Thyroglossal duct and other congenital midline cervical anomalies.

Thyroglossal duct anomalies and dermoid cysts comprise the vast majority of congenital midline cervical masses seen in children. Unusual lesions of the midline neck include ranulae and midline cervical clefts. Workup and management is lesion-dependent, and an accurate preoperative diagnosis is essential for planning and performing an appropriate surgical procedure. This discussion presents an overview of the relevant embryology, pathophysiology, and diagnostic modalities for these congenital midline cervical anomalies. Additionally, the current principles of surgical management are described.

Lumbo-costo-vertebral syndrome with posterior spinal dysraphism.

Lumbo-costo-vertebral syndrome, which includes abnormalities of the vertebral bodies, ribs and trunk musculature, is very rare and only few cases have been reported. We report a case of 18-month-old female child with absent ribs, hemivertebrae, superior lumbar hernia (features of lumbo-costo-vertebral syndrome) and posterior spinal dysraphism, which to our knowledge is the first case in the English literature with such a combination of defects. Embryology and management of the case is discussed.

Pediatric umbilical problems.

After birth, the normal umbilicus is a relatively simple structure. During the development of the embryo, however, this region is highly complex. Vestigial of the umbilical cord can be responsible for umbilical inflammation and drainage. This article reviews the embryology of the umbilicus and discusses a number of clinical problems seen in this area. The authors' aim is to aid the primary care pediatrician in evaluating, treating, and appropriately referring umbilical problems encountered in office practice.

[A peculiar anomaly of the main biliary duct: choledochal cystic dilatation].

The Authors report a case of choledochal cystic dilatation and examine this particular anomaly of the main biliary duct analysing the complex classification. After a brief review of the embryology of the hepato-pancreatic ring, etiopathogenetic theories, clinic characteristics, as well as diagnostic and therapeutic possibilities for this affection are taken into account. It is concluded that the best therapeutic choice should be always based on an accurate evaluation of the anatomo-pathological conditions in each single case.

Spontaneous rupture of a vesicourachal diverticulum manifesting as acute abdominal pain.

Urachal abnormalities are rare clinical entities. This article reports a case of a spontaneously ruptured vesicourachal diverticulum manifesting as acute abdominal pain in a healthy 38-year-old man. The embryology, clinical diagnosis, and management of urachal abnormalities are discussed. Emergency physicians should include this clinical entity in their differential diagnoses of patients who develop acute abdominal pain in the absence of trauma, especially when the pain immediately follows a maneuver that increases intra-abdominal pressure.

Incomplete penoscrotal transposition associated with hemivertebrae.

Penoscrotal transposition is a rare anomaly of external genitalia, often associated with a variety of defects usually related to the urinary and the gastrointestinal tracts and the vertebral column. We report such a patient with hemivertebrae at L4-5, who had no other urological anomaly. To our knowledge this anomaly as an isolated urological entity with hemivertebrae has not been reported before. A brief mention is made of the embryology, incidence and various surgical techniques of correction.

Anatomical basis of inguinal surgery in children.

This paper begins with a review of the embryology of the inguinal region. The authors then point out the special morphological and topographical features of the inguinal canal in male children. These observations were based on the study of 50 inguinal canals from male children. From the anatomical observations described in this study guidelines are proposed for the surgical approach and dissection of the spermatic cord used in the treatment of congenital hernias or abnormal descent of the testis.

Müllerian dysgenesis.

The multifaceted syndrome of müllerian dysgenesis appears to represent a defect in mesodermal organization and is often associated with renal and skeletal anomalies. This report discusses the embryology, multifactorial etiology, clinical presentation and management of the syndrome from a historical standpoint and details our experience with 35 affected patients. Both Frank dilatation and McIndoe vaginoplasty appear to be acceptable means of therapy. The therapeutic success depends to a large extent on the psychosocial adaptation of the patient.

[Ethmoidal mucocele in a nursing infant. Apropos of 1 case].

Characteristic features of mucoceles of the paranasal sinuses in infants are their extreme rarity, their localization exclusively in the ethmoidal sinuses, and the specific nature of their clinical expression. Partial explanation of these specific features is provided form knowledge of the embryology of the facial sinuses and the various pathogenic theories developed concerning these mucocele lesions. Two out of four cases observed were associated with viscidosis, and a sweat test must be routinely performed in all cases.

Embryonic development of the head and neck: part 2, the branchial region.

