Inspiratory and expiratory bronchograms performed on one of the siblings demonstrated marked ballooning and collapse of proximal bronchi during tidal breathing. The clinical courses and roentgenographic findings, in the absence of other underlying abnormalities, suggest that the basic disease process was the absence of bronchial cartilage (Williams-Campbell syndrome). This would be the first reported fam…

Primary tooth aspiration during conscious sedation with N2O: foreign body removal with rigid bronchoscopy.

A 7-year-old girl visited the Samsung Medical Center emergency room for primary tooth aspiration during primary tooth extraction under conscious sedation with N2O. The patient showed no signs of respiratory complications. Chest radiography and CT revealed a tooth in the right bronchi. Foreign body removal using rigid bronchoscopy was performed on the day of aspiration. With close monitoring of the airway in the pediatric ICU, extubation was performed the next day, and the patient was discharged the same day. The primary objective of this case report was to highlight the potential risk of aspir…

Recurrent Croup.

Croup refers to airway inflammation and edema leading to obstruction of the larynx, trachea, and bronchi. Croup is the most common cause of acute airway obstruction in young children. It is characterized by the onset of low-grade fever, barky cough, stridor, hoarseness, and respiratory distress. Croup is typically caused by a viral infection (viral croup) but can occur suddenly without a viral prodrome (spasmodic croup). Recurrent croup is defined as two or more episodes per year. Recurrent croup should be considered a symptom of an underlying structural or inflammatory airway abnormality and …

External Stenting for Bronchomalacia Involving the Bronchus Intermedius.

In recent years, external stenting has been used as a reliable method to relieve airway compression of the trachea and main bronchi in young children with acceptable age-proportional airway growth. However, to our knowledge, little literature supporting this approach for the distal airway has been published. A 1-year-old girl with absent pulmonary valve syndrome who had recurrent respiratory infections was diagnosed with bronchomalacia. She underwent external stenting; the infections disappeared postoperatively. This case describes a successful external stenting for malacia of the bronchus int…

Airway management of a patient incidentally diagnosed with Mounier-Kuhn syndrome during general anesthesia.

Mounier-Kuhn syndrome (MKS) is a disease characterized by dilation of the trachea and mainstem bronchi. Due to the risk of airway leakage, pulmonary aspiration, and tracheal damage, MKS can be fatal in patients undergoing tracheal intubation. Moreover, MKS may not be diagnosed preoperatively due to its rarity. In this case, a patient undergoing neurosurgery was incidentally diagnosed with MKS during general anesthesia. During anesthesia induction, difficulties in airway management led the anesthesiologist to suspect MKS. Airway leakage was resolved in this case using oropharyngeal gauze packin…

Pulmonary Sarcoidosis: A Pictorial Review.

Sarcoidosis is a multisystem granulomatous process that most commonly involves the lungs. Radiographic findings consistent with sarcoidosis … imes take atypical forms. Additional manifestations of sarcoidosis are caused by involvement of the bronchi and bronchioles, the pulmonary arteries, and the heart. Fungal colonization may also occur. A range of thoracic imaging manifestations of sarcoidosis is illustrated to facilitate the diagnosis of this common, multifaceted disease.

Pseudomembranous Invasive Tracheobronchial Aspergillosis with Fulminant Hepatitis and Hemophagocytic Syndrome.

Invasive tracheobronchial aspergillosis (ITBA), a rare form of invasive pulmonary Aspergillus infection (IPA), is predominantly confined to the tracheobronchial tree. We herein report a case of ITBA with severe necrotic pseudomembrane in a 57-year-old woman with fulminant hepatitis and hemophagocytic syndrome. Bronchoscopic findings revealed a widespread pseudomembranous formation of the trachea and bronchi. Aspergillus fumigatus was cultured from bronchial lavage fluid, and the histological findings of an endobronchial biopsy revealed necrosis and invasive hyphae. Although she responded to an…

[DIAGNOSIS AND TREATMENT OF BRONCHIECTASIS: POSITION PAPER OF THE ISRAELI PULMONOLOGY SOCIETY AND THE ISRAELI PEDIATRIC PULMONOLOGY SOCIETY].

Bronchiectasis is anatomically defined by irreversible distortion of the bronchi. Clinically, its manifestations are cough with sputum production and a predisposition to pulmonary infections. Unlike asthma and COPD, where ample clinical data are present regarding the course and effective treatment, knowledge of bronchiectasis has yet to evolve. Lately, bronchiectasis is gaining renewed attention among the medical community, with growing basic and clinical research-based data. In Israel, no registered treatments exist for bronchiectasis, which makes it difficult to treat these patients. This pa…

Bioprosthetics and repair of complex aerodigestive defects.

Aerodigestive defects involving the trachea, bronchi and esophagus are a result of prolonged intubation, operative complications, congenital defects, trauma, radiation and neoplastic disease. The vast majority of these defects may be repaired primarily. Rarely, due the size of the defect, underlying complexity, or unfavorable patient characteristics, primary repair is not possible. One alternative to primary repair is bioprosthetic repair. Materials such as acellular dermal matrix and aortic homograft have been used in a variety of applications, including closure of tracheal, bronchial and eso…

Tracheobronchial involvement of mantle cell lymphoma.

Mantle cell lymphoma (MCL) is a rare type of B-cell non-Hodgkin's lymphoma that commonly affects extranodal sites; however, tracheobronchial involvement is rare. We report the case of a 65-year-old male who presented with cough and dyspnoea. A chest computed tomography (CT) revealed irregular wall thickening of the trachea and bilateral bronchi and bilateral bronchiectasis. A bronchoscopy revealed a diffuse irregular surface of the tracheal and bilateral bronchial mucosa and polyposis-like lesions. He was diagnosed with MCL based on an endobronchial biopsy, and then, the diagnosis was confirme…

A Suspected Case of an Alveolar Haemorrhage Caused by Dasatinib.

A 39-year-old man treated with dasatinib for chronic myelogenous leukaemia presented to our hospital with haemoptysis, coughing, and dyspnoea. Chest radiography and computed tomography revealed ground-glass opacities and a crazy-paving pattern. Bronchoalveolar lavage was not performed due to serious hypoxemia and bleeding. Significant bleeding from the peripheral bronchi led to a diagnosis of an alveolar haemorrhage. Dasatinib-induced alveolar haemorrhaging was suspected based on the clinical findings. His condition improved immediately after dasatinib withdrawal and initiation of steroid ther…

Delayed asphyxia due to inhalation injury.

We report the case of a 53-year-old man who was found dead in his apartment 2 days after a little fire during which he had suffered from second to third degree burns at 2% of the body surface, also involving the skin around the mouth and the nose. At autopsy, severe acute emphysema and mucopurulent obstructive laryngotracheobronchitis were observed. The histology showed desquamative loss of the respiratory epithelium up to the middle bronchi; the bronchial lumen was filled with clumps of mucopurulent secretions mixed with necrotic epithelial cells. The cause of death was a delayed asphyxia due…

Management of tracheomediastinal fistula using self-expanding metallic stents.

Tracheomediastinal fistula is a rare condition usually associated with a fatal outcome. We report the case of a 65-year-old man with a subcarinal mass causing total destruction of the carina and proximal mainstem bronchi. The mass was diagnosed as a large cell lymphoma, and the decision was made to undertake chemotherapy only after stabilization of the endobronchial tree. The endobronchial defects were successfully palliated with placement of three different types of self-expanding metallic stents using a flexible bronchoscope under conscious sedation. The unique properties of each stent were …
