Pre-surgical evaluation for epilepsy surgery.

Neuropsychological testing may reflect subtle structural changes that may not be readily apparent with neuroimaging studies, and physiologic disruption of normal neural function secondary to epileptic activity. Neuropsychological testing is used during the pre-operative evaluation for epilepsy surgery to assess functional brain status, which, in turn, provides important information on the risks for post-operative neuropsychological deficits and also provides confirmatory evidence of seizure onset laterality in patients whose seizures originate in temporal lobes. This review will focus primaril…

Source localization in refractory partial epilepsy.

In this paper, 51 patients with refractory complex partial seizures (CPS) and intracranial structural abnormalities demonstrated with optimum MR (space-occupying: n = 16; atrophic: n = 32; dysplastic: n = 3) were studied. Video-EEG monitoring showed CPS in all patients. In 13 patients, additional intracranial EEG monitoring demonstrated hippocampal seizure onset in 12 and medial occipital ictal onset in 1 patient. Interictal and ictal dipole modeling using a spherical head model and realistic electrode coordinates were performed. Spatiotemporal dipole mapping of interictal epileptic discharges

Callosotomy for epilepsy after West syndrome.

PURPOSE: To analyze the results of callosotomy in 17 children with symptomatic generalized epilepsy after West syndrome, according to the different seizure types and surgical procedures, to define selection criteria for candidates to callosotomy. METHODS: Callosotomy was performed in two successive stages. Partial callosotomy, anterior in 13 and posterior in three, was followed by completion in 14 cases in all but four patients (complete callosotomy in one stage in one, no completion in three). All patients had clinical, video-EEG, and neuropsychological evaluation before and after each stage…

[Vigabatrin in childhood epilepsy--personal experience].

Vigabatrin has been in clinical use for 18 years. It has not been used widely in our country because of unavailability and costs of therapy. After the end of war in Bosnia and Herzegovina we started to use it in treatment of childhood epilepsies. We studied 19 patients that received vigabatrin as add on therapy and monotherapy. Follow up period was 6 months to 2 years. Nine of those patients were diagnosed as West syndrome, three had tuberous sclerosis, seven had intractable partial epilepsies. In the group with West syndrome 7 patients (5 as add on therapy and 2 as monotherapy) responded with

[Autosomal dominant nocturnal frontal lobe epilepsy].

Autosomal dominant nocturnal frontal lobe epilepsy (ADNFLE) is a recently identified seizure disorder. The disease maps to the long arm of chromosome 20 and may be related in some families to a missense mutation in the neuronal nicotinic acetylcholine receptor alpha 4 subunit. We describe one of the first European family with ADNFLE, including five affected individuals spanning four generations. In the most severely affected subject, the onset was during the second month of life and persisted through adult life. Seizures were very frequent during infancy, although long-term evolution was relat…

Bilateral parasagittal parietooccipital polymicrogyria and epilepsy.

We describe 9 patients with a bilateral malformation of cortical development, centered around the parasagittal and mesial aspects of the parietooccipital cortex, with magnetic resonance imaging findings suggestive of polymicrogyria. No familial distribution or etiologic factors were identified. Location in a watershed area between anterior and posterior cerebral arteries suggests postmigratory perfusion failure as the underlying cause. In most patients the malformation was detected by magnetic resonance imaging after computed tomography scans with 10-mm-thick sections were considered normal. S

Dipole modeling in epilepsy surgery candidates.

PURPOSE: The validity and clinical significance of dipole modeling in epilepsy surgery candidates is not fully established. PATIENTS AND METHODS: Interictal and ictal dipole modeling was performed in 43 patients with refractory complex partial seizures (CPS) and intracranial structural abnormalities demonstrated with optimum magnetic resonance imaging (MRI: space-occupying, n = 15; atrophic, n = 26; dysplastic, n = 2). [Idiopathic partial epilepsy with occipital paroxysms].

