[Combination of rare anomalies of the cervical spine and peripheral skeleton].

This paper presents a case report on a 59-year old patient of small stature with moderate mental retardation, strabismus and several rare skeletal anomalies. We found an os odontoideum, a dysontogenetic 3fold fusion of cervical vertebrae, multiple double-sided wrist bone dysplasias and brachytelephalangical digiti pedis I, but no anomalies of the heart or genitals. Clinical signs, biochemical parameters and results of x-ray and ultrasonic examination are demonstrated in this complex malformation syndrome that combines different anomalies known from other syndromes.

Median labiomandibular glossotomy.

The median labiomandibular glossotomy has been advocated to facilitate the removal of tumors in the base of the tongue, epiglottis, and posterior pharyngeal wall. Recently, the technique has been incorporated into a method that provides exposure to the clivus, upper cervical vertebrae, and the cervicomedullary junction. In this instance, the removal of a healthy mandibular incisor tooth was recommended. The case reported in this article illustrates the success of median labiomandibular glossotomy without sacrificing healthy teeth or jeopardizing a functional occlusion.

Soft tissue metastasis of a chemodectoma: a case report and review of the literature.

A case of a single soft tissue metastases (abdominal wall) occurring 15 years after removal of chemodectoma of the right carotid bifurcation is described. The tumor had apparently been completely removed with excision of portions of the common, external and internal carotid arteries up to the base of the skull. Seven years later osteolytic and osteoblastic lesions were noted in the cervical and thoracic vertebrae which were radiated with 3500 rad. The 19 previously reported cases of chemodectomas with distant metastases are reviewed.

Primary renal mesenchymal chondrosarcoma: a case report.

A 16-year-old female patient presented to our emergency department with a left upper extremity radiculopathy prompting several imaging studies. Magnetic resonance imaging of the neck revealed multiple cervical vertebrae lesions and computed tomography imaging demonstrated a 15-cm calcified mass originating from the left kidney and extending into the surrounding structures. Pathologic assessment of the open left radical nephrectomy specimen revealed a primary renal mesenchymal chondrosarcoma, a tumor that has only been documented in 5 prior case reports in the literature.

[Surgical treatment of back problems of a growing child].

During the years when a child is developing, the most common problems requiring surgical treatment of the back are scoliosis and spondylolisthesis of various causes. Spinal tumors and bacterial inflammations of the vertebrae or intervertebral disks occur rarely in children. Due to spinal injury, approximately 20 children require surgical treatment annually. The possibility of instability of the cervical spine should be taken into account especially in connection with anesthesia procedures in children with Down's syndrome and those suffering from disturbances of skeletal development.

Klippel-Feil syndrome associated with posterior fossa dermoid cyst. Case report.

A patient with association of Klippel-Feil syndrome and posterior fossa dermoid cyst is presented. The patient, a 36-year-old man, presented with an acute obstructive hydrocephalus due to the cyst and exhibited the typical triad of the Klippel-Feil abnormality with short neck, low hairline implantation and limited neck motion along with a complex cervical vertebrae fusion. The anatomical and clinical features as well as the pathophysiology of this rare association are discussed after a review of the literature.

Radiological evaluation of posterior cervical misalignment by ligamentum flavum angle in patients with degenerative cervical myelopathy.

PURPOSE: Posterior cervical misalignment is commonly observed in patients with degenerative cervical myelopathy (DCM), yet its clinical significance remains unclear. This study takes aim to investigate the radiological value of ligamentum flavum angle (LFA) to evaluate the implications of posterior cervical misalignment in DCM. METHODS: Baseline LFA was determined in 76 adults with normal cervical alignment. LFA was then analyzed in 155 DCM patients and categorized into enlarging (eLFA), normal (nLFA), and decreasing LFA (dLFA) groups. Cervical spine parameters and MRI findings were compared,

Sagittal alignment of the cervical spine after neck injury.

The normal sagittal alignment of the cervical spine is lordotic and is affected by the posture of the head and neck. The question of whether loss of cervical lordosis is the result of muscle spasm after injury or a normal variation, and the clinical significance of such changes in sagittal profile of the cervical spine has been an issue of several studies. The purpose of this paper is to study the incidence of normal cervical lordosis and its changes after neck injury compared to the healthy population. We studied the lateral radiographs of the cervical spine of 60 patients with neck injury co

Cervical column morphology and craniofacial profiles in monozygotic twins.

