[Standards in pediatric pacemaker therapy].

Despite differences between pediatric and adult pacing have narrowed, implantation procedures in children require special considerations. Patients are physically smaller, they often suffer from complex cardiac defects necessitating further open heart surgery procedures. As there are no pacemakers and only some electrodes designed specially for pediatric pacing, selection of appropriate systems is important. At this time we care for 18 patients with a mean age of 11.5 +/- 6.5 years (= 138.6 +/- 78.4 months, range 10 to 280 months). 50% of them received a permanent pacemaker because of surgicall

Reproducible stimulation of ciliary muscle contraction in the cynomolgus monkey via a permanent indwelling midbrain electrode.

To better study the pivotal role of the ciliary muscle in ocular accommodation and aqueous humor outflow, a technique was developed to permit long-term reproducible stimulation of the muscle via the normal efferent neuronal pathway in cynomolgus monkeys. Following a small midline craniotomy, the stereotaxic coordinates of the Edinger-Westphal (E-W) nucleus were localized by contrast ventriculoradiography. A bipolar stimulating electrode was stereotaxically positioned to generate the maximum accommodative amplitude, and then permanently affixed to the surrounding cranium. Once experience was ga

Radiographic imaging for the cochlear implant.

Imaging plays an essential role in evaluating the cochlear implant candidate and the results of implantation. Eighteen candidates for cochlear implantation were reviewed for this study. Fourteen received implants; all but one, intracochlear devices. Based on the trial use of multiple techniques, a protocol has been established for the use of radiography in implant studies. The protocol suggests preoperative high resolution semiaxial computed tomographic scanning, intraoperative anteroposterior transorbital plain films if there is doubt concerning the electrode placement, and postoperative plai

An active electronic, high-density epidural paddle array for chronic spinal cord neuromodulation.

Objective. Epidural electrical stimulation (EES) has shown promise as both a clinical therapy and research tool for studying nervous system function. However, available clinical EES paddles are limited to using a small number of contacts due to the burden of wires necessary to connect each contact to the therapeutic delivery device, limiting the treatment area or density of epidural electrode arrays. We aimed to eliminate this burden using advanced on-paddle electronics.Approach. We developed a smart EES paddle with a 60-electrode programmable array, addressable using an active electronic mult

Accuracy, Repeatability, and Reproducibility of a Radiographic Technique to Assess Spinal Cord Stimulation Lead Position: A Validation Study.

Spinal cord stimulation with implantable leads is a valuable therapy used to treat a variety of chronic pain conditions. However, lead migration is a common complication causing loss of efficacy. Previous reports have characterized lead migration using radiographs, but methods are not consistent and lack rigorous validation. The purpose of this study was to develop a technique to perform radiographic measurements of the position of epidural spinal cord leads within the lumbosacral spinal canal and establish its accuracy, repeatability, and reproducibility. Computed tomography scans were acquir

Musical pitch perception with electrical stimulation of the cochlea.

Studies were undertaken to investigate the ability of a user of the Nucleus multi-electrode cochlear implant to judge pitch in the context of musical intervals. The subject had qualified as a musical instrument tuner before he received his implant, and was able to judge the intervals between electrical sensations with neither training nor the guidance of familiar melodies. The procedures used were interval estimation, and interval production by the method of adjustment. The pitch of the electrical stimulation was controlled by varying the pulse repetition rate, the active electrode position, o

NeuroBus - Architecture for an Ultra-Flexible Neural Interface.

This article presents the system architecture for an implant concept called NeuroBus. Tiny distributed direct digitizing neural recorder ASICs on an ultra-flexible polyimide substrate are connected in a bus-like structure, allowing short connections between electrode and recording front-end with low wiring effort and high customizability. The small size (344 μm × 294 μm) of the ASICs and the ultraflexible substrate allow a low bending stiffness, enabling the implant to adapt to the curvature of the brain and achieving high structural biocompatibility. We introduce the architecture, the integra

Neural response telemetry and auditory/nonauditory sensations in 15 recipients of auditory brainstem implants.

Auditory brainstem implants (ABIs) provide a means of restoring some hearing sensations to individuals with neurofibromatosis type 2 (NF2) who are deaf after vestibular schwannoma removal. In this study, neural response telemetry (NRT) was used to record electrically evoked neuronal activity near the ABI electrode array in 15 such subjects. Our interest was to investigate whether NRT recordings from the brainstem might be useful in implanting or programming ABIs. We therefore sought relationships between postoperative NRT recordings and the sensations reported by the subjects in response to th

Effects of iron accumulation and its chelation on oxidative stress in intracortical implants.

Long-term reliability of microelectrodes implanted in the cortex is hindered due to the foreign body response that occurs at the electrode-tissue interface. Following implantation, there is disruption of the blood-brain-barrier and vasculature, resulting in activation of immune cells and release of erythrocytes. As a result of hemolysis, erythrocytes degrade to heme and then to free iron. Excess free iron can participate in the Fenton Reaction, producing reactive oxygen species (ROS). Iron-mediated ROS production can contribute to oxidation of lipids, proteins, and DNA, facilitating a hostile

Investigation of a chondroitin sulfate-based bioactive coating for neural interface applications.

Invasive neural implants allow for high-resolution bidirectional communication with the nervous tissue and have demonstrated the ability to record neural activity, stimulate neurons, and sense neurochemical species with high spatial selectivity and resolution. However, upon implantation, they are exposed to a foreign body response which can disrupt the seamless integration of the device with the native tissue and lead to deterioration in device functionality for chronic implantation. Modifying the device surface by incorporating bioactive coatings has been a promising approach to camouflage th

Monitoring Cochlear Nerve Integrity During Vestibular Schwannoma Microsurgery in Real-Time Using Cochlear Implant Evoked Auditory Brainstem Response and Streaming Neural Response Imaging.

OBJECTIVE: Intraoperative far-field auditory brainstem response (ABR) and direct cochlear nerve action potential monitoring using neural response imaging (NRI) are techniques for monitoring the cochlear nerve during vestibular schwannoma (VS) surgery. A new paradigm has developed where the cochlear nerve is anatomically preserved during tumor removal to facilitate cochlear implantation in select circumstances. This report describes the use of an unmodified commercial cochlear implant (CI) to elicit electrically-evoked direct cochlear nerve and far-field ABR monitoring to evaluate the status of

Chronic neural probe for simultaneous recording of single-unit, multi-unit, and local field potential activity from multiple brain sites.

OBJECTIVE: Drug resistant focal epilepsy can be treated by resecting the epileptic focus requiring a precise focus localisation using stereoelectroencephalography (SEEG) probes. As commercial SEEG probes offer only a limited spatial resolution, probes of higher channel count and design freedom enabling the incorporation of macro and microelectrodes would help increasing spatial resolution and thus open new perspectives for investigating mechanisms underlying focal epilepsy and its treatment. This work describes a new fabrication process for SEEG probes with materials and dimensions similar to
