Food intolerances.

Food intolerances are prevalent and often confused with food allergies. This article reviews the complex landscape of adverse reactions to food, distinguishing between immune-mediated responses (food allergies) and nonimmune reactions (food intolerances). It also explores specific food intolerances such as lactose intolerance, nonceliac gluten sensitivity, fructose intolerance, and salicylate sensitivity.

Common psychiatric disorders in cancer patients. II. Anxiety and acute confusional states.

Anxiety is a very prominent symptom of cancer patients and appears most frequently in connection with adjustment disorders. However, it can also be part of a normal adaptation to the disease, related to an anxiety disorder, or can be caused by variety of other reasons, including underlying somatic processes. The clinical presentation, etiology, diagnostic proceedings and the treatment of anxiety in patients with cancer is surveyed. Delirium (acute confusional states) is a neglected psychiatric syndrome in regard to diagnosis, treatment and research efforts. Especially in cancer patients, delir…

Always Getting Lost: Defining Developmental Topographical Disorientation (DTD)-A Systematic Literature Review.

Developmental topographical disorientation (DTD) refers to a condition of highly impaired navigation ability in healthy individuals. DTD often leads to severe consequences in daily life, affecting education and professional choices and limited everyday mobility. Since its first description in 2009, a substantial number of empirical studies on DTD have appeared, but a clear clinical definition of DTD that can be used to develop a behavioral assessment tool is not yet available. The aim of the current study was to shed more light on the precise behavioral characteristics of DTD by examining the

Orientation and disorientation: lessons from patients with epilepsy.

Orientation in time, space, and person is a fundamental cognitive faculty and the bedrock of neurological and psychiatric mental status examination. Nevertheless, research in orientation and disorientation is neglected in both cognitive science and neuropsychiatry. Specifically, it is still unclear whether disorientations in time, space, and person represent a failure of the same system or merely share a common nomenclature and whether these three domains of orientation depend on different psychological and neural systems. Here, we analyzed descriptions of patients with specific orientation fa

Potential of GPT-4 for Detecting Errors in Radiology Reports: Implications for Reporting Accuracy.

Background Errors in radiology reports may occur because of resident-to-attending discrepancies, speech recognition inaccuracies, and large … ution. There were 150 errors from five common error categories (omission, insertion, spelling, side confusion, and other) intentionally inserted into 100 of the reports and used as the reference standard. Six radiologists (two senior radiologists, two attending physicians, and two residents) and GPT-4 were tasked with detecting these errors. Overall error d …

Bewilderment, aquilombar, and the antimanicolonial: three ideas to radicalize Brazilian Psychiatric Reform.

This article is part of a study aimed to map antiracist knowledge and practices in mental health by monitoring the practices of three collectives of professionals working in/with the psychosocial care network in the city of São Paulo, allowing us to characterize their intervention strategies. To contribute to the conceptualization of this article, through a review of the decolonial literature, three major ideas have been outlined that have allowed us to give substance to the decolonization of Psychiatric Reform: bewilderment, which, in dialogue with Achille Mbembe and Frantz Fanon, invites us

Distress, burden, and wellbeing in siblings of people with mental illness: a mixed studies systematic review and meta-analysis.

BACKGROUND: Family members of people with mental illness (MI) may experience a host of psychological adversities such as increased stress, b … ularly vulnerable during their siblings' illness onset and times of relapse. Limited communication, confusion about MI, and the need to compensate may contribute to siblings' distress and/or burden. Siblings' experience of wellbeing and caregiving were closely related. CONCLUSION: This review highlights the complex psychological experience of siblings and …

[Ammonia as a cause for hepatic encephalopathy].

In a patient with cirrhosis of the liver, associated with hepatitis B virus, who was admitted for confusion and acute elevation of liver enzymes, a diagnosis of hepatic encephalopathy was made. A serum ammonia level of 54 (normal less than 33) microgram/liter, supported the diagnosis, but puzzled the medical staff regarding the possibility that ammonia may directly induce the confusion. While it is widely accepted that the ammonia level is a marker that usually parallels the amount of toxins and metabolites that bypasses the liver, its role in causing brain dysfunction is debated. However, sin…

[Pure agraphia with topographical disorientation caused by right hemisphere lesion].

