Paediatric Sternoclavicular Joint Pseudodislocation Fixation Technique.

BACKGROUND: Posterior sternoclavicular joint (SCJ) injuries in children are rare and frequently misdiagnosed. Due to the anatomy of the immature medial clavicle, many of these injuries are Salter-Harris type I/II physeal fractures rather than true joint dislocations-termed pseudodislocations. These injuries require careful diagnosis and tailored management to ensure safe and functional outcomes. INDICATIONS: Surgical intervention is indicated in paediatric SCJ pseudodislocations with significant posterior displacement of the medial clavicular fragment, particularly when the posterior metaphys

Minimally-Invasive, Retensionable, Onlay Double-Bundle Medial Collateral Ligament Reconstruction.

BACKGROUND: The superficial medial collateral ligament (sMCL) is the most injured knee ligament, and the most commonly coinjured ligament in anterior cruciate ligament tears. The technique's goal is to be less invasive while maintaining the hamstrings' function. INDICATIONS: Surgical indications for this technique include isolated grade 3 injuries that fail nonoperative management and multiligament knee injuries that include the MCL. TECHNIQUE DESCRIPTION: Items needed for the double-bundle reconstruction technique include an anterior tibialis graft > 270 mm, C-Arm for visualization of radio

Ulnar Collateral Ligament Hybrid Reconstruction With Palmaris Longus Autograft, Suture Brace Augmentation, and Ulnar Nerve Transposition.

BACKGROUND: Many overhead athletes, both amateur and professional, experience an increasing number of injuries to the ulnar collateral ligament (UCL) as a result of overuse or trauma. As a result of this increasing frequency, multiple techniques have been developed to reconstruct or repair the UCL with success and faster return to sports (RTS) for athletes who are ideal candidates for a repair. However, a large proportion of UCL tear patients cannot undergo repair due to chronic injury, leading to poor tissue quality, midsubstance tears, or older age, which predispose them to slower healing an

Meniscal Root Repair With Centralization.

BACKGROUND: Meniscal root tears compromise hoop stress resistance and lead to elevated tibiofemoral contact pressures, meniscal extrusion, and rapid progression of osteoarthritis. Prompt diagnosis and intervention are essential to prevent further degenerative changes. INDICATIONS: Meniscal root repair is indicated in patients with acute traumatic tears or chronic symptomatic root tears without advanced arthritis. Root repair has been shown to be beneficial regardless of the patient's age, even in the setting of early arthritis. Contraindications to root repair are severe osteoarthritis or poo

Distal Medial Collateral Ligament Repair With Suture Augmentation.

BACKGROUND: Distal medial collateral ligament (MCL) injuries are less common and have worse healing potential than proximal MCL injuries. The purpose of this video is to demonstrate the surgical technique of distal MCL repair with suture augmentation. INDICATIONS: The patient is a 17-year-old man who sustained a distal MCL injury and medial meniscocapsular junction tear while playing soccer. He was indicated for surgical management due to a complete distal MCL tear with symptomatic instability and concomitant medial meniscocapsular injury. TECHNIQUE DESCRIPTION: The patient was placed in a s

Tibial Tubercle Osteotomy With Medial Patellofemoral Ligament Reconstruction Utilizing Autograft Hamstring Tendon and Knotless Suture Anchor Fixation.

BACKGROUND: Recurrent patellar instability is defined as ≥2 dislocation events or recurrence after surgical treatment. Surgical intervention is aimed at correcting pathoanatomic abnormalities. A tibial tubercle osteotomy (TTO) is an intervention that corrects patients with high tibial tuberosity-trochlear groove (TT-TG) distances and/or patella alta as measured by increased Caton-Deschamps index (CDI). Anteromedialization osteotomies improve abnormal TT-TG distances and offload the patellofemoral joint in patients with chondral lesions. Distalization can be utilized to correct patella alta and

Taping for an Acute and Subacute Patellofemoral Dislocation, Recurrent Subluxation.

