Ectopic fat in liver and skeletal muscle is associated with shorter overall survival in patients with colorectal liver metastases.

BACKGROUND: Myosteatosis has been associated with shorter overall survival in cancer patients. The increase in ectopic fat might not be limited to skeletal muscle only and might also extend to other sites such as the liver, resulting in non-alcoholic fatty liver disease (NAFLD). In this study, we assessed the relationship between myosteatosis and NAFLD and their association with overall survival in patients with colorectal liver metastases undergoing partial hepatectomy. METHODS: Patients were selected from a prospective cohort of 289 consecutive patients with colorectal liver metastases. All

Cronkhite-Canada syndrome: An investigation in clinical features and pathogenesis.

OBJECTIVE: This study aimed to investigate the clinical features and potential pathogenesis of a rare nonhereditary polyposis syndrome, Cronkhite-Canada syndrome (CCS). METHODS: Medical records of eight patients with CCS who were admitted to our hospital from January 2005 to November 2019 were reviewed. Transcriptome profiling was performed in one patient to investigate its difference between gastric polyp tissue and normal mucosa. Differentially expressed genes (DEGs) were determined for functional analysis. The expression of inhibin beta A (INHBA) was further assessed by using immunohistoch

Dealing with insufficient liver remnant: Associating liver partition and portal vein ligation for staged hepatectomy.

Liver resection for colorectal liver metastases has emerged to highly successful treatment in the last decades. Key to this success is complete hepatic tumor removal and systemic disease control by chemotherapy. Associating liver partition and portal vein ligation for staged hepatectomy is the most recent two-stage resection strategy for patients with very small future liver remnant making complete tumor removal possible within 1 to 2 weeks. Oncological outcome data are being collected at the moment and first results from small series reveal promising results.

External auditory canal lesion: colorectal metastatic adenocarcinoma.

The patient presented to the ear, nose and throat clinic with failed conservative treatment of persistent right otitis externa. On examination, the roof of the right ear canal was polypoid and the tympanic membrane could not be visualised. There was a fragile mass noted in the external auditory canal (EAC) which on microsuction started to bleed. CT internal auditory meatus and MRI internal auditory meatus identified soft tissue mass in the EAC. The patient underwent urgent examination under anaesthetic of the ear and biopsies were taken. He had a background of ascending colon cancer; Duke's C1

[Single cardiac metastasis from colorectal cancer: an unusual localization].

Metastasis to the heart from malignancy is a frequent but underestimated event. Tumors that are located in the mediastinum, such as pleural mesothelioma, are more frequently associated with cardiac colonization. Few reports have described metastasis from colon adenocarcinoma, which usually colonizes liver and lungs. Moreover, intracardiac localization is more common for primary cardiac neoplasms than for metastasis. We present an unusual case of a patient operated for colon adenocarcinoma who exhibited a single intracardiac secondary localization. Although the mass was huge, the patient was co

Management of extracolonic tumours in patients with Lynch syndrome.

Hereditary nonpolyposis colorectal cancer, or Lynch syndrome, is responsible for 2-3% of all colorectal cancers. Lynch syndrome is also associated with a high risk of extracolonic cancers, including endometrial, stomach, small bowel, pancreas, biliary tract, ovary, urinary tract, brain, and skin cancer. In this Review, we discuss the risks, surveillance tests, and guidelines for the management of extracolonic tumours associated with Lynch syndrome. For all types of extracolonic cancer, evidence supporting surveillance is scarce. A benefit of surveillance is evident only for endometrial cancer,

Unusual sites of colorectal cancer metastasis.

We present 2 separate cases of adenocarcinoma of the colon with metastasis to the chin and the bladder, both of which are unusual sites of colorectal cancer metastasis. Patient 1 is a 77-year-old man who was diagnosed with adenocarcinoma of the colon, American Joint Committee on Cancer (AJCC) T4 N0 M0 (stage II), and underwent a right hemicolectomy. Fourteen months later he developed a firm 2.5-cm mass involving the chin. Excisional biopsy revealed moderately differentiated adenocarcinoma, consistent with the known colon primary tumor. Patient 2 is a 75-year-old man who was diagnosed with AJCC

[Gene therapy and liver metastases].

Liver metastases occur in approximately 60% of patients with colorectal cancer. Liver resection is currently the only treatment that offers long-term survival. The 5-year survival rates range from 25 to 40%; however, less than 20% of patients are candidates for resection. The prognosis for the remaining patients is grim, since palliative chemotherapy and symptomatic care are the only available options. The ability to transfer therapeutic genes to target cells in vivo has opened up unprecedented possibilities for the management of metastatic liver cancer. A large number of genes with therapeuti

[Radiotherapy and symptomatic treatments of advanced colorectal cancers].

