Thirty-three patients with Mallory-Weiss syndrome have been treated at the Clinic of Emergency Surgery in Sofia for a period of 11 years (1980-1990). According to treatment and outcome they were classified as follows: conservative treatment--7 patients, electrocoagulation--16, sclerosing procedure--10, successful endoscopic hemostasis--17, emergency surgical intervention--9, relapse of bleeding after endoscopic surgery--9, death from hemorrhage--4, total case fatality--12.1 per cent, operative case fatality rate--33.3 per cent.

The effects of Arctotheca calendula (capeweed) on digestive function in the rumen, caecum and colon of sheep were studied. Three groups of 2-year-old Merino wethers were compared: 13 grazing capeweed-free pasture for 23 days; 8 fed 700 g of lucerne hay daily for 18 days, and 13 fed 6 kg of freshly cut capeweed (dry matter 11.3%) daily for 7 days. Capeweed did not cause diarrhoea and based on measurements of products of fermentation in the rumen, the plants were as nutritious as pasture containing grasses and clovers. The authors review the most urgent problems of alimentary diseases in children: diagnosis, differential diagnosis and early detection with the use of remote thermography, an up-to-date noninvasive and atraumatic research method. The method was demonstrated to be highly informative in diseases such as gastroduodenitis, peptic ulcer, chronic pancreatitis and others. The significance of remote thermography in revealing functional pathology of the alimentary organs in children is emphasized.

Seborrheic dermatitis (SD) is frequent in adults. Its aetiology is unknown, and the alleged causative effect of Pityrosporum-type yeasts is strongly controverted. Towards the end of 1988, 85 out of 170 patients indicated for endoscopic papillotomy (EPT) were over 65 years of age, and, of these, 54% were 75 and more. EPT was successful in 90.6% of those over 65. Total morbidity was 4.7% and mortality 1.2%. Endoscopic colorectal polypectomy (EPE) was performed during the same period in 591 subjects. 143 of them (24.2%) were over 65 and had a total of 284 polyps removed. Between April 1984 and October 1988, 10 patients with hepatocellular carcinoma who bled from esophageal varices were included in a polidocanol sclerotherapy program, after the cessation of bleeding. Sixty cirrhotic patients without hepatocellular carcinoma were included as controls in the same sclerotherapy program for the same period. According to Okuda's classification, 1 patient was grade I, and 9 were grade II. Seven cases of upper gastrointestinal hemorrhage after gastroduodenal surgery are presented; in all the cases the pyloric region had been resected or excluded. The gastritis was documented by endoscopy and/or surgery; it was located in the area proximal to the anastomosis. There were no other causes of bleeding. Pathologico-morphological changes in the digestive tract were investigated in fifty naturally infected common foxes (Vulpes vulpes crucigera). Nineteen animals were found to suffer from mild cachexia, 31 animals suffered from complete cachexia. Erosions were observed in the mucosa of oral cavities of 21 foxes. A case is presented of massive lower intestinal bleeding due to typhoid fever. Because of uncontrollable bleeding, emergency right hemicolectomy and resection of the last 50 cm of the distal ileum was performed after endoscopy and arteriography, both of which were negative. The histology of the lesions typical of this disease is described.

Dysbiosis does not necessarily mean disease. Typical clinical picture of disease caused by metabolic activity of dysbiotic microflora is the microbial over-growth syndrome. We studied case histories of 434 patients without consideration of their basic illnesses and correlated them with types of dysbioses of duodenal juice. 55% of 314 patients with gastrointestinal disorders revealed a dysbiosis.
