The chemokine interleukin 8 (IL-8) was instilled into the round window niche of rats through a small perforation in the tympanic membrane in order to study its effect on inner ear function by electrophysiological and morphological techniques. The frequency-specific auditory brainstem response (ABR) was recorded at the frequencies 4, 8, 10, 12, 16 and 20 kHz just before and 1, 2, 5 and 14 days after instilling IL-8 to ascertain the hearing level during each interval. Morphological examination by light microscopy was performed during the same interval following the instillation of IL-8. On day 1…

One hundred and fifty-eight patients with congenital microtia and atresia were treated with simultaneous total ear reconstruction and hearing rehabilitation including meatoplasty, tympanoplasty as well as fenestration. Using this operation, the authors have reconstructed 160 auricles and simultaneously restored their auditory function. After the operation, the hearing gain reached 20-45 dB in 132 cases (82.5%). The success rate of the reconstructed ear was 158/160 (98.2%). Most of the reconstructed ears were of excellent appearance.

Klippel-Feil syndrome is characterized by a short-neck and a low occipital hairline due to such deformaties of the cervical spine as aplasia, dysplasia and fusions of the cervical and thoracic vertebrae. Approximately 1/3 of cases have sensorineural deafness, although occasional unilateral or bilateral conductive hearing loss due to middle ear malformations have been described. We have now treated two cases with unilateral middle ear malformations in patients with known Klippel-Feil syndrome. In the first case a completely malformed stapes was found as well as an atypical course of the facial …

We report a rare case of relapsing polychondritis with an initial symptom of inner ear involvement. This 53-year-old Japanese man experienced a hearing difficulty, tinnitus in both ears, and dizziness of sudden onset, but lacked auricular chondritis at that time, which is the most frequent finding in relapsing polychondritis. Thus it was difficult to reach a correct diagnosis. Steroid therapy, with oral prednisolone 15 mg daily, was effective. Almost two months after we began the steroid therapy, the patient complained of losing interest in his work and reported a hallucination vision on the T…

We study several cases of patients who presented clinical symptoms indicating the presence of a middle ear mass (tinnitus, hearing loss, otorrhea, otorrhagia...). We had to rule out the presence of an expanding tumour (either in the form of a cholesteatoma or a glomus tumor) and the existence of vascular anomalies of the middle ear (enlarged jugular bulb or jugular megabulb deformity). Due to the possibility of a different treatment in each particular case (radiotherapy, radical surgery, jugular vein ligation or even no treatment at all), the relative value of various imaging techniques and/or…

Transiently evoked otoacustic emissions (TEOAE) recorded in 98 ears were compared depending on the results of tympanometry. The values of middle ear pressure in patients without detectable TEOAE are significantly smaller compared to those of patients with detectable TEOAE. In cases of detectable TEOAE, the amplitudes and frequency components were compared to the results of tympanometry. When maximal compliances are shifted towards negative values of middle ear pressure, the amplitudes of TEOAE are reduced with a loss of low frequency components. Small compliance values result in reduced amplit…

Arthrogryposis multiplex congenita (AMC) is a rare congenital syndrome consisting of two or more joint contractures in different parts of the body. Otolaryngological abnormalities have been previously described in a significant proportion of cases. However, to date, the only frequent otologic deformity recorded in the published literature has been low-set ears. We present a case with a combination of AMC and a bilateral conductive hearing loss due to an unusual middle ear deformity. To our knowledge, no such case has been previously reported.

Adenomas of the middle ear are quite rare tumours of the ear and of recent recognition. Two cases of adenomatous neoplasms confined to the middle ear are presented. Usually these tumours are located on the promontorium or in the hypotympanon. The unique feature of the first described case is based on its origin in the epitympanic pouch of the tympanic membrane. The second case originated from the mucosa in the hypotympanon. The third case is a tumour-like lesion which was not distinguishable on clinical grounds from a jugulo-tympanic paraganglioma or a middle ear adenoma. The adenomas of the m…

Quite often a sudden surdity can appear after baro or mechanical traumas, it happens because of rupture of the round window membrane which leads to the formation of the perilymphatic fistula. The article deals with the syndrome of the round window membrane rupture. The authors describe two successful cases of treatment of patients with vestibular disorders caused by posttraumatic perilymphatic fistula. Practical recommendations are given for providing a tertiary medical care in emergency situations.

Inner ear pathology was studied in adult rats with lipoid nephrosis induced by puromycin aminonucleoside. Although no abnormality was observed in auditory brain-stem responses, significant changes were noted in the stria vascularis. The most striking observation was that intermediate cells were markedly swelled, there-by pressing adjacent marginal cells. Severely affected marginal cells have vacuoles and increased lysosomes and protruded toward the endolymphatic space. The organ of Corti remained virtually intact. Although the vestibular maculae were relatively normal, type I hair cells in the…

Recently, it has been suggested that otosclerosis represents the host's ongoing immunologic response to measles or other viral antigens. Documentation of past inflammation within the inner ear would serve as further evidence that this mechanism may be at play in the pathogenesis of the disease. Among the characteristic signs of prior inflammation in the inner ear is the presence of lamellar bone at the site of inflammation. This has been described in the temporal bone of a patient with immune-mediated deafness and with the temporal bones of experimental models of immune-mediated inner ear dise…

Localized osteoclastic bone resorption is responsible for the pathological changes within the middle and inner ear, which result in hearing loss and vertigo in chronic otitis media and otosclerosis. The local control of osteoclastic bone resorption is incompletely understood. Various small, locally active molecules, cytokines, have been shown to affect resorptive processes. Additionally, prostaglandins and their inhibitors have been shown to modulate the resorptive process in a number of in vitro studies. In this study, indomethacin, a cyclooxygenase inhibitor, was tested in a model of localiz…

A series of Swedish studies has shown that the application of middle ear over-pressure may be effective in ameliorating the symptoms associated with Meniere's disease. Other studies have shown that electrocochleography (ECochG) is an effective tool in the identification of Meniere's disease, especially in the presence of active symptoms. Based on the therapeutic potential of middle ear overpressure as a treatment for Meniere's symptoms and on the relationship between these symptoms and ECochG, we investigated the effects of ear canal pressure change on human electrocochleographic components in…

Otoacoustic emissions are sounds emitted by the inner ear. Their genesis requires the outer hair cells of the organ of Corti to be intact. Analysis of these sounds provides a fine evaluation of cochlear function. Evoked otoacoustic emissions (in response to a sound) are constant in subjects with normal hearing, whereas they always have pathological characteristics in patients with endocochlear conduction or perceptive deafness. When the acoustic nerve or the central nervous system are affected, otoacoustic emissions are normal for as long as the pathological process has no repercussions on the…

The purpose of this study was to develop a high-risk clinical profile of patients with autoimmune (immune-mediated) inner-ear disease. The records of 52 patients diagnosed over the past 5 years were reviewed. Age, sex, bilateral versus unilateral involvement, otologic symptoms, concomitant systemic immune disease, and presenting clinical diagnoses were recorded. The presenting diagnoses were Cogan's syndrome, Meniere's syndrome, Dandy's syndrome without hearing loss, or progressive sensorineural hearing loss without dizziness. Because Cogan's and Dandy's syndromes were relatively uncommon, the…
