Mesenchymal chondrosarcoma with secondary orbital invasion.

Orbital involvement with mesenchymal chondrosarcoma is rare. Until recently, despite the young age of the affected population, exenteration was the recommended management. We report a patient with orbital invasion by mesenchymal chondrosarcoma managed surgically without exenteration. Adjuvant chemotherapy and radiotherapy were administered. There is no evidence of local or distant tumor recurrence with 30 months of postoperative follow-up. This and other recent case reports suggest that exenteration may not be necessary for local tumor control of mesenchymal chondrosarcoma in the orbit.

Arteriovenous malformation of the posterior ethmoidal artery as an unusual cause of amaurosis fugax. The ophthalmic steal syndrome.

We report the case of a patient with arteriovenous malformation fed by a posterior ethmoidal artery. The pre-operative amaurotic attacks on the diseased eye subsided after removal of the malformation. Doppler sonographic examination of the peripheral ophthalmic artery showed an inversion of the blood flow direction postoperatively. We describe a concept of an ophthalmic steal syndrome treated in this patient by removal of an arteriovenous malformation.

Bony interorbital distance (BIOD), head size, and level of the cribriform plate relative to orbital height: II. Possible pathogenesis of orbital hypertelorism.

This investigation was designed to test hypothesized pathogenetic mechanisms for orbital hypertelorism (OrH). Serial measurements of bony interorbital distance (BIOD), level of cribriform plate relative to orbital height (LOCP), and cranial width (CW) were made from postero-anterior cephalometric radiographs on two pathologic groups craniofacial clefts (FC) and premature craniofacial synostosis syndromes (PCS) compared with normal controls matched for age and sex. Both groups of malformations showed significantly larger mean values (P less than 0.01) for BIOD alone and when adjusted for CW. Th

A literature review of the vascular system in the human orbit.

Some clinical ophthalmological problems led us to a study of literature data about the blood vessels in the human orbit. This inquiry confronted us with a profound knowledge of individual vessels. Remarkably, the arterial system appears to be in general more variable than the venous system. Radiologists have also noticed the difference in variability between the two vascular systems. Mainly for this reason, they consider topographical information obtained from phlebograms more useful than that obtained from arteriograms. However, the spatial arrangement of the orbital vessels has been given li

Osteoid-producing orbital ethmoid tumor.

Fibro-osseus lesions of the paranasal sinuses, especially those that contain multinucleated giant cells, demonstrate a significant histopathologic overlap although they manifest themselves throughout a wide range of clinical behavior. They routinely require a critical clinical and pathologic interpretation. Often, the clinical course of the disease is the only guide to diagnosis and mode of treatment. This study presents the case of a 4-year-old boy with an osteoid-producing tumor of the ethmoid sinus. Despite an original diagnosis of fibrous dysplasia, the lesion followed a relentless course.

[Surgical management of complicated orbital fractures: experiences with 89 orbital corrections and reconstructions].

OBJECTIVE: To summarize the experiences in the treatment of complicated orbital fractures. METHODS: A total of 89 patients with complicated orbital fractures treated in Department of Oral and Maxillofacial Surgery, China Medical University from January 2005 to January 2010 were retrospectively reviewed. The classification of these cases included naso-orbital-ethmoid fracture, frontal orbital fracture and orbitozygomatic fracture. All patients were followed up for 6 - 36 months. RESULTS: The orbital frame was repaired or reconstructed in these patients. The function of lacrimal pathways was i

Unique case of a post-traumatic nasoseptal arteriovenous malformation.

Extracranial manifestation of arteriovenous malformations (AVMs) is uncommon. Nasoseptal AVMs are an even rarer entity. In this case report, we present an interesting and first-of-its-kind case of the development of a left nasoseptal AVM in a 60-year-old man after a fall. This was likely post-traumatic, unlike the usual congenital AVMs described in the literature. The patient was managed conservatively with regular follow-up for the AVM as he was asymptomatic.

Fibroblast Growth Factor 23-Producing Phosphaturic Mesenchymal Tumor with Extraordinary Morphology Causing Oncogenic Osteomalacia.

A possible cause of hypophosphatemia is paraneoplastic secretion of fibroblast growth factor 23 (FGF-23). Tumors secreting FGF-23 are rare, mostly of mesenchymal origin, usually benign, and may be located anywhere in the body, including hands and feet, which are often not represented in conventional imaging. A 50-year-old woman presented with diffuse musculoskeletal pain and several fractures. Secondary causes of osteoporosis were excluded. Laboratory analysis revealed hypophosphatemia and elevated alkaline phosphatase, parathyroid hormone, and FGF-23. Thus, oncogenic osteomalacia due to neopl

A possible venous connection between the cranial and nasal cavity.

PURPOSE: The foramen cecum (FC) is a fine bony canal with the aperture located immediately anterior to the crista galli (CG). The venous structures in the regions of the FC and CG have been inconsistently described and are not well understood. Here we explore these veins using magnetic resonance imaging. MATERIALS AND METHODS: We enrolled 101 patients who underwent contrast examinations and exhibited intact skin, skull, dura mater, and intracranial dural sinuses. Imaging data were obtained as thin-sliced, seamless sagittal sections and were transferred to a workstation for analysis. RESULTS:

Image-Guided Dacryolocalization (IGDL) in Traumatic Secondary Acquired Lacrimal drainage Obstructions (SALDO).

PURPOSE: The aim of this study was to illustrate the usefulness of stereotactic or image-guided navigation in the management of traumatic secondary acquired lacrimal drainage obstructions. METHODS: Image-guided dacryolocalization was performed on 3 patients. All the patients were posttraumatic nasolacrimal duct obstruction as a consequence of gross naso-orbito-ethmoid fractures. All the procedures were performed through an endoscopic endonasal approach using the intraoperative electromagnetic image-guided Fusion ENT navigation system. Intraoperative anatomical guidance in localizing the lacri

Skull base injury leading to death after conventional septoplasty.

Treatment of nasal obstruction with conventional septoplasty is a very common operation in otorhinolaryngology. There are considerable complications that may become even life threatening. We report a rare complication of septoplasty, namely, skull base injury. The possible mechanisms and treatment modality are discussed. Like other similar procedures, prevention of the skull base injury is more important than its treatment. Correct knowledge about anatomy of the region and meticulous manipulation and instrumentation are the most considerable topics.

[Radiological findings of the nasal septum in CT scan in patients with deviated noses].

OBJECTIVE: The bony-cartilaginous crooked nose is characterized by a deviation of the nasal dorsum from the median sagittal plane. Since the nasal dorsum and the septum form an anatomical entity, a deviation of the external nose always results in a deviation of the cartilaginous and bony nasal septum. The aim of this study was to analyze the pathologies of the septum in patients with crooked noses based on CT images of the midface. METHODS: The CT images of patients with a documented crooked nose, who presented between 2005 and 2011 in our hospital, were analyzed retrospectively considering p

Acquired Brown syndrome: report of two cases.

We report 2 cases of acquired Brown syndrome, each associated with a mass in the supranasal area of the involved eye. Meticulous endoscopic surgery of an ethmoidal mucocele in the first case resulted in the recovery of superior oblique muscle function. Imaging studies suggested the presence of an orbital venous malformation in the second case, the clinical course of which was stable, with minimal intermittent diplopia over time.
