INTRODUCTION: Traditional management of uncomplicated acute diverticulitis (UAD) includes antibiotic therapy, nonsteroidal anti-inflammatory drugs (NSAIDs), and dietary restriction, albeit recent evidence questions their benefit. The aim of the present study was to evaluate the safety and efficacy of an outpatient protocol without antibiotic therapy, NSAIDs, or dietary restriction.
METHODS: A single-center prospective study was conducted within the time frame of 2022 and 2024. Patients with UAD were divided into a historic cohort (HC), treated with the traditional protocol, and a study group

We present a comparison of the mucosal microbiota within different diverticula in a patient with diverticular disease (DD) complicated by diverticulitis and pelvic abscess. The conventional culture method and the 16S rRNA-based sequencing approach were employed to characterize the microbiota of perforated diverticulum (PD) and adjacent visually intact diverticulum (ID) from the same surgically resected colonic segment. Compared to PD, the microbiota of ID demonstrated depletion in butyrate-producing genera and increased abundances of Proteobacteria, Enterobacteriaceae, and Bacteroides. The pre

INTRODUCTION AND IMPORTANCE: Diverticula of the vermiform appendix are rare entities. Although the pathogenesis and natural course of appendiceal diverticulitis (AD) are different from acute appendicitis, AD is treated like acute appendicitis because of similar clinical manifestations and low incidence.
CASE PRESENTATION: We describe cases of two male patients of different ages who respectively underwent elective laparoscopic appendectomy and an emergent laparoscopic appendectomy in a multi-speciality hospital in Kerala, India. Both of them had acute appendicitis, as confirmed by imaging and

AIM: The role of laparoscopic surgery in the management of patients with diverticular disease is still not universally accepted. The aim of our study was to evaluate the results of laparoscopic surgery for diverticular disease in a centre with a specialist interest in minimally invasive surgery.
METHOD: All diverticular resections carried out between 2006 and 2010 were reviewed. Data recorded included baseline demographics, indication for surgery, operative details, length of hospital stay and complications. Complicated diverticular disease was defined as diverticulitis with associated absces

Traditionally, surgical sigmoid diverticular emergencies used to be treated in stages, but more recently there has been a trend towards definitive surgery with immediate resection plus anastomosis under certain conditions. The aim of this study was to define the morbidity and mortality of resection plus anastomosis with on-table antegrade irrigation and of the Hartmann procedure for complicated sigmoid diverticulitis in relation to the type of peritonitis and to the American Society of Anesthesiologists (ASA) grade of the patients. From April 1999 to April 2002, 38 emergency operations for com

AIM: The aim of our prospective dynamic cohort trial is the evaluation of indication for surgery for diverticular disease and the evaluation of morbidity and mortality.
MATERIAL AND METHOD: All patients operated for diverticular disease and its complications were involved in the study. The conservatively treated patients were not involved. 104 patients with diverticular disease and its complication were operated from August 2007 till July 2008.46 men and 58 women at average age of 63.9 (31-85) years were in this group. 78 patients were electively operated in noninflammatory stage of diverticu

Diverticulosis is the most common structural abnormality of the colon and affects up to one third of those over the age of 60 years. Its complications account for more than 3,000 deaths per year. Acute diverticulitis is the most common complication and 10-25% of patients with diverticulosis may have one or more episodes. Patients present with left-sided abdominal pain that localises in the left iliac fossa. This left-sided pain reflects the underlying predominant left-sided distribution of the diverticula on this side. Patients may also present with right-sided or suprapubic pain, caused eithe

A 61-year-old female complaining of arthralgia was repeatedly treated with antibiotics and also with prednisolone. A chronic polyarthritis was assumed. In hospital, leukocytosis of 21.000 was found one day before death as well as moderate anemia. Colonoscopy was rejected by the patient. A computer tomography revealed destructive arthritis of the symphysis, vertebral osteochondrosis L5/S1, and sigmoid diverticulosis. The patient died with clinical signs of central dysregulation. At autopsy, a covered perforation of a sigmoid diverticulum with purulent peridiverticulitis was found. The 5th lumba

Segmental colitis associated with diverticulosis (SCAD) is a rare inflammatory condition limited to colonic segments affected by diverticulosis. Its coexistence with ulcerative colitis (UC) is extremely rare and poses a diagnostic and therapeutic challenge. We report the case of a 56-year-old woman with a history of sigmoidectomy for recurrent sigmoid diverticulitis, who subsequently developed SCAD in the residual diverticula adjacent to the anastomosis, along with concomitant left-sided UC. Despite treatment with mesalazine and vedolizumab and rectal healing of UC, SCAD was therapy-refractory

Twenty-six patients who were 60 years of age or older underwent a surgical procedure for inflammatory bowel disease (IBD). Twenty patients had involvement of the colon (14 had ulcerative colitis and six, Crohn's disease). The indications for operation were intractable colitis (eight patients), perforation (three patients), stricture (three patients), toxic megacolon (three patients), anorectal disease (two patients) and carcinoma (one patient). Operations performed were abdominal colectomy (ten patients), segmental colectomy (seven patients), local anorectal procedures (two patients) and total

All patients with left colon obstruction or infection admitted under the care of one surgical firm over a 2.5-year period underwent on-table colonic lavage and primary anastomosis. Results were compared with all other similar patients treated during the same period in the same health district (involving a stoma in all cases). No significant difference in perioperative mortality or morbidity between primary anastomosis and Hartmann's groups was demonstrated. Hartmann's procedure necessitated a stoma for over 2 months with an 11% incidence of stoma-related complications. The stoma was not revers

OBJECTIVE: The aim of this study was to assess CT-colonography (CTC) in the follow-up of diverticulitis regarding patient acceptance and diagnostic accuracy for diverticular disease, adenomas and cancer, with colonoscopy as a reference standard.
METHODS: A prospective comparative study where half of the patients underwent colonoscopy first, followed immediately by CTC. The other half had the examinations in the reverse order. Patient experiences and findings were registered after every examination, blinded to the examiner.
RESULTS: Of a total of 110 consecutive patients, 108 were included in

This is a case of a 53-year-old African American woman with newly diagnosed concomitant acute cholecystitis, diverticulitis, and SARS-COV-2 infection. She underwent treatment for COVID-19 with antibiotics and supportive treatment, and on day 32 after initial symptoms presented, she had an elective laparoscopic cholecystectomy. Our patient was one of the first cases to present with acute cholecystitis and diverticulitis complicated by COVID-19, and serves as a template for surgical management of non-emergent abdominal pathologies in the midst of the COVID-19 pandemic.

INTRODUCTION: The surgical management of perforated sigmoid diverticulitis and generalised peritonitis is challenging. Surgical resection is the established standard of care. However, there is debate as to whether a primary anastomosis (PA) or a Hartmann's procedure (HP) should be performed. The aim of the present study was to perform a review of the literature comparing HP to PA for the treatment of perforated sigmoid diverticulitis with generalised peritonitis.
METHODS: A systematic literature search was performed for articles published up to March 2018. We considered only randomised contro

BACKGROUND: Laparoscopic colorectal surgery has become the gold standard in the therapy of benignant and malignant colorectal pathologies. Anastomotic leakage is still a reason for laparotomy; applying a diverting stoma or performing a Hartman's procedure is common [1, 2]. Laparoscopic treatment of an early-detected anastomotic leakage is suggested from other authors [3, 4]. In our video we demonstrate a combined minimal invasive transabdominal and transanal treatment concept in patients with early-detected anastomotic leakage.
METHODS: Two consecutive patients developing an anastomotic leaka
