Predictive factors for conservative treatment failure of acute complicated diverticulitis: a single-center retrospective analysis.

According to WSES guidelines conservative/non-operative treatment is the first-line therapy for acute complicated diverticulitis (ACD) but failure may represent a challenge in terms of hospitalization, medical costs, morbidities, and mortality. This study aimed to evaluate failure rates following non-operative management of acute complicated diverticulitis and to examine possible predictive factors for failure. We retrospectively analyzed all patients with a diagnosis of diverticulitis consecutively admitted to our Institution from 1 January 2017 to 31 December 2021. Patient demographics, bloo…

[Clinical cases of complicated diverticulitis against the background of severe course COVID-19. Case report].

Two clinical cases of perforation of a previously undiagnosed colon diverticulum in patients with coronavirus infection caused by the SARS-CoV-2 virus treated at the Hospital №1 of Nalchik. Both patients were elderly, overweight, had a lot of chronic concomitant diseases. Patients received hormone therapy and were targeted: the first patient twice (tocilizumab on the first day of hospitalization and olokizumab on the 7th day of inpatient treatment). The second patient received levilimab on the 3rd day of his stay in the hospital. A short time after targeting, both patients developed acute diff

Multiple inflammatory polyps resulted from Meckel's diverticulitis presenting with chronic abdominal pain: A case report.

INTRODUCTION AND IMPORTANCE: In most diverticulitis of Meckel's Diverticulum (MD), perforation, obstruction and hemorrhage causing acute abdomen are seen, however, multiple inflammatory polyps(IPs) are rare, which are seen in our case. CASE PRESENTATION: We present the case of 28-year-old female, who was admitted in the clinic for intermittent abdominal pain for 5 years. Histopathology suggested MD chronic diverticulitis and multiple inflammatory polyps. CLINICAL DISCUSSION: Our patient presented was diagnosed preoperatively with suspected MD, and an laparoscopy was indicated. Intraoperative…

Transvaginal hybrid-NOTES vs. traditional laparoscopic sigmoid resection for diverticulitis: a short-term comparative study.

The aim was to compare short-term results of transvaginal hybrid-NOTES (NSR) with traditional laparoscopic technique in sigmoid resection (LSR) in cases of diverticulitis. Natural Orifice Transluminal Endoscopic Surgery has been evolved as a minimally invasive procedure to reduce the operative trauma due to the absence of specimen extraction through the abdominal wall causing less postoperative pain, and shorter hospital stay. Despite the increasing use and published case series of NSR for diverticulitis as a laparoscopic procedure with transvaginal stapling and specimen extraction, there are …

Differentiation between diverticulitis and colorectal cancer: quantitative CT perfusion measurements versus morphologic criteria--initial experience.

PURPOSE: To determine whether computed tomographic (CT) perfusion measurements in prospectively recruited patients can be used to differentiate between diverticulitis and colorectal cancer and to compare this discrimination with that of standard morphologic criteria. MATERIALS AND METHODS: After institutional review board approval and written informed consent were obtained, 60 patients (24 men, 36 women; mean age, 69 years; range, 33.5-90.4 years; 20 patients with cancer, 20 with diverticulitis, and 20 with inactive diverticular disease) underwent CT perfusion imaging at the level of the colo…

Laparoscopic resection of sigmoid diverticulitis. Results of a multicenter study. Laparoscopic Colorectal Surgery Study Group.

BACKGROUND: In the large bowel, resection of the sigmoid colon is the most commonly performed laparoscopic intervention because large bowel lesions often are located in this part of the bowel and the procedure technically is the most favorable one. A number of publications involving case series or the results of highly experienced individual surgeons already have confirmed the feasibility of laparoscopic resection in cases of diverticulitis. The aim of the present prospective multicentric investigation was to check the results obtained by a large number of surgeons performing laparoscopic rese…

Efficacy of Short- vs Long-Course Antibiotics in Treatment of Hinchey Stage 3 and 4 Diverticulitis.

BackgroundThis study aimed to evaluate outcomes of different antibiotic durations in patients with severe diverticulitis.MethodsInitial electronic medical record review identified 2437 adult patients who underwent sigmoid colectomy. Manual chart review identified 133 patients who underwent sigmoid resection and proximal diversion for Hinchey stage 3 or 4 diverticulitis. Patients who received a short antibiotic course (SAC, ≤5 days) were compared with those who received a long antibiotic course (LAC, >5 days). The primary composite outcome included superficial, incisional, organ-space infection…

Diverticulitis-Associated Mesenteric Venous Thrombophlebitis: A Case Report and Systematic Review of Diagnosis, Management, and Outcomes.

