Brachialis to Anterior Interosseous Nerve Transfer: Comprehensive Anatomic Rationale.

BACKGROUND AND OBJECTIVES: Distal nerve transfers for muscle reinnervation and restoration of function after upper and lower motor neuron lesions are a well-established surgical approach. The brachialis to anterior interosseous nerve (BrAIN) transfer is performed for prehension reanimation in lower brachial plexus and traumatic cervical spinal cord injuries. The aim of the study is to shed light on the inconsistent results observed in patients who undergo the BrAIN transfer. METHODS: An anatomic dissection was conducted on 30 fresh upper limb specimens to examine the intraneural topography of

Hydrocephalus secondary to obstruction of the lateral apertures in two dogs.

BACKGROUND: Traditionally, hydrocephalus is divided into communicating or non-communicating (obstructive) based on the identification of a blockage of cerebrospinal fluid (CSF) flow through the ventricular system. Hydrocephalus ex vacuo refers to ventricular enlargement as a consequence of neuroparenchymal loss. Hydrocephalus related to obstruction of the lateral apertures of the fourth ventricles has rarely been described. CASE REPORT: The clinicopathologic findings in two dogs with hydrocephalus secondary to obstruction of the lateral apertures of the fourth ventricle are reported. Signs we

Injuries to the spinal accessory nerve: a lesson to surgeons.

The integrity of the spinal accessory nerve is fundamental to thoracoscapular function and essential for scapulohumeral rhythm. This nerve i … accessory nerve in relation to the sternocleidomastoid muscle was constant, with branches from the cervical plexus rarely conveying motor fibres. Damage to the nerve was predominantly posterior to this muscle. Despite the delay, the results of repair were surprising, with early relief of pain, implying a neuropathic source, which preceded generally good recovery …

Spontaneous intracranial hypotension: clinical and neuroimaging findings in six cases with literature review.

Spontaneous intracranial hypotension (SIH) is an uncommon but increasingly recognized syndrome. Orthostatic headache with typical findings on magnetic resonance imaging (MRI) are the key to diagnosis. Delayed diagnosis of this condition may subject patients to unnecessary procedures and prolong morbidity. We describe six patients with SIH and outline the important clinical and neuroimaging findings. They were all relatively young, 20-54 years old, with clearly orthostatic headache, minimal neurological signs (only abducent nerve paresis in two) and diffuse pachymeningeal gadolinium enhancement

[Fibrous-muscular and vessels anomalies revealing during surgery of the scalenus anticus syndrome].

One hundred and twenty five operations on 100 patients were performed due to the symptoms suggesting presence of the pressure on the neurovascular supraclavicular structures, although X-ray showed no evidence of cervical rib. In 15 cases of patients with so-called scalenus anticus syndrome as one of the forms of thoracic outlet syndrome (TOS) seemed to be compressing the neurovascular bundle. Scalenus mediums muscle appeared to be producing more compression of the nerve roots than did the scalenus anterior in 5 patients. Pressure from Rudimentary First Rib--in 2 cases in our series malformatio

Ideal intraspinal implantation site for the repair of ventral root avulsion after brachial plexus injury in humans. A preliminary anatomical study.

The advances made to date in root reimplantation for avulsion of the brachial plexus are modest considering that there are only reports from ten patients in the literature. However, the results are promising and should be applauded. The problem with reimplantation, in addition to the difficult surgical exposure, is to determine the ideal intraspinal implantation site for the graft. Given the non-permissive substrate properties of the CNS white matter, the outcome of grafts elsewhere in the spinal cord need to be evaluated. An inappropriate implantation site might explain the still modest recov

The sympathetic superior cervical ganglia as "little neuroendocrine brains".

The superior cervical ganglia (SCG) provide sympathetic innervation to the pineal gland, cephalic blood vessels, the choroid plexus, the eye, carotid body and the salivary and thyroid glands. Removal of the ganglia brings about several neuroendocrine changes in mammals, including the disruption of water balance in pituitary stalk-sectioned rats and the alteration of normal photoperiodic control of reproduction and thyroid function in hamsters, ferrets, voles, rams and goats. These effects are commonly attributed to pineal denervation. However, pinealectomy does not always mimic ganglionectomy

[About of 82 obstetrical astro-articular injuries of the new-born (excepting brachial plexus palsies). Limits of initial therapeutic aggression and follow-up of evolution, particulary concerning traumatic separation of upper femoral epiphysis (author's transl)].

Obstetrical fracture has not a bad character, because of quick healing and, above all, because of extraordinary modelling by growth. The authors record 33 fractures of long bones which confirm this good prognosis. 20 birth fractures of the clavicule associated with brachial plexus palsies are recorded on account of their pathogenic, diagnostic and therapeutic intricacies. Prognosis of obstetrical traumatic separation of the epiphysis (epiphysiolysis) would be more preoccupying. This feeling is not given off the series of 28 observations recorded which conern 13 upper humeral, 6 lower humeral e

Anatomical basis of female pelvic cavity for nerve sparing radical hysterectomy.