Previous studies have described the relationships between cervical column morphology and craniofacial morphology. The aims of the present study were to describe cervical column morphology in 38 pairs of adult monozygotic (MZ) twins, and compare craniofacial morphology in twins with fusions with craniofacial morphology in twins without fusion. Visual assessment of cervical column morphology and cephalometric measurements of craniofacial morphology were performed on profile radiographs. In the cervical column, fusion between corpora of the second and third vertebrae was registered as fusion. In

Management of Pharyngeal Perforation with Cervical Spine Injury Following Blunt Trauma on Electric Vehicle Accident.

BACKGROUND Cases of pharyngeal perforation (PP) associated with cervical spine injury following blunt trauma are extremely rare. CASE REPORT A 58-year-old woman was hit by a car while riding an electric bicycle. She presented with neck pain and dyspnea on admission to the hospital. The CT scan showed suspected fractures of the 5th and 6th cervical vertebrae without evidence of cervical emphysema. Serial blood tests revealed a rapidly decreasing hemoglobin level. An urgent surgical exploration of the neck was performed. Intraoperative findings included complete tears of the anterior and posteri…

Short-term Systemic Chemotherapy for Cervical Vertebral Single-system Single-site Langerhans Cell Histiocytosis.

Single-system single-site Langerhans cell histiocytosis (SS-s LCH) can involve the spine and cause local neurological and structural complications. A 7-year-old boy with SS-s LCH of the fifth cervical vertebra presented with neck pain and torticollis. Owing to osteolytic destruction and concern for cord compression, the patient received 6 weeks of vinblastine and prednisolone. The lesion regressed, and treatment was discontinued after the local risk was reduced. Although an iliac recurrence developed later, the cervical lesion remained under control. This case suggests that response-adapted ea

Management of Neglected Post-traumatic Bilateral Facet Dislocation of Sub-axial Cervical Spine: A Case Series.

Neglected bilateral facet dislocation of the lower cervical spine is a rare condition and found mostly in developing countries like Nepal. Delayed presentation makes treatment more challenging concerning decompression, reduction, neurological recovery, and overall outcome. We managed three cases of bilateral facet dislocations of the fifth-sixth-seventh cervical vertebra level presented after three months of injury. All of those were treated surgically by combined anterior-posterioranterior approaches with the same principle. One patient had a complete neurological recovery, the second one rec

The cervical anterior approach for the resection of superior posterior neurogenic tumor: a case report.

Many approaches for resection of the superior mediastinal tumors have been reported. We introduce an approach, which we call the cervical anterior approach. This approach is only cervical and does not require a sternotomy. Merits of this approach include the ability to remove the tumor without opening the mediastinal or parietal pleura, as well as obviating draining the thoracic cavity. The tumor is also directly visible, and the surgeon can avoid injury to the great vessels. This approach is recommended when the tumor is located superior to the third thoracic vertebra level, when it borders t

Posterior decompression of the vertebral artery narrowed by cervical osteophyte: case report.

BACKGROUND: Symptomatic vertebral artery compression caused by cervical spondylosis usually is caused by compression of the artery by osteophytes arising from the uncinate process. Compression from facet joint osteophytes is seldom reported. CASE DESCRIPTION: A 69-year-old male developed symptomatic vertebral artery stenosis secondary to an osteophyte arising from the superior facet of the sixth cervical vertebra posterior to the artery. A posterior decompression of the vertebral artery with removal of the offending facet joint complex relieved the patient's transient neurologic events. CONC

Comparison of sagittal measurements of cervical spine in spondylosis patients between Magnetic Resonance Imaging and Radiograph.

BACKGROUND: Cervical spondylosis is the common degenerative disease of the vertebrae in adults which can lead to change in sagittal alignment of cervical spine. Radiograph and Magnetic resonance imaging (MRI) are widely used imaging modalities for measuring the sagittal parameters. However sagittal parameters measured using radiograph and MRI can be influence by patient positioning and imaging technique. The study aims to compare sagittal parameters measured using MRI (Magnetic Resonance Imaging) and radiograph in cervical spondylosis patients. METHODS: The study was done retrospectively. 77
