[Long lasting intracranial artery stenosis of embolic origin].

INTRODUCTION: The term evanescent, or vanishing, has been used to describe those intracranial stenoses that are consecutive to the partial recanalization of an embolic occlusion. In general terms it has been admitted that those stenoses quickly disappear in parallel to the complete recanalization of the artery. This behaviour allows their differentiation from the atherosclerotic stenoses. CASE REPORT: We present here a 61-year-old male who suffered a middle cerebral artery territory stroke secondary to an atrial fibrillation. In the acute phase, a stenotic flow was found in the proximal porti

Transient ischaemic attack due to incidental late electrical isolation of the left atrial appendage: a case report.

BACKGROUND: Electrical isolation of the left atrial appendage (LAA) during catheter ablation may increase the risk of thrombo-embolism, even in patients with low stroke risk scores and sinus rhythm. Late incidental isolation of the LAA has been rarely reported as a cause of transient ischaemic attack (TIA). CASE SUMMARY: A 73-year-old woman with a CHADS2 score of 0 underwent three catheter ablation sessions for long-standing persistent atrial fibrillation. Incomplete anterior mitral line ablation during the first and second sessions led to recurrent mitral flutter. At the third session, mitra

Thromboembolism associated with atrial fibrillation as a cause of limb and organ ischemia.

Atrial fibrillation (AF) is associated with a high risk of thromboembolic episodes. Stroke is the most common embolic complication of AF. Consequently, the majority of the conclusions regarding the association between AF and embolism are based on clinical studies of patients with ischemic stroke. The AF-related thromboembolism of limbs and visceral arteries is rarely recognized, and there is little data on this matter. For this reason it is often a neglected issue. Nevertheless, AF is diagnosed in 60-95% of patients operated on due to acute limb ischemia, 31% of patients with splenic artery em

Mechanisms of in-hospital acute ischemic stroke and their relevance to prognosis: A retrospective analysis.

OBJECTIVES: In-hospital stroke (IHS) is common and has a poor prognosis. Limited data were about the mechanisms of IHS, posing a challenge in taking measures to prevent stroke during hospitalization. This study aims to investigate the mechanisms of IHS and their relevance to prognosis. MATERIALS AND METHODS: Patients with in-hospital acute ischemic stroke at Peking Union Medical College Hospital from June 2012 to April 2022 were consecutively enrolled. The Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification of stroke and detailed mechanisms were evaluated by two experienced ne

Management of right-to-left shunt in cryptogenic cerebrovascular disease: results from the observational Austrian paradoxical cerebral embolism trial (TACET) registry.

Paradoxical embolism due to a patent foramen ovale (PFO) is a possible cause of ischemic stroke, particularly in young cryptogenic stroke patients. In most cases, however, it is difficult to establish a firm etiological association and the debate about management is ongoing. The Austrian Paradoxical Cerebral Embolism Trial was designed as a prospective, national, multi-center, non-randomized registry to add further data on this topic before the completion of randomized controlled trials. Over 27 months 188 cryptogenic stroke/TIA patients ≤55 years were entered by 15 Austrian stroke units. Cont

Myocardial infarction following t-PA for acute stroke.

BACKGROUND AND PURPOSE: Complications of intravenous (IV) thrombolysis with tissue plasminogen activator (t-PA) for acute stroke are commonly related to hemorrhage, anaphylaxis, or arterial re-occlusion. Embolic complications of t-PA are beginning to be recognized with increased use of t-PA for acute ischemic stroke. We hypothesize that disruption of intra-cardiac thrombus may result in myocardial infarction (MI) after use of t-PA for acute ischemic stroke. SUMMARY OF CASES: We describe three cases of acute MI immediately following IV t-PA infusion for acute stroke. In patient #1 apical throm

Stroke following injection of a melted suppository.

BACKGROUND AND PURPOSE: Stroke following the intravenous injection of drugs may be secondary to the direct effect of the substance itself or from embolization by the inert vehicle used in the manufacturing process of the drug. CASE DESCRIPTION: We describe a 39-year-old man who developed a left homonymous hemianopia after intravenous injection of a melted suppository. CONCLUSIONS: Paradoxical fat embolism should be added to the possible list of stroke mechanisms in intravenous drug abusers.

Outcomes of endovascular occlusion and stenting in the treatment of carotid blowout.

BACKGROUND AND PURPOSE: Carotid blowout is a life threatening complication of invasive head and neck cancers and their treatments. This is commonly treated with endovascular embolization and carotid stenting. Using the Nationwide Inpatient Sample, we report the immediate clinical results of patients receiving embolization and/or stenting for treatment of carotid blowout associated with head and neck cancer. MATERIALS AND METHODS: Using the Nationwide Inpatient Sample from the period 2003-2011, we defined carotid blowout patients as those with head and neck malignancies receiving carotid stent

Complications of Infective Endocarditis: A Case Report of Intracerebral Hemorrhage Exacerbated by Enoxaparin.

