Perfecting the venerable SSC.

Stainless steel crowns (SSCs) are the preferred restoration of pediatric dentists for children requiring a pediatric dental rehabilitation due to their low cost and durability. Despite low technique sensitivity SSC placement can be challenging in some clinical situations including when placed on primary canines. Primary canines restored with SSCs often result in premature contacts and posterior open bite making acquiring an "as the patient presented" (ATPP) occlusion difficult when providing a pediatric dental rehabilitation. Opening of the posterior occlusion due to premature canine contact i…

High blood pressure detection: a new public health measure for the dental profession.

Detection of high blood pressure can be a major public health service contribution by the dental profession. The pioneer program at Fairleigh Dickinson University, in conjunction with the Bergen County Health Department and the Bergen County Dental Society, is encouraging dentists to participate in the National High Blood Pressure Program. Participants are urged to measure the blood pressures of all their adolescent and adult patients and to refer those with sustained elevated readings to their physicians, preferably by the arrangements of appointments.

[Dental decisions in the shadow of complications: risk assessment and analysis of treatment strategies].

INTRODUCTION: During tooth extractions, there is always a possibility of complications. In the case of maxillary teeth, this can be oroantral communication. Its surgical management is important to prevent long-term complications. There are factors which can predict the possibility of oroantral communication, even with the help of a preoperative panoramic radiograph. OBJECTIVE: In our questionnaire-based study, we wanted to determine what aspects dentists take into consideration before tooth extractions, during referral to a specialist, and in complication management. METHOD: Our Google Forms…

Esthetic perception of smiles with different gingival conditions.

This study aimed to compare the perceptions of smile esthetics among general practitioners, periodontists, and laypeople. Photographs of smiles were taken and digitally modified to create different gingival conditions: gingival asymmetry, unilateral gingival recession, and bilateral gingival recession. These photographs were evaluated by 150 volunteers (19-70 years old) using a 100.0-mm visual analog scale. The volunteers consisted of 50 laypeople with no knowledge of dentistry, 50 general dentists, and 50 periodontists. Analysis of variance and Student t test were performed when data presente…

Model-Based Orthodontic Assessments for Dental Panoramic Radiographs.

For better treatment outcomes, dentists usually use a set of parameters for orthodontic evaluation. In this study, a new method is proposed to assist dentists in obtaining reliable assessment of these parameters. The proposed method is based on dental panoramic radiographs and can be divided into four stages: image preprocessing, model training, tooth segmentation, and assessment of orthodontic parameters. The image is first normalized and enhanced. Then, the model training stage consists of shape and image model training, energy function training, and weight training. Next, we automatically s…

Medications' impact on oral health.

BACKGROUND: Over-the-counter and prescription drugs are used frequently, in large quantities and by many adults, particularly by those older than 65 years of age. A number of medications (prescription, over-the-counter, vitamins and minerals, herbal preparations) can affect oral health. With the population's aging, and as more drugs become available, dentists can expect to encounter medication-related oral side effects among their patients. TYPES OF STUDIES REVIEWED: The author reviewed studies that ranged from case reports to randomly controlled, double-blinded studies. However, in view of t…

Management of patients with facial paralysis in the dental office: A brief review of the literature and case report.

In the dental office, the dentist may have to examine patients with facial asymmetry and functional disorders caused by facial paralysis (FP). Following clinical examination, it is important for the dental practitioner to establish whether FP was caused by injury to the facial nerve, and to focus on the site of the lesion and potential risk factors. The risks of dental treatment in a patient with FP should also be assessed. Through dental or surgical procedures, the dentist may cause transient or permanent FP. Interdisciplinary collaboration is required for the confirmation of diagnosis and et

A critical view of the rationale for routine, initial and periodic radiographic surveys.

Most dentists and dental faculties use an initial radiographic survey for the comprehensive-care patient rather than selective radiography, as recommended by professional organizations and radiation protection agencies. Survey or screening radiography has been justified in the belief that it is an essential health service designed to disclose serious occult disease and that failure to use such films might expose the dentist to successful malpractice litigation. However, incidence data indicate the chance of disclosing tumors, non-inflammatory cysts or other serious bone disease in the asymptom…

A guide to recognizing maxillary sinus pathology and for deciding on further preoperative assessment prior to maxillary sinus augmentation.

Dentists contemplating sinus augmentation frequently take a cone beam computed tomography (CBCT) scan as part of the work-up. Maxillary sinus pathology is common, and such dentists will be faced with pathologic sinus findings. Not all sinus pathology needs to be treated. The purpose of this article is twofold: to describe the most common sinus findings as seen on a CBCT scan and a way to recognize them, as well as to provide an algorithm for decision making on when further evaluation of the sinus is necessary.

Unilateral facial swelling caused by Ramsay Hunt syndrome resembles odontogenic infection.

Facial cellulitis and swellings of the head and neck are worrisome signs of odontogenic infection, which can be life threatening. Most head and neck infections are caused by bacterial pathogens. When treating such infections, dentists must also be aware of possible viral or fungal causes and their associated presentations. This report documents a case of viral infection that initially resembled a bacterial odontogenic infection. It is intended to familiarize dentists with the Ramsay Hunt syndrome and the need for prompt recognition and early definitive treatment.

Clinical diagnosis and treatment of temporomandibular disorders in children and adolescents: a case series.

The early diagnosis of temporomandibular disorders (TMDs) in children and adolescents is important because they can affect oral and maxillofacial growth and development. This case series introduces patients with various clinical features of TMDs and demonstrates how symptoms were reduced through appropriate interventions in collaboration with oral medicine specialists and pediatric dentists. TMDs symptoms in children are often mild and difficult to express accurately; therefore, diagnosis through clinical evaluation is important. Pediatric dentists should be aware of TMDs in children and adole…

The clinical view for dissection of the lingual nerve with application to minimizing iatrogenic injury.

The importance of the position of the lingual nerve in the oral cavity cannot be understated for dentists and oral surgeons. Therefore, the location of the lingual nerve with various positions of the tongue is important. However, most dental and medical students are taught oral cadaveric anatomy where tissues are fixed. Therefore, the focus of this article is to demonstrate how the lingual nerve moves based on tongue movement in fresh tissues and how this is important for dentists and oral surgeons. Clin. Use of botulinum toxin in dentistry.

A growing number of dentists are providing botulinum toxin to patients. The research presented here outlines potential uses of Botox related to oral health and facial problems as compared to traditional treatment methods. The administration of Botox (historically done by dermatologists and neurologists) may fall under dentists' jurisdiction, as their training and knowledge encompasses the entire head and neck. A review is made of the literature, based on Ovid and PubMed searches, selecting articles describing the injection of botulinum toxin A in areas related to the oral cavity and the face, …
