BACKGROUND: It is believed that cancers of the small intestine represent <2% of all malignant tumors of the gastrointestinal tract, although the accuracy of this estimate is unknown, because the current methodologies for examining the small bowel have proved inadequate. We identified three encapsulated Bacillus cereus strains, isolated from patients with severe pneumonia, in a collection of B. cereus isolates associated with human illness. We found that the extent of capsule expression was influenced by culturing conditions. Our findings highlight consequent clinical and laboratory diagnostic challenges posed by such isolates.

BACKGROUND: Celiac disease may be complicated by symptoms that raise the suspicion of small-intestinal malignancy. The objective is to evaluate wireless capsule endoscopy (WCE) in complicated celiac disease.
METHODS: This is a prospective study at a university referral center. There were 47 patients. The indications for WCE were abdominal pain (57%), cancer surveillance (23%), blood in the stool, or persistent iron deficiency (19%).

A 67-year-old diabetic man presented with progressive multifocal myeloradiculopathy for 6 months, with no pulmonary symptoms. A chest x-ray and CT scan of the lungs revealed bilateral multiple nodular infiltrates in the right upper lobe and the lower lobes bilaterally, mimicking metastases. A thoracoscopic lung biopsy demonstrated bronchiolitis obliterans organizing pneumonia caused by capsule-deficient cryptococcosis.

OBJECTIVE: To evaluate the surgical technique and outcome of pars plana capsulotomy and vitrectomy with infusion through the limbus to remove the posterior capsule opacification (PCO) in children with pseudophakic eyes.
METHODS: This technique was performed in 51 cases (57 eyes) of complicated PCO with pseudophakic eyes, which was almost impossible to remove through the Nd:YAG capsulotomy. OBJECTIVE: To evaluate the role of capsule endoscopy in the detection of causes of obscure gastrointestinal bleeding.
METHODS: Fifteen patients, nine males and six females, mean age 46 years (range 20-75 years), were investigated. All patients had undergone upper and lower gastrointestinal endoscopy with no evidence of causes of bleeding. Indication for capsule endoscopy was overt bleeding in 10 patients (eight with melaena and two with rectal bleeding) and anaemia in five patients.

A bilaterally aphakic patient received a silicone intraocular lens (IOL) over an intact capsular bag. The fellow eye had a large capsule rupture. Implantation of a 3-piece silicone IOL was attempted over the capsule remnant and achieved without complication. One year postoperatively, the IOL remained stable.

In order to evaluate the capsular reaction to high intraarticular pressure (IAP) in the knee during arthroscopy, we examined 13 patients admitted for elective surgery with joint infusion to 30, 70, 120 and 170 mmHg IAP (4, 9.3, 16 and 22.7 kPa).
