Carpal tunnel syndrome: reconciling "demand management" with clinical need.

Purchasers of surgical services are seeking justification for operative interventions with increasing frequency. This paper seeks to identify all relevant data currently available for carpal tunnel decompression; one of the commonest operative interventions in hand surgery. Such data, as is available, would suggest carpal tunnel decompression rates in the United Kingdom are relatively low, with fairly prolonged preoperative duration of symptoms.

Vestibular nerve dysfunction and decompression in Engelmann's disease.

Vestibular nerve dysfunction as the major neuro-otological symptom in one of the osteopetrosis group of bone disorders is unusual. We describe a patient with Engelmann's disease who presented in this manner and who benefited from an eighth nerve decompression procedure. Surgical decompression of the internal auditory meatus to relieve vertigo has to our knowledge not been previously reported in this condition.

Decompressive hemicraniectomy in supra-tentorial malignant infarcts.

BACKGROUND: Decompressive hemicraniectomy not only reduces the intracranial pressure but has been demonstrated to increase survival and decrease the morbidity in patients with supratentorial malignant brain infarcts (STMBI). The aim of this study was to assess the efficacy of surgical decompression to decrease the mortality and morbidity in patients with STMBI refractory to medical therapy and to compare the results with those of the medically managed patients. METHODS: All the 24 consecutive patients with clinical and radiological diagnosis of STMBI, refractory to medical management in 2 yea

Necessity of Direct Decompression for Thoracolumbar Junction Burst Fractures with Neurological Compromise.

BACKGROUND: Surgical management of burst fractures is controversial, with many different operative options. From a posterior approach, decompression of the spinal cord can be performed through both indirect and direct methods, the former relying on ligamentotaxis. It is unclear whether indirect decompression with ligamentotaxis is as effective as direct decompression. METHODS: Prospective, randomized controlled data were retrospectively analyzed to include only burst fractures of the thoracolumbar junction. Patients were treated with either direct decompression, involving wide posterior decom

A breakthrough in lumbar surgery: ipsilateral interlaminar endoscopy and its impact on spinal stability and patient outcomes.

Lumbar stenosis (LS) often leads to debilitating symptoms due to nerve compression in the spinal canal. As this condition becomes increasingly prevalent among the elderly, innovative surgical techniques are crucial. This letter examines a recent advancement introduced by Marco Aurélio Moscatelli et al., who have developed an ipsilateral interlaminar endoscopic approach for decompression at the L5-S1 level. Their study, involving 30 patients with degenerative stenosis, reports significant improvements in quality-of-life metrics, including the Oswestry Disability Index (ODI) and visual analog sc

Total decompression of facial nerve for Melkersson-Rosenthal syndrome.

Melkersson-Rosenthal syndrome (MRS) is a rare condition characterized by recurrent facial paralysis in addition to various orofacial manifestations. The condition appears to be a granulomatous disorder causing oedema and inflammation of the soft tissues of the face, lips, oral cavity and particularly, the facial nerve. There is general agreement that the symptoms and signs resolve spontaneously, aided perhaps by an empirical course of oral steroids. However, in some patients the condition may be progressive, leading to disfiguring facial synkinesis and increasing residual paralysis. There is a

Establishing benchmarks for the volume-outcome relationship for common lumbar spine surgical procedures.

BACKGROUND CONTEXT: The importance of surgeon volume as a quality measure has been defined for a number of surgical specialties. Meaningful procedural volume benchmarks have not been established, however, particularly with respect to lumbar spine surgery. PURPOSE: We aimed to establish surgeon volume benchmarks for the performance of four common lumbar spine surgical procedures (discectomy, decompression, lumbar interbody fusion, and lumbar posterolateral fusion). STUDY DESIGN: A retrospective review of data in the Florida Statewide Inpatient Dataset (2011-2014) was carried out. PATIENT SAM

[Pure Endoscopic Microvascular Decompression for Trigeminal Neuralgia and Hemifacial Spasm].

The panoramic view offered by endoscopy has been used to observe neurovascular compression in endoscope-assisted microsurgery to compensate for the blind spots in microvascular decompression(MVD)for trigeminal neuralgia and hemifacial spasm. In recent years, purely endoscopic MVD has been performed as a minimally invasive surgery and has become a new surgical technique for MVD. Endoscopic MVD has gained acceptance, and further developments are expected. Here, we present endoscopic MVD, including the setup, equipment, surgical procedures, and decompression techniques by transposition, as well a

Lumbar Spinal Stenosis: Review Update 2022.