BACKGROUND: A primary acute patellofemoral dislocation needs to be managed well to ensure adequate tissue healing. Evidence that surgical intervention is better than conservative management is equivocal, particularly in the skeletally immature individual. Conservative management is less expensive and less invasive, and it may be better at maintaining knee function than surgical intervention. Along with appropriate immobilization, using rigid strapping tape, conservative management should consist of a tailored rehabilitation program to improve dynamic lower limb loading and quadriceps control,

Evolution of Tensionless Repair of Lateral Radial Meniscus Tear With the Use of the Traction Stitch.

BACKGROUND: Meniscus surgery remains one of the most common orthopaedic surgeries performed in the United States each year, with radial tear patterns being a common subset. In young, active patients with mechanical symptoms, surgery is often indicated. While management via partial meniscectomy predominates historically in tears of this type, recent success has been found in surgical repair. As repair techniques continue to evolve, this case demonstrates the use of the traction suture to maintain anatomic reduction throughout repair, ultimately resulting in a low-tension construct. INDICATIONS

Patient-Specific Custom Patellofemoral Arthroplasty.

BACKGROUND: Patellofemoral arthroplasty (PFA) can be done with a number of devices that implement both inlay and onlay designs. Recent innovations in custom instrumentation allow for a new class of PFA that can precisely match the variable trochlear morphology. INDICATIONS: Patients with severe patellofemoral arthritis and preserved tibiofemoral joint space who had conservative treatment that has failed may benefit from PFA. TECHNIQUE DESCRIPTION: A medial parapatellar arthrotomy of roughly 10 cm is used to evert the patella for visualization of the patellofemoral cartilaginous surfaces. An

Anatomic Multiligament Knee Reconstruction With Biceps Femoris Tendon and Medial Meniscal Repair.

BACKGROUND: Multiligament knee injuries are defined as tears of ≥2 of the following ligament structures: anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), posteromedial corner, or posterolateral corner (PLC). These injuries occur in various settings and may be associated with knee dislocations. INDICATIONS: The literature has reported that anatomic technique and a single-;stage approach for treating multiligament knee injuries are optimal because they may lead to improved outcomes. An anatomic double-;bundle PCL reconstruction is suggested to improve knee kinematics compare

Double-Bundle Posterior Cruciate Ligament and Concomitant Medial Collateral Ligament and Posterior Oblique Ligament Reconstruction After a Motor Vehicle Accident.

BACKGROUND: Motor vehicle accidents constitute the most prevalent high-energy mechanisms for knee dislocation, resulting in multiligament knee injuries (MLKIs). When concomitant posterior cruciate ligament (PCL) and posteromedial corner injury becomes chronic after failed conservative treatment, surgical intervention is indicated to restore the native knee biomechanics. INDICATIONS: Double-bundle PCL reconstruction is indicated for chronic instability, increased posterior tibial translation, decreased tibial slope, and failure of conservative treatment measures. posteromedial corner reconstru

Acetabular Rim Variants.

BACKGROUND AND INDICATIONS: Acetabular rim ossification variants have a reported incidence of 17%. These variants include labral calcifications, os acetabuli or acetabular rim fractures, and labral ossification. Labral calcifications are small soft calcification deposits within the labrum in patients with femoroacetabular impingement (FAI). The overall cause is unknown. For acetabular rim fractures/os acetabuli, there are several proposed causes, including unfused secondary ossification center (true os acetabuli) and repetitive microtrauma leading to a stress fracture of the acetabulum (acetab

Arthroscopic Biceps Tenodesis With Interference Screw Fixation: A Technique Video.