The embryology of the branchial (pharyngeal) region is reviewed in terms of the embryonic pharynx, branchial apparatus and derivatives, tongue, larynx, and trachea. Derivatives of the branchial pouches, grooves (clefts), and arches are analyzed in relation to each other and to the first arch syndrome. Other common head and neck defects attributable to abnormal development of the branchial region are also discussed, particularly those defects involving the incomplete degeneration or the abnormal migration of branchial tissues.

Anatomy of the thoracic duct.

The thoracic duct is a major anatomic structure of the upper part of the abdomen, chest, and the lower part of the neck. This article reviews the embryology, anatomy, and multiple variations of the thoracic duct. Proper knowledge of this anatomy should ease understanding the pathophysiology of diseases involving the lymph channels and also prevent injury to the duct during major procedures in which the duct or its tributaries can be involved.

Quadricuspid aortic valve by cardiac magnetic resonance imaging: a case report and review of the literature.

Quadricuspid aortic valve (QAV) is a rare congenital cardiac entity. The recognition of QAV has clinical significance as it causes aortic valve dysfunction, commonly aortic regurgitation, and is often associated with other congenital cardiac abnormalities. We showed the important role played by cardiac magnetic resonance imaging in detecting QAV and review the available literature to explain its incidence, diagnosis, classifications, embryology, correlation between morphology of the QAV and its function, associated conditions, and management.

Malformations of dorsal induction.

Dorsal induction includes the formation and closure of neural tube, occurs during 3-5 weeks of gestation. Neurulation occurs in two phases, primary neurulation (formation of the neural plate and subsequently neural tube) and secondary neurulation (formation of distal cord and sacral and coccygeal segments). Failure of dorsal induction leads to anencephaly, exencephaly, cephaloceles, Chiari malformation and spinal dysraphism. In this article we discuss the relevant embryology, etiopathology and detail imaging appearances of these malformations.

Congenital penile urethrocutaneous fistula: A rare anomaly and review of literature.

Congenital penile urethrocutaneous fistula is described as an unusual developmental anomaly in children who present with an abnormal opening on the ventral aspect of penis with a normal foreskin and an absence of chordee and hypospadias. The authors present a discussion on the etiology, embryology, and management of this entity along with a description of three cases. We emphasize meticulous clinical examination for the diagnosis and to rule out other associated anomalies.

Terminal triplication of the left vertebral artery: a unique variation.

This report describes a unique case of triplication of the terminal left vertebral artery, forming the basilar artery in a 75-year-old male. CT angiography of cranio-cervical vessels also demonstrated the right vertebral artery originating from the right common carotid and an aberrant right subclavian artery. To the best of our knowledge this is the first report of a variation of this nature. The embryology and the clinical importance are discussed.

Rare case of fetal gastroschisis with aplasia of the foot and external genital organs.

Gastroschisis is a rare anomaly and is usually not associated with any other congenital anomalies. The embryology of gastroschisis and omphalocele remains a matter of speculation. Incidences of gastroschisis are particularly high among pregnancies in very young women. The present case is reported because of its rare association with the condition of gastroschisis, disrupted omphalocele, with aplasia of the foot and external genital organs, as well as imperforate anus with distal rectal atresia.

Congenital and developmental pancreatic anomalies: ultrasound, computed tomography, and magnetic resonance imaging features.

The purpose of this article is to provide an overview of the most common anatomic, congenital, and developmental anomalies of the pancreas. Familiarity with the imaging appearances of these anomalies as well as the range of normal appearance is important to differentiate them from other conditions, especially since some of these variants are commonly encountered. Basic knowledge of the normal embryology is of utmost importance to be able to recognize these disorders.

Congenital malformations of the skull and meninges.

The surgery and management of children who have congenital malformations of the skull and meninges require multidisciplinary care and long-term follow-up by multiple specialists in birth defects. The high definition of three-dimensional CT and MRI allows precise surgery planning of reconstruction and management of associated malformations. The reconstruction of meningoencephaloceles and craniosynostosis are challenging procedures that transform the child's appearance. The embryology, clinical presentation, and surgical management of these malformations are reviewed.

Intrathoracic ectopic lobe of liver presenting as pulmonary sequestration.

Intrathoracic ectopic lobe of the liver in the presence of a normal intact diaphragm is extremely rare. We report a case of a 13-month-old male initially diagnosed with pneumonia and pulmonary sequestration who was found to have an intrathoracic liver lobe and intact diaphragm. The presence of this condition suggests an event preceding closure of the diaphragm and illustrates the unique potential of pediatric pathology to shed light on human embryology.