PURPOSE: The separation and sparing of the vesical branch of the inferior hypogastric plexus (IHP), to preserve the pelvic autonomic nerve, is critical and difficult in nerve sparing radical hysterectomy. Pelvic local anatomy was performed to provide the necessary anatomical information. METHODS: Precise pelvic anatomy was performed in 15 adult corpses, and immunohistochemical analysis was carried out on the parametrial tissues of three fresh cadavers to analyze the nerve fiber type and content of the vesical branch of the IHP. RESULTS: The deep uterine vein is located in the upper region of

The spectrum of neurological disorders presenting at a neurology clinic in Yaoundé, Cameroon.

INTRODUCTION: The burden of these neurological diseases is higher in developing countries. However, there is a paucity and scarcity of literature on neurological diseases in sub-Saharan Africa. This study was therefore undertaken to determine the pattern of neurological diseases in this setting and then, compare to those elsewhere in the African continent and also serve as a baseline for planning and care for neurological disorders in Cameroon. METHODS: The study was conducted at the Clinique Bastos, in Yaoundé, city capital of Cameroon, centre region. Over a period of six years, all medical

[Roles of cerebrovascular innervations and their neurotransmitters in various pathological conditions of central nervous system].

The concept of neurogenic control of the cerebral circulation began to develop only as recent as the middle of the 20(th) century, when it was elucidated that the cerebral blood vessels are supplied by a very dense plexus of adrenergic nerves innervating the vascular bed. After then, already in the course of several decades, more than ten types of neurotransimitters including acetylcholine, serotonin, substance P, and calcitonin gene-related peptide, etc., have been observed in these nerves. To determine the origins and pathways of cerebrovascular parasympathetic nerves and sensory nerves, ret

Anatomical and electrophysiological myotomes corresponding to the flexor carpi ulnaris muscle.

This study was designed to investigate the incidence of lateral root of the ulnar nerve through cadaveric dissection and to analyze its impact on myotomes corresponding to the flexor carpi ulnaris (FCU) assessed by electrodiagnostic study. Dissection of the brachial plexus (BP) was performed in 38 arms from 19 cadavers, and the connecting branches between the lateral cord and medial cord (or between lateral cord and ulnar nerve) were investigated. We also reviewed electrodiagnostic reports from January 2006 to May 2008 and selected 106 cases of single-level radiculopathy at C6, C7, and C8. The

Variations in the surface anatomy of the spinal accessory nerve in the posterior triangle.

Iatrogenic injury to the spinal accessory nerve has been widely documented and can have medico-legal implications. The resulting syndrome of … variation was seen in the surface and regional anatomy of the nerve and in the contribution of the cervical plexus to the spinal accessory nerve in the posterior triangle. Measurements of the running course and exit point of the nerve into and from the posterior triangle differed significantly from those previously recorded. Delineation of an accurate surface an …

Unrecognized pneumothorax as a cause of intraspinal air.

STUDY DESIGN: This is a case report. OBJECTIVE: A case of intraspinal air in a polytraumatized patient with unrecognized pneumothorax after blunt chest trauma is reported. SUMMARY OF BACKGROUND DATA: In most cases, intraspinal air is associated with degenerative disc disease, epidural abscess, or synovial cyst, or it follows iatrogenic manipulation. Scialdone and Wagle first reported a case of intraspinal air due to a laceration of the right main bronchus. No previous case of intraspinal air after a simple unsuspected pneumothorax following blunt chest trauma has been reported. METHODS: Dur

Developmental anomalies at the thoracic outlet: an analysis of 200 consecutive cases.

Anatomic observations were made during 200 consecutive transaxillary surgical procedures performed in 175 patients because of unremitting signs and symptoms of nerve or vascular compression at the thoracic outlet. There were 160 cases of brachial plexus compression and 40 cases of arterial or venous occlusion. In 132 (66%) of these cases, single or multiple abnormalities were recognized that represented developmental variations previously described in anatomic dissections or in embryologic studies. There were 17 cases of cervical rib or first thoracic rib abnormalities and 20 supernumerary sca

Effects of bupivacaine on the membrane properties of nerve cell soma.

While the effects of local anaesthetics on axonal conduction and axonal membrane have been extensively studied, there is little information about the actions of these agents on nerve cell soma. Therefore, the effects of the amide local anaesthetic bupivacaine on the electrophysiologic properties of the nerve cell soma were studied on isolated superfused superior cervical ganglia of rats. Administration of 100-200 nM of bupivacaine to the preparation produced marked changes in membrane properties of the cell soma. The resting membrane potential did not change, but the membrane resistance decrea

An Anatomic Variant of the Ansa Cervicalis Precluding Its Use as a Donor Nerve.

BACKGROUND: The ansa cervicalis is useful for cranial nerve repair, and may be harvested without apparent morbidity. Herein we report an unusual and surgically relevant anatomic variant of the ansa cervicalis. METHODS: An adult male with left parotid adenoid cystic carcinoma underwent parotidectomy with upper-division facial nerve resection and planned interposition repair using the ansa cervicalis. The ipsilateral hypoglossal nerve was identified, together with a descending branch producing strap muscle contraction when stimulated. This presumed descendens hypoglossi was unusually large in c
