Simulated laryngoscopy with supraglottic high-pressure source ventilation: an in vitro study of tracheal airflow to determine optimal positioning of laryngoscope and jet ventilation cannula.

PURPOSE: To investigate how variations in positioning of laryngoscope and location of jet cannula on the laryngoscope body influence tracheal airflow during simulated high-pressure source supraglottic (HPSV) jet ventilation laryngoscopy using an anatomical model. METHODS: A Broncho Boy Bronchoscopy model was modified to allow recording of tracheal airflow. A laryngoscope was suspended and positioned to simulate laryngoscopy. HPSV was delivered by a jet cannula attached to the body of the laryngoscope. Different combinations of laryngoscope angulation and cannula attachment were used and air f…

Use of a tracheal dilator for percutaneous insertion of 27F and 31F Avalon© dual-lumen cannulae for veno-venous extracorporeal membrane oxygenation in adults.

Veno-venous extracorporeal membrane oxygenation can be performed either by two cannulae or by a single dual-lumen cannula. The dual-lumen cannulation configuration offers multiple advantages: it avoids the femoral site which may be at greater risk of infection, it improves patient mobility, eases prone positioning and greatly reduces recirculation. The Avalon was the first commercially available dual-lumen cannula for adults. It has gained much popularity, but, for more than a year now, the adult vascular access kit with the 30Fr dilator has been discontinued in the United Kingdom. In this art…

Evidence Support and Guidelines for Using Heated, Humidified, High-Flow Nasal Cannulae in Neonatology: Oxford Nasal High-Flow Therapy Meeting, 2015.

Nasal high-flow therapy (nHFT) has become a popular form of noninvasive respiratory support in neonatal intensive care units. A meeting held in Oxford, UK, in June 2015 examined the evidence base and proposed a consensus statement. In summary, nHFT is effective for support of preterm infants following extubation. There is growing evidence evaluating its use in the primary treatment of respiratory distress. Further study is needed to assess which clinical conditions are most amenable to nHFT support, the most effective flow rates, and escalation and weaning strategies. Its suitability as first-

High-flow nasal cannulae and nasal continuous positive airway pressure use in non-tertiary special care nurseries in Australia and New Zealand.

AIM: Non-tertiary centres (NTCs) in Australia and New Zealand are increasingly providing non-invasive respiratory support, including high-flow nasal cannulae (HFNC) and nasal continuous positive airway pressure (nCPAP), to newborn infants. We aimed to determine the proportion of NTCs in these countries treating newborn infants with HFNC and nCPAP, and how these therapies are used. METHODS: We surveyed public and private NTCs in Australia and public NTCs in New Zealand. The survey, directed at senior medical and nursing staff, consisted of questions regarding unit demographics, HFNC and nCPAP …

Evaluating the effect of heart and respiratory rate measurement errors on the ability to predict the outcome of high flow nasal cannula therapy: a multi-centre study.

BACKGROUND: The respiratory rate oxygenation (ROX) index, and machine learning (ML) models, are promising approaches to help clinicians identify earlier those patients at risk of failing high flow nasal cannula (HFNC) therapy. Respiratory rate (RR) and heart rate (HR) are key inputs to these models, but their measurement in a hospital environment may be subject to significant errors. The effect of these errors on the accuracy of HFNC outcome predictions is currently unknown. METHODS: We evaluated the capability of a recently-proposed ML model called Tabular Prior-data Fitted Network (TabPFN),…

Effects of Home Nursing and Rehabilitation on Daily Life and Outgoing Activities in a Patient With Interstitial Pneumonia Supported by a High-Flow Nasal Cannula: A Case Report.

A high-flow nasal cannula is used as the primary therapy for patients with respiratory failure. Its effectiveness, however, is contingent upon proper patient monitoring by trained healthcare professionals. The case report highlights the benefits of home nursing and rehabilitation in an 81-year-old woman with rheumatoid-related interstitial pneumonia (complicated by drug-induced interstitial pneumonia) who developed respiratory failure. She lived at home with her family while undergoing high-flow therapy. At the outset of the home visit care, she was bedridden, had an indwelling urinary cathete…

Clinical efficacy and safety of high-flow nasal cannula (HFNC) in acute hypoxaemic patients with COVID-19: a protocol for a systematic review and meta-analysis.

INTRODUCTION: When COVID-19 patients develop hypoxaemic respiratory failure, they often undergo early intubation. Such a potentially aerosol-generating approach places caregivers at increased risk of contracting COVID-19. This protocol aims to evaluate the clinical efficacy and safety of a high-flow nasal cannula (HFNC) for the treatment of COVID-19 patients with acute hypoxaemic respiratory failure. METHODS AND ANALYSIS: We intend to search MEDLINE, Embase, Web of Science and Cochrane Library to identify all randomised controlled trials (RCTs) on the use of HFNC in COVID-19 patients with acu…

Use of High-Flow Nasal Cannula in Patients With Pneumonia and Hypoxemic Respiratory Failure at Altitudes Above 2600 m: What Is the Best Predictor of Success?

Introduction: The use of high-flow nasal cannulas (HFNC) in patients with hypoxemic ventilatory failure reduces the need for mechanical ventilation and does not increase mortality when intubation is promptly applied. The aim of the study is to describe the behavior of HFNC in patients who live at high altitudes, and the performance of predictors of success/failure of this strategy. Methods: Prospective multicenter cohort study, with patients aged over 18 years recruited for 12 months in 2020 to 21. All had a diagnosis of hypoxemic respiratory failure secondary to pneumonia, were admitted to in…

Use of the Arrow percutaneous thrombectomy device and transjugular liver biopsy cannula for mechanical thrombectomy of the inferior vena cava in a patient with Budd-Chiari syndrome.

A 33-year-old woman with Budd-Chiari syndrome and hypercoagulability was sequentially treated with the placement of hepatic vein stents and transjugular intrahepatic portosystemic shunts (TIPS), all of which repeatedly thrombosed. Four months after TIPS revision with an endoprosthesis, a large inferior vena cava (IVC) thrombus developed caudal to an IVC stenosis. A percutaneous thrombectomy device was introduced coaxially through a transjugular liver biopsy cannula to extend its effective diameter range of attack and was steered within the IVC to successfully clear the thrombus. The condition …