BACKGROUND: Management of long head of the biceps tendon (LHBT) pathology is accomplished through a biceps tenotomy or tenodesis. While both modalities provide beneficial outcomes, a biceps tenodesis may confer improved cosmesis, functional outcomes, and decreased muscle cramping postoperatively. Many procedural considerations are undertaken prior to a tenodesis, such as the surgical approach and fixation device. While similar clinical outcomes are achieved between an open subpectoral and arthroscopic suprapectoral biceps tenodesis (ASPBT) with interference screw (IS) fixation, the latter tech

Lateral Collateral Ligament Reconstruction With Tensionable Loops and Suture Tape Reinforcement.

BACKGROUND: The posterolateral corner (PLC) is an important knee stabilizer that resists varus stress, external tibial rotation, and posterior tibial translation. Untreated PLC injuries have been shown to increase failure rates of anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) reconstructions and lead to degenerative changes. Our modified Arciero technique reconstructs the femoral insertion site and functionality of the lateral collateral ligament (LCL) and popliteal fibular ligament, components of the PLC, with an internal brace and tensionable loops. Osteochondral Allograft Transplantation: A Technical Note.

BACKGROUND: Chondral injuries in the knee are commonly discovered during knee arthroscopy procedures. Due to the poor restoration potential and avascular nature of cartilage, large defects are commonly treated with such surgical procedures. Treatment utilizing an osteochondral allograft (OCA) transplant for symptomatic focal cartilage defects in the patellofemoral joint has demonstrated strong, lasting clinical and radiographic outcomes. INDICATIONS: Active and otherwise healthy patients are indicated for surgery when presenting with significant joint-line pain and a large focal chondral defe

The Active External Rotation Sign: A Physical Exam Technique for Detecting Posterolateral Corner Injury in Chronic Multiligament Knee Injuries.

BACKGROUND: The posterolateral corner (PLC) of the knee comprises important static and dynamic stabilizers, including the lateral collateral ligament, popliteus tendon, biceps femoris tendon, the popliteofibular ligament, and the posterolateral capsule. These structures are disrupted in many knee injuries, yet may be overlooked due to the complex anatomy, subjective clinical diagnostic exams, and poorly defined imaging techniques. Complete PLC lesions rarely heal with non-operative treatment, and can lead to significant instability and poor outcomes, and are therefore most successfully treated

Revision Patellar Tendon Reconstruction Using Hamstring Tendon Autograft.

BACKGROUND: Patellar tendon ruptures presenting in a chronic setting are rare events that impose technical surgical challenges due to proximal retraction of the patella, quadriceps muscle atrophy and contracture, and peripatellar adhesions. Various reconstruction techniques have been described using different grafts and fixation methods; however, there is a paucity of reported outcomes and there is no consensus on standard of care. INDICATIONS: The patient is a 36-year-old man who presented with a failed patellar tendon reconstruction 14 months after his initial surgery (performed 1 year afte

Anterior Opening-Wedge Proximal Tibial Osteotomy for Slope Correction of Genu Recurvatum.

BACKGROUND: Symptomatic genu recurvatum is defined as greater than 5° of knee hyperextension and can be caused by osseous deformity, soft tissue laxity, or a combination. Common symptoms include pain, weakness, instability, decreased range of motion, leg length discrepancy, and stretching of the posterior capsuloligamentous structures of the knee. In instances where the genu recurvatum is caused by reverse tibial slope, literature supports the use of anterior opening-wedge proximal tibial osteotomy (OW PTO) to treat genu recurvatum by increasing tibial slope. Correction of anterior slope to a

Anatomic Popliteus Tendon and Tibiofibular Joint Reconstructions.

BACKGROUND: The popliteus tendon is a primary stabilizer of external knee rotation and has been described as the "fifth ligament of the knee." Injuries involving the posterolateral corner of the knee commonly involve the popliteus tendon; isolated injury to the popliteus tendon is relatively rare and usually involves a rotatory injury mechanism and symptoms of instability and pain. In this patient, there was a concomitant injury to the proximal tibiofibular joint. INDICATIONS: Popliteus tendon reconstruction is indicated for patients with abnormal external rotation on examination and symptoms