Treatments of colorectal cancers can still be curative up to fairly advanced stage. Clinical situations vary greatly depending on the primary: local recurrence is a major event in rectal cancer whereas metastases are the main problem in colon cancer. Therefore, rectal cancer benefit from locoregional combined treatments, chemotherapy, radiotherapy and surgery, to control the pelvic disease. In the metastatic setting, it is important to offer potentially curative treatment as often as possible. Radiotherapy for bone or brain metastasis is well known and frequently used; irradiation of painful l

Follow-up after polypectomy.

Progress in understanding the biology, natural history, and relationship to colorectal cancer has provided the basis for a rational approach to patients with adenomas. Pathological assessment of polyps is essential for initial management; only adenomas require search for synchronous neoplasia. A finding of a 30%-50% synchronous rate has been observed. After clearing of the colon of all adenomas, follow-up examination will reveal a 30%-50% rate of metachronous adenomas, but usually with unimportant pathology. Patients with no special concerns at initial polypectomy can have surveillance at inte

Update on colorectal cancer. Risk factors, diagnosis, and treatment.

Colorectal cancer is more common in the Western world than in underdeveloped countries. Diet, longevity, heredity, and presence of other bowel diseases may affect the incidence. Diagnosis is based on results of routine laboratory studies and evaluation of the entire large bowel with air-contrast barium enema and colonoscopy. Surgical resection is the primary therapy for colorectal cancer. Postoperative systemic chemotherapy yields poor results, but hepatic artery infusional chemotherapy offers some benefit to patients who have only hepatic metastases. Follow-up evaluation includes physical exa

Current understanding and management of brain metastasis from colorectal cancer. A review.

BACKGROUND: Colorectal cancer (CRC) ranks as the third most diagnosed cancer worldwide and the second leading cause of cancer-related death. Although relatively rare, brain metastases from CRC (BMCRC) have been linked to poor prognosis and reduced survival rates. The incidence of BMCRC is increasing, possibly because of improved diagnostic techniques and prolonged patient survival due to advances in systemic treatments. METHODS: A systematic literature review was conducted following PRISMA guidelines, utilizing databases such as PubMed, Embase, and the Cochrane Library. Eligible studies inclu

Colorectal cancer liver metastases: mechanism and therapy.

Colorectal cancer (CRC) is a leading cause of cancer mortality worldwide, with liver metastasis representing the principal cause of death in CRC patients. Current primary treatment options for colorectal cancer liver metastases (CRLM) encompass chemotherapy, radiotherapy, targeted therapy, and immunotherapy. However, the survival rate for CRLM patients remains below 40%. The unique immune-tolerant microenvironment of the liver has recently been recognized as a central driver of metastatic colonization, immune evasion, and therapeutic resistance. This review examines the development of CRLM, wi

Expanding the therapeutic Frontier: Liver transplantation for unresectable colorectal liver metastases.

A substantial proportion of patients with colorectal cancer present with or ultimately develop metastatic disease confined to the liver. The management of colorectal liver metastases remains one of the most complex and debated areas in oncological practice, with curative resection feasible in only a minority of cases. However, emerging evidence, most notably from the TransMet study, has reshaped the therapeutic landscape. The demonstration of a clear survival advantage in carefully selected patients with unresectable hepatic disease undergoing liver transplantation represents a transformative

Living Donor Liver Transplantation for Colorectal Cancer Liver Metastasis.

Liver transplantation (LT) for unresectable colorectal liver metastases has evolved from early feasibility studies to a biologically selected, multidisciplinary strategy for patients with liver-confined disease. Evidence from the SECA program and randomized data from TransMet support meaningful survival in carefully chosen candidates. Contemporary selection emphasizes sustained chemotherapy response, time-based observation, PET-derived metabolic burden, CEA kinetics, and extrahepatic disease. Living donor LT has expanded LT access within these established frameworks, offering elective timing a

Exploring the Role of Epithelial-Mesenchymal Transition During Colorectal Cancer Peritoneal Metastasis: Update on Their Mechanisms.

Colorectal cancer is a common malignant tumor worldwide. The prognosis of patients with colorectal cancer peritoneal metastasis is very poor. The study of the specific mechanisms of colorectal cancer peritoneal metastasis plays an important role in the treatment of patients with this disease. The mechanisms of colorectal cancer peritoneal metastasis are mainly pathological and biological. Biologically, the epithelial-mesenchymal transition process is an important precursor to tumor cell metastasis. Therefore, it is necessary to study the mechanisms of colorectal cancer peritoneal metastasis, e