Diverticulitis can rarely seed the portal‑mesenteric venous system, causing septic thrombophlebitis with morbidity and management uncertainty. This study systematically reviewed diverticulitis‑associated mesenteric venous thrombophlebitis to summarize its presentation, diagnostics, treatment, and outcomes. Twenty‑eight single‑patient reports met the selection criteria. The mean age was 54 years; fever and abdominal pain predominated. Contrast‑enhanced CT established the diagnosis in most patients, usually showing sigmoid diverticulitis with inferior mesenteric or portal‑mesenteric vein thrombu…

[A Case of Adenocarcinoma Arising from Meckel's Diverticulum with Liver Metastases Diagnosed by CT for Suspected Diverticulitis].

A man in his 40s presented with fever and right lower abdominal pain. Laboratory data showed elevated inflammatory markers. Abdominal CT showed a 3 cm mass in the mid-lower abdomen with fluid retention and gas continuity with the small bowel, consistent with diverticulitis. Multiple hepatic nodules were also noted. After antibiotic therapy, laparoscopic surgery was performed on hospital day 3. Intraoperatively, a firm mass was found approximately 30 cm proximal to the ileocecal valve, with extensive adhesions to the mesentery and omentum. Multiple white nodules were observed in both hepatic lo…

Diagnostic Performance of a Next-Generation Virtual/Augmented Reality Headset: A Pilot Study of Diverticulitis on CT.

Next-generation virtual/augmented reality (VR/AR) headsets may rival the desktop computer systems that are approved for clinical interpretation of radiologic images, but require validation for high-resolution low-luminance diagnoses like diverticulitis. The primary aim of this study is to compare diagnostic performance for detecting diverticulitis on CT between radiologists using a headset versus a desktop. The secondary aim is to survey participating radiologists about the usage of both devices. This pilot study retrospectively included 110 patients (mean age 64 ± 14 years, 62 women) who had …

Right-sided acute diverticulitis in a North African country: Presentation and management in one surgical center.

PURPOSE: Right-sided diverticulitis is a rare entity in North Africa. Therefore, it is usually misdiagnosed, and the management of cecal diverticulitis is aggressive in most cases, whereas nowadays, most studies recommend a conservative approach with promising results. This study aims to describe the presentation, management, and outcomes of right-sided diverticulitis (RSD) and to present the experience of one surgical center in Tunisia. METHODS: This is a retrospective study including all patients presented with RSD, in the Department of Surgery "A" of the Charles Nicolle Hospital between 20…

Managing Perforated Diverticulitis: An Overview of Treatment Trends and Clinical Outcomes at a Single Centre in the United Kingdom.

Introduction Perforation represents the most critical manifestation of complicated diverticulitis. In 2008, it was estimated that about 2,000 cases of perforated diverticulitis (PD) were diagnosed in the United Kingdom (UK). Management of PD is evolving with considerable variation in approaches between hospitals and countries with an increased trend towards a conservative approach. Objective Our aim is to provide a comprehensive overview of the management strategies and treatment outcomes for PD, with a particular focus on the influence of abscess size and the presence of distant air (DA) on t…

Influence of Socioeconomic Deprivation on Surgical Outcomes for Patients With Sigmoid Diverticulitis in France: A Multicenter Retrospective Study.

OBJECTIVES: To evaluate the relationship between socioeconomic deprivation and postoperative outcomes in patients who underwent colonic resection for sigmoid diverticulitis (SD). BACKGROUND: The potential impact of socioeconomic inequalities on the management of SD has been scarcely studied in the literature. Considering other gastrointestinal pathologies for which lesser access to optimal treatment and poorer survival have been shown, we hypothesize that deprivation could be associated with outcomes for SD. METHODS: This multicenter retrospective study was conducted at 41 French hospitals b…

Perforated Jejunal Diverticulum Mimicking Diverticulitis: A Case Report of Acute Abdominal Pain in an Elderly Patient.

Jejunal diverticulosis is a rare form of diverticulosis characterized by acquired pseudodiverticula in the small bowel. Although most cases are asymptomatic, the condition can present diagnostic challenges due to its atypical presentation. Complications such as perforation can lead to acute abdomen, significantly increasing morbidity and mortality. We report a rare case of perforated jejunal diverticulitis in an 84-year-old female with a known history of diverticular disease. The patient presented to the emergency department with worsening left lower quadrant abdominal pain. A computed tomogra…

Meta-analysis of Randomized Clinical Trials on Long-term Outcomes of Surgical Treatment of Perforated Diverticulitis.

OBJECTIVE: To assess long-term outcomes of patients with perforated diverticulitis treated with resection or laparoscopic lavage (LL). BACKGROUND: Surgical treatment of perforated diverticulitis has changed in the last few decades. LL and increasing evidence that primary anastomosis (PRA) is feasible in certain patients have broadened surgical options. However, debate about the optimal surgical strategy lingers. METHODS: PubMed, Scopus, and Web of Science were searched for randomized clinical trials (RCT) on surgical treatment of perforated diverticulitis from inception to October 2022. Long…