Patients with lumbar spinal stenosis (LSS) may experience neuropathic symptoms, such as back pain, radiating pain, and neurogenic claudication. Although the long-term outcomes of both nonsurgical and surgical treatments are similar, surgery may provide shortterm benefits, including improved symptoms and lower risk of falling. Decompression is mainly used for surgical treatment, and depending on the decompression degree and associated instability, combination therapy may be given. Minimally invasive surgery has been demonstrated to produce excellent results in the treatment of LSS. Thus, an app

Spinal cord compression from Wegener's granulomatosis: an unusual presentation.

Wegener's granulomatosis (WG) causing spinal cord compression is very rare with only few cases reported in literature. We present a case report with review of literature. A 55-year-old lady with known WG presented with acute on chronic spinal cord compression. MRI scan revealed spinal cord compression anteriorly and posteriorly at T2-T5 level. Patient underwent urgent surgical decompression with excision of the posterior dural lesion with synthetic duraplasty. Patient made good neurological recovery. Histopathology revealed features consistent with WG. A rare case of spinal cord compression fr

Patient satisfaction with tourniquet application and local anaesthesia infiltration in carpal tunnel decompression and the relationship with overall satisfaction.

This was a prospective study designed to determine what effect poor tolerance to tourniquet application and local anaesthesia infiltration in open carpal tunnel decompression has on overall patient satisfaction with the surgical procedure. Fifty patients were recruited into the study. Visual analogue scores (VAS) were recorded postoperatively for pain related to tourniquet application, local anaesthesia infiltration and the surgical procedure overall. In terms of the procedure, poor tolerance to the tourniquet and local anaesthetic showed no significant relationship to the overall patient sati

[Value of various radiological study results in the follow-up of Camurati-Engelmann disease].

Camurati-Engelmann disease is a rare progressive bone dysplasia; involvement of the skull base can lead to deafness, vestibular disturbances, facial paralysis and damage to the optic nerves. Treatment with corticosteroids, calcitonin and diphosphonates promises only very limited success. Conservative treatment of compression of the cranial nerves is almost ineffective, but the aim of surgical treatment is decompression of involved nerves. The differential diagnosis depends on radiological findings and clinical symptoms. Follow-up depends on radiographic examination and skeletal scintigraphy fo

Lateral Retrocanthal Orbital Decompression for Children With Thyroid Eye Disease: A Canthus-sparing Approach.

Thyroid eye disease (TED) is one of the most common causes of proptosis in children. The traditional surgical approach to manage disfiguring proptosis associated with TED is orbital decompression surgery, which is often accomplished via lateral wall decompression through canthotomy and cantholysis. However, this technique can result in lateral canthus scarring and migration. Here, we describe the effectiveness of a skin-sparing retrocanthal transconjunctival surgical technique for lateral orbital decompression in pediatric patients with TED. Twelve pediatric patients with TED and disfiguring p

Cost-effectiveness and Safety of Interspinous Process Decompression (Superion).

OBJECTIVE: There are several treatment options for patients suffering from lumbar spinal stenosis, including surgical and conservative care. Interspinous spacer decompression using the Superion device offers a less invasive procedure for patients who fail conservative treatment before traditional decompression surgery. This review assesses the current cost-effectiveness, safety, and performance of lumbar spinal stenosis treatment modalities compared with the Superion interspinous spacer procedure. METHODS: EMBASE and PubMed were searched to find studies reporting on the cost-effectiveness, sa

Clinical outcomes of combined surgical treatment of medial epicondylitis and cubital tunnel syndrome.

Surgical results for treatment of medial epicondylitis and cubital tunnel syndrome are generally satisfactory when performed alone. However, our experience suggests a combined procedure is associated with inferior outcomes. A retrospective review was conducted of consecutive surgical cases of medial epicondylectomy/debridement and ulnar nerve decompression during a single operation at our institution from March 2008 to February 2017 using CPT codes. Thirty combined procedures were identified in 29 patients. Fourteen patients and 15 elbows returned to clinic for evaluation at average 4.3 years