Infective endocarditis (IE) can cause life-threatening intracerebral hemorrhage via the transformation of an embolic ischemic stroke. Navigating anticoagulant therapy for IE patients is challenging due to this risk. Hospitalized patients often receive anticoagulation to minimize venous thromboembolism (VTE). Those at higher VTE risk may require full anticoagulation, particularly if there is an initial suspicion of a blood clot. A timely IE diagnosis is crucial but is often delayed during inpatient stays, with the patient potentially already on anticoagulants for other conditions. Our case disc

Study design of the CLOSURE I Trial: a prospective, multicenter, randomized, controlled trial to evaluate the safety and efficacy of the STARFlex septal closure system versus best medical therapy in patients with stroke or transient ischemic attack due to presumed paradoxical embolism through a patent foramen ovale.

BACKGROUND AND PURPOSE: Some strokes of unknown etiology may be the result of a paradoxical embolism traversing through a nonfused foramen ovale (patent foramen ovale [PFO]). The utility of percutaneously placed devices for treatment of patients with cryptogenic stroke or transient ischemic attack (TIA) and PFO is unknown. In addition, there are no clear data about the utility of medical interventions or other surgical procedures in this situation. Despite limited data, many patients are being treated with PFO closure devices. Thus, there is a strong need for clinical trials that test the pote

Routine transesophageal echocardiography for the evaluation of cerebral emboli in elderly patients.

BACKGROUND: Approximately 20% of cerebral infarctions are cardioembolic in nature. Transesophageal echocardiography (TEE) is widely regarded as the initial study of choice for evaluating cardiac source of embolism. Although the majority of cerebrovascular accidents occur in elderly patients, the value of TEE in this population is poorly defined. METHODS: We compared 491 patients older than 65 years with suspected embolic stroke or transient ischemic attack (TIA) who had undergone TEE evaluation between April 2000 and February 2004 to an age-, sex-, and time-matched control group that consiste…

Scattered brain infarct pattern on diffusion-weighted magnetic resonance imaging in patients with acute ischemic stroke.

BACKGROUND AND PURPOSE: Infarct patterns on brain imaging contribute to the etiologic classification of ischemic stroke. However, the association of specific subtypes of infarcts and etiologic mechanisms is often weak, and acute lesions are frequently missed on initial computed tomography (CT). Diffusion-weighted imaging (DWI) is superior in visualizing acute ischemic lesions as compared to CT and conventional magnetic resonance imaging (MRI). In our prospective study, we addressed the question whether a distinct pattern of infarction on DWI is associated with infarct etiology and clinical out

Transesophageal echocardiographic diagnosis of right to left shunting across the foramen ovale in adults without prior stroke.

OBJECTIVES: The purpose of this study was to estimate the prevalence of potential right to left interatrial shunting and to quantify the morphologic characteristics of the fossa ovalis in adults without a prior history of stroke or systemic embolism. BACKGROUND: Paradoxic embolization through a patent foramen ovale is an important cardiac mechanism for embolic stroke. Although anatomic and physiologic data obtained by transesophageal echocardiography increase the frequency of demonstration of potential cardiac sources of systemic embolism and occasionally can conclusively demonstrate the mech…

Anticipating stroke in patients undergoing transcatheter aortic valve replacement.

INTRODUCTION: Transcatheter aortic valve replacement (TAVR) has transformed treatment of severe aortic stenosis, offering a less invasive alternative to surgery. Despite advances, stroke remains a serious complication, impacting morbidity, mortality, and long-term cognitive function, thus the need for effective prevention strategies. AREAS COVERED: Based on a targeted review of the PubMed literature, this review addresses mechanisms and risk factors for stroke after TAVR and evaluates current preventive approaches. Cerebral embolic protection devices (CEPDs), particularly the Sentinel system,

Detection of occult paroxysmal atrial fibrilation by implantable long-term electrocardiographic monitoring in cryptogenic stroke and transient ischemic attack population: a study protocol for prospective matched cohort study.

BACKGROUND: Cardio-embolic etiology is the most frequently predicted cause of cryptogenic stroke/TIA. Detection of occult paroxysmal atrial fibrillation is crucial for selection of appropriate medication. METHODS: Enrolment of eligible cryptogenic stroke and TIA patients began in 2014 and will continue until 2018. The patients undergo long-term (12 months) ECG monitoring (implantable loop recorder) and testing for PITX2 (chromosome 4q25) and ZFHX3 (chromosome 16q22) gene mutations. There will be an appropriate control group of age- and sex-matched healthy volunteers. To analyse the results de

[Prevention of ischemic stroke in middle aged patients with atrial fibrillation. Effect of sinus rhythm maintenance, aspirin, warfarin, and simvastatin].

Patients (n=263, age 49-64 years) with persistent nonvalvular atrial fibrillation were subjected to controlled treatment aimed at restoration and maintenance of sinus rhythm (n=134) or slowing of rate of ventricular contractions. Efficacy of complementary use of combination of simvastatin with aspirin, warfarin and aspirin for prevention of ischemic stroke was assessed in both of these strategies. First strategy, supplemented with combination of simvastatin and aspirin prevented ischemic stroke and lethal outcome most successfully, while the use of warfarin was unexpectedly associated with com

The etiological diagnosis of ischemic stroke is crucial for secondary prevention, but often complex for the patients' cardiovascular comorbidities, each of which may cause a stroke. We report the case of an 84-year-old patient with severe left ventricular systolic dysfunction due to dilated cardiomyopathy and implantable cardioverter-defibrillator in primary prevention, hospitalized for atherothrombotic ischemic stroke treated conservatively and later by carotid thromboendarterectomy for ulcerated plaque of the right internal carotid artery.