INTRODUCTION: Partial benign epilepsy of childhood with occipital paroxysms (EPBI-O) described by Gastaut in 1982, has been recognized in the latest Classification of Epilepsy and Epileptic Syndromes as being a form of idiopathic partial epilepsy. In 1989 Panayiotopoulos described a different form of idiopathic occipital epilepsy. OBJECTIVE: We analyzed the electro-clinical characteristics of EPBI-O in order to identify the different forms of idiopathic occipital epilepsy. MATERIAL AND METHODS: This prospective study included 74 patients who fulfilled the diagnostic criteria of EPBI-O. The p…

[Reading-induced epilepsy: three new cases].

INTRODUCTION: Primary epilepsy of reading is a rare syndrome in which patients present with mandibular myoclonia when reading texts. Seizures are also frequently provoked by other stimuli. Occasionally they may be followed by generalized tonic-clonic seizures. CLINICAL CASES: We present the clinical features and electroencephalographs of three patients with epilepsy of reading, one of them also had seizures when playing chess and after doing arithmetic and the third after reading music. Treatment with valproic acid and clonazepam completely controlled the seizures in all three patients. In tw…

Callosotomy for intractable epilepsy: overall outcome.

The effectiveness of callosotomy to the control of medically intractable epilepsy is still discussed fifty years after the first reported cases. Nevertheless patient selection, type of seizures and epileptic syndromes are now better determined. Atonic and tonic astatic seizures characterized both by clinical and electroencephalographical specific patterns, are the most responsive. A favorable outcome, from > 50% reduction in seizure frequency to a complete cessation, is obtained from 60 to 80% of the patients. For tonic-clonic seizures, favorable outcome fluctuates from 40% to 80% principally …

Rotatory seizures in juvenile myoclonic epilepsy.

Rotatory seizures have been reported in association with focal intracranial lesions. This type of seizure was also described in patients with primary generalized epilepsies. To our knowledge, there is only one previous publication denoted an association between juvenile myoclonic epilepsy (JME) and rotatory seizures. We present two female patients with JME and rotatory seizures together. The onset of myoclonic jerks and generalized tonic clonic (GTC) seizures was in their midteens. Their interictal EEGs showed bilateral symmetric spike and polyspike wave discharges. The rotatory seizures of th…

[Benign reflex myoclonic epilepsy in infants].

BACKGROUND: Myoclonic epilepsy of infancy are seldom benign. CASE REPORT: A 25-month old girl developed myoclonic jerks either spontaneously either as reflex responses to auditory and tactile stimuli, such as sudden touching of the face or trunk from the age of 4 months. The jerks disappeared after valproate therapy. Neurological examination was normal with a follow-up of 9 months. CONCLUSION: This condition resembles that described in 1995 by Ricci et al. In must be differentiated from other myoclonic epilepsies of infancy, reflex epilepsies and hyperekplexia. It could be the earliest from …

[Theophylline-associated epilepsy with lacuna infarction].

We reported a 9-year-old boy who had complex partial seizure for the first time, 4 years and 7 months after an episode of theophylline--associated convulsive status. An ictal EEG showed abnormal theta wave burst which showed no superficial focus, implying existence of deep focus in the brain. MRI revealed left thalamic infarction and hippocampal sclerosis. We regarded these abnormalities as the sequelae of theophylline-associated convulsive status.

Methsuximide therapy of juvenile myoclonic epilepsy.

Currently valproic acid is considered to be the drug of first choice for juvenile myoclonic epilepsy (JME) resulting in a 70-90% control rate for all seizure types associated with JME. In those situations where valproic acid fails to control seizure activity, results in unacceptable side-effects, or is declined due to potential side-effects, an alternative effective monotherapy would be desirable. Five adolescent female patients were placed on methsuximide for JME. All five patients have been seizure free with the use of methsuximide and four out of five are now on methsuximide monotherapy wit…

Early onset bilateral calcifications and epilepsy.

Bilateral occipital calcifications associated with epilepsy and sometimes with celiac disease have been described previously. A boy with bilateral frontal and occipital diffuse calcifications accompanied by failure to thrive, nephrogenic diabetes insipidus, developmental delay and seizures, but without celiac disease is presented. Follow-up at 3 years of age disclosed neurodevelopmental delay, height and weight less than expected for age, and seizures controlled with carbamazepine. The uncommon association of these features and the early onset of symptoms is discussed. Although bilateral occip…