A 69-year-old, right-handed, Japanese male patient presented with pure agraphia with topographical disorientation after hemorrhage in the right parietal lobe. Upon developing cerebral hemorrhage, he was referred to our hospital for close examination of agraphia. There was no paresis or clumsiness in his extremities. His speech was fluent and well-articulated. Neither aphasia nor reading impairments was found, although there was a clear writing impairment with effort and hesitation. His writing of both kanji and kana letters contained additional, absent or deformed strokes or parts. No hemianop

[Status epilepticus with confusional symptomatology].

Non-convulsive confusional status epilepticus (NCSE) is classically separated into two forms on the basis of the ictal EEG, i.e., absence status (AS) and complex partial status epilepticus (CPSE). The diagnosis is difficult on the basis of clinical semiology alone, and requires emergency EEG investigation. Absence status, or 'petit mal' status, is a polymorphic condition that can complicate many epileptic syndromes, and is the most frequently encountered form of NCSE. It is characterized by confusion of varying intensity, associated in 50% of cases with bilateral periocular myoclonias. The EEG…

[Ideational apraxia in confusional psychosis].

Traditionally, Pick's ideational apraxia has been considered a neurologist's realm since it is one of the permanent manifestations of brain lesions located in the left hemisphere posterior territory. Besides, however, the ideational apraxia is likely to appear--altogether with misidentifications and spatial disorientation--as a part of the transient manifestations of confusional psychosis. It is thus quite possible that a reversible alteration of the posterior brain mechanisms could be the basis for the confusional psychosis. It seems also reasonable to include the ideational apraxia within th…

Competency versus proficiency: trying to develop an understanding of these terms in the context of practice learning.

The language of nurse education in the UK has shifted from 'competency' to 'proficiency' since 2018. The shift in language has been poorly articulated leaving individual higher education institutions, practice supervisors and assessors to interpret what these terms mean in relation to assessing student nurses. There appeared to be some confusion regarding the interpretation of the term proficiency among those who engaged in an exchange on the topic via Twitter. Students may focus on the procedure-based checklists within the standards. The relationship between proficiency and implementing a per…

Disorienting or Transforming? Using the Arts and Humanities to Foster Social Advocacy.

INTRODUCTION: The arts and humanities (AH) have transformative potential in medical education. Research suggests that AH-based pedagogies may facilitate both personal and professional transformation in medical learners, which may then further enhance the teaching and learning of social advocacy skills. However, the potential for such curricula to advance social advocacy training remains under-explored. Therefore, we sought to identify how AH may facilitate transformative learning of social advocacy in medical education. METHODS: Building upon previous research, we conducted a critical narrati

Perception of illusory clusters: the role of native timing.

We explore the influence of native timing patterns on nonnative speech perception, by asking whether a nonnative CVCV sequence can be percei … f an illusory cluster when Georgian listeners hear French CVCá. The findings reveal such perceptual confusion, particularly in the CCa-CøCá contrast in which the nonnative /ø/ is phonetically similar to the CC transition in Georgian, both in terms of temporal organizations and tongue shape. This confirms the possibility of illusory clusters, which is consis …

Transient global amnesia following a transoceanic flight.

Transient global amnesia (TGA) is the abrupt onset of temporary anterograde amnesia without any focal neurological features. It is a benign condition and is completely reversible in most cases. However, the sudden occurrence of memory impairment can be especially alarming to an individual who has recently arrived in an unfamiliar setting. The cause of TGA remains unknown. Reported here is the unusual occurrence of TGA in a man following the completion of an international airplane flight. Possible etiologies of the episode are considered.

Confusional symptomatology distinguishes early- and late-onset Alzheimer's disease.

Symptoms of confusion were examined in 75 patients with Alzheimer's disease (AD). Mild confusion was found in 20, and mode-rate/severe confusion in 8 patients. Confusion was more frequent in the late-onset (26/44-59%) than in the early-onset AD group (2/31-6%) (p less than 0.0001), and patients with confusion were older (p less than 0.0001) than those without confusion. The frequency of confusion was higher in patients with ischemic heart disease (13/28-46%) than in patients without this vascular factor (10/47-21%) (p less than 0.05). An inverse relation was found between confusional symptomat…
