Gaucher disease and its treatment options.

OBJECTIVE: To review the epidemiology, pathophysiology, and treatments of Gaucher disease (GD), focusing on the role of enzyme replacement therapy (ERT), andsubstrate reduction therapy (SRT). DATA SOURCES: A literature search through PubMed (1984-May 2013) of English language articles was performed with terms: Gaucher's disease, lysosomal storage disease. Secondary and tertiary references were obtained by reviewing related articles. STUDY SELECTION AND DATA EXTRACTION: All articles in English identified from the data sources, clinical studies using ERT, SRT and articles containing other inte…

Unreliability of the Fecal Pancreatic Elastase-1 Test In Predicting a Response to Pancreatic Enzyme Replacement.

BACKGROUND: The fecal elastase-1 assay is the most frequently used screening test for pancreatic enzyme insufficiency (PEI). It has a sensitivity of 73%, with lower sensitivities for mild and moderate PEI. Reliance on this test has resulted in "ruling out" PEI in patients with undiagnosed chronic gastrointestinal complaints. Some of these patients will respond dramatically to a simple and safe trial of pancreatic enzyme replacement. METHODS: A list of all fecal elastase-1 tests performed at VA Connecticut during calendar year 2025 was obtained (N=102). The charts of patients with elastase-1 r

[Molecular basis of treatment in Gaucher's disease].

In Gaucher's disease (GD) there is an imbalance in the monocyte-macrophage system between the rate of formation and degradation of glucosylc … of ceramide-glucosyltransferase used mimic glucose or ceramide. All recombinant enzymes used in GD enzyme replacement therapy have similar kinetic parameters, but differ in their amino acid sequence, as well as, in the exposed mannose oligosaccharide chains. The mannose content and localization in the oligosaccharide chains are essential for cell uptake. Velaglucerase alph …

Management of Pompe disease alongside and beyond ERT: a narrative review.

BACKGROUND: Pompe disease is a lysosomal storage disorder that primarily affects muscles, and its natural history has been transformed over the past 20 years by therapies designed to restore the deficient enzyme function, from the first enzyme replacement therapies (ERTs) to the gene therapy currently in development. However, despite these ground-breaking innovations, the importance of a multi-system and rehabilitative approach remains critical, as it addresses the complex systems involved in the disease and optimizes the success of pharmacological treatments. METHODS: We conducted a narrativ

Humanistic burden of mucopolysaccharidoses: a systematic literature review.

OBJECTIVE: To systematically review the literature and summarize the health-related quality-of-life (HRQoL) of patients undergoing treatment for mucopolysaccharidoses (MPS), a rare, hereditary lysosomal storage disorder. METHODS: A systematic review was performed in accordance with PRISMA guidelines to identify research studies that describe the humanistic burden of MPS. A comprehensive literature search was conducted in EMBASE, MEDLINE, and eligible conferences were screened to include applicable abstracts. RESULTS: Of 870 identified articles, 15 studies reported the HRQoL burden of patient

Fabry Disease: Current and Novel Therapeutic Strategies. A Narrative Review.

BACKGROUND: Fabry disease (FD) is an inherited lysosomal storage disorder, leading to multisystemic manifestations and causing significant morbidity and mortality. OBJECTIVE: The aim of this narrative review is to present the current and novel therapeutic strategies in FD, including symptomatic and specific treatment options. METHODS: A systematic literature search was conducted to identify relevant studies, including completed and ongoing randomized-controlled clinical trials (RCTs), prospective or retrospective cohort studies, case series and case reports that provided clinical data regard

Adaptive pathway development for Fabry disease: a clinical approach.

Fabry disease (FD) is a rare X-chromosome-linked lysosomal storage disorder. Although initial expectations of enzyme replacement therapy (ERT) were high, it is now clear that real-world effectiveness is disappointing and evidence gathering has been inadequate. In retrospect, development of ERT for FD had several shortcomings. Little convincing evidence on the effectiveness existed at time of authorization. Also, post-marketing evaluation failed to generate sufficient and relevant data for adequate evaluation on effectiveness. Adaptive pathways might have benefitted ERT development by: (i) invo…

European expert consensus statement on therapeutic goals in Fabry disease.

BACKGROUND: Fabry disease, an inherited lysosomal storage disorder, causes multi-organ pathology resulting in substantial morbidity and a reduced life expectancy. Although Fabry disease is an X-linked disorder, both genders may be affected, but generally to a lesser extent in females. The disease spectrum ranges from classic early-onset disease to non-classic later-onset phenotypes, with complications occurring in multiple organs or being confined to a single organ system depending on the stage of the disease. The impact of therapy depends upon patient- and disease-specific factors and timing

Long-term treatment outcomes in Gaucher disease.

Following the treatment of the first Gaucher disease patient with enzyme replacement therapy (ERT), it was clear that ERT had the potential to be transformative with dramatic improvements in systemic manifestations of the disease within 2 years. Following over 20 years existence of the International Collaborative Gaucher Group Gaucher Registry and evidence from ∼6000 patients, the long-term effects of therapy have been documented. It has been shown that ERT can result in improvements in all clinical and laboratory parameters of nonneuronopathic disease. However, different aspects of the diseas…

Gaucher's disease: the changing paradigm of a lysosomal disorder.

Gaucher's disease (GD) is the most common lysosomal storage disease with a frequency of approximately 1:50,000 people. It is the result of the deficiency of the lysosomal enzyme beta-glucocerebrosidase. The deficiency of the enzyme results in the accumulation of the substrate, glucosyl-ceramide, in the organs. Substitutive enzymatic treatment has been available since almost 20 years. This brief overview highlights some of the most important milestones and the treatments for this disease. The study of this rare disorder is beginning to provide information on the pathogenesis of common diseases

[Current treatment for Gaucher's disease and new prospects].

Two new useful enzymes have recently undergone clinical trials for the treatment of Gaucher's disease (GD): velaglucerase alpha and taliglucerase alpha were both approved for early access programs as of the June 2009 shortage in Imiglucerase supply. Velaglucerase has been approved by both, Food and Drug Administration and European Medicines Agency. The Phase I/II trial of velaglucerase is in its 8th year: There were no drug-related serious adverse events or withdrawals, and no antibodies. Statistically significant improvements (p<0.004) were noted in mean percent change from baseline to nine m

Brief Clinical Report: Hypophosphatasia-Diagnostic Considerations and Treatment Outcomes in an Infant.

Hypophosphatasia (HPP) is a rare, inherited metabolic bone disorder characterized by low serum alkaline phosphatase activity and impaired bone mineralization. Clinical manifestations and severity of symptoms vary widely in HPP, ranging from in utero death to isolated dental manifestations in adults. Treatment with enzyme replacement therapy has been reported to improve outcomes in perinatal, infantile, and childhood forms of HPP. Here, we present a case of a boy with poor linear growth, mild limb bowing, and radiographic rickets who was diagnosed with HPP before 6 months of age. Treatment with…

Enzyme replacement for lysosomal diseases.

Following the demonstration of the nature of the enzymatic defects in the sphingolipid storage disorders in the mid-1960s, consideration was directed to the development of therapy for patients with these conditions. High on the list of possibilities was enzyme supplementation or replacement. Many years of arduous investigation and the development of novel protein targeting strategies were required to bring this concept to fruition. Enzyme replacement therapy (ERT) was eventually shown to be extraordinarily effective for patients with Gaucher disease, the most prevalent metabolic storage disord…
