Complex soft tissue and facial bone reconstruction after a bear attack in Indonesia: a case report of definitive management following an 8-hour interfacility transfer.

Bear attacks, although rare, may result in severe maxillofacial trauma, highlighting the devastating consequences of human-wildlife conflict in rural areas. Bear maulings typically cause cutting, penetrating, and crushing injuries, with most victims sustaining contaminated maxillofacial trauma requiring comprehensive management to prevent complications. We report a case of a 68-year-old woman presenting with sustained severe left-sided facial trauma following a bear attack. The patient had a full-thickness hemifacial avulsion, fractures of the zygomaticum and maxilla, and a non-salvageable ocu

Ophthalmic complications following maxillofacial and midface fractures: a systematic review".

Maxillofacial fractures can lead to significant ophthalmic complications, affecting patient outcomes. This systematic review synthesizes current evidence regarding the incidence, causes, and types of ocular injuries associated with facial fractures, while evaluating inter-study variability and risk of bias. Following the PRISMA guidelines, a comprehensive search of PubMed, Cochrane, and Web of Science databases was performed. Twenty-one studies, including 7,998 participants, met the inclusion criteria. Among them, 3,693 patients presented with ophthalmic complications after facial fractures. O

Would brief alcohol intervention be helpful in facial trauma patients?A Narrative Review.

Facial trauma is commonly associated with excessive consumption of alcohol and is often associated with interpersonal violence or motor vehicle accidents. Alcohol-related trauma presentation to hospitals causes a major service burden, and there have been efforts to reduce such trauma load with educational programs and social support. Brief alcohol intervention (BAI) in an acute setting (emergency department or trauma centre) has been shown as an effective means to reduce future alcohol intake and the incidence of future alcohol-related injuries, especially in the period immediately following i

Orbital and Ophthalmologic Injuries Associated With Standing Electric Scooter Accidents: A Narrative Synthesis of Clinical Outcomes and Injury Patterns.

Standing electric scooters (e-scooters) have introduced a distinct pattern of facial trauma to emergency departments worldwide. Their unique biomechanics, with riders standing upright on a narrow platform, creates a characteristic forward pitching mechanism during sudden stops that directs high-energy impacts to the anterior midface and orbital region. This differs fundamentally from bicycle accidents, where lateral falls and arm bracing are typical. This review synthesises available clinical evidence on orbital and ophthalmologic injuries from electric scooter accidents. Twelve primary studie

Epidemiology of Traumatic Peripheral Nerve Injuries Evaluated with Electrodiagnostic Studies in a Tertiary Care Hospital Clinic.

OBJECTIVE: To describe the etiologies and frequency of traumatic peripheral nerve injury (TPNI) seen in the electrodiagnostic laboratory of a tertiary care hospital in Puerto Rico. METHODS: The charts of patients who underwent an electrodiagnostic study for a TPNI were revised. The main outcome measure was the frequency of each injury by anatomic location, specific nerve or nerves affected, injury mechanism, and injury severity. RESULTS: One hundred forty-six charts were included, and in them were listed a total of 163 nerve injuries; 109 (74.7%) cases were men and 37 (25.3%) were women. The

Tactics of surgical treatment at traumatic injuries of maxillo-zygomatic-orbital complex and the lower edge of the orbit.

The maxilla-facial traumatic injuries, zygomatic - orbital complex and orbit lower edge injuries are characterized by dislocation of bone fragments, causing not only the anatomo-functional disorders but well-expressed deformity/ abnormality of the patient. 28 patients with traumatic fractures of zygomatic - alveolar complex and orbit lower edge have been studied and got surgical treatment by the authors. At admission to the hospitals, all the patients were the subjects to a classic comprehensive / complex examination including a Spiral Computed Tomography, with 3D reconstruction (three-dimensi

[Epidemiology and management in emergency room patients with maxillofacial fractures].

OBJECTIVE: To minimize overall mortality and optimise reconstructive and cosmetic outcome in severely injured patients with maxillofacial injuries the interdisciplinary coordination of several surgical disciplines is required. It is still discussed controversy whether patients with maxillofacial fractures benefit from early fracture repair or if delayed operative management also yields in good results. METHODS: Herein we analysed the data of 1252 severely injured patients between May 1998 through June 2002 in our trauma department regarding fractures of the maxillofacial region, injury severi…

Dog bite injury - alar repair with composite graft.

Dog bite injuries are an important emergency. Facial dog bites can lead to disfigurement and stigmatization. Dog owners themselves are the usual victims of facial dog bites. Early surgery provides best results. A 28-year-old male patient presented 1 year after he had been bitten on his nose by his own dog. The pronounced nasal disfigurement with nostril stenosis was treated by reconstruction using a free composite graft from the crus of the ipsilateral ear with good results. Delayed reconstruction can provide satisfying results.

Principles and clinical application of micronerve surgery and nerve transplantation in the maxillofacial area.

Modern microsurgery techniques using perineural interfascicular-nerve suturing permit successful nerve reconstruction in tumor defects and injuries of the maxillo-facial area. Since 1972 we have concentrated on nerve grafts and reconstructed the facial, accessory, hypoglossal, lingual, and mandibular nerves. In all cases, exclusively autologous nerve grafts were used; as donor nerves we found the great auricular and sural nerves especially suitable. Along with typical examples of the different nerve defects in the maxillofacial surgery, our technique in these special nerve transplantation proc

[Botulinum therapy within a three-stage medical rehabilitation framework for patients with disorders of craniofacial muscles].

Disorders of craniofacial muscles (CFM) frequently arise from various conditions and injuries affecting the central and peripheral nervous systems, as well as from neurosurgical, reconstructive, and cosmetic interventions. The principal clinical manifestations include facial asymmetry, lagophthalmos, paralytic strabismus, synkinesis, contractures, trismus, bruxism, temporomandibular joint dysfunction, chronic facial pain, and sialorrhea. Such disorders not only restrict daily functioning but also lead to significant psychosocial maladjustment, a decline in quality of life, and increased disabi

Versatility of the Free Multipedicled Anterolateral Thigh Flap for Restoring Complex Facial Soft Tissue Defects.

INTRODUCTION: Complex soft tissue injuries in the facial area can arise from various causes. Surgeons face significant challenges when reconstructing these injuries, as they must select appropriate materials based on texture and color, while also considering their composition and properties. The anterolateral thigh (ALT) flap has emerged as a versatile option in clinical reconstructive surgery, offering many advantages over other free flaps. One of its main advantages is the ability to create a multicomponent chimeric flap, allowing for the reconstruction of various complex facial defects. ME

A Retrospective Study of Facial Fractures Associated With the Amazon Region at a Trauma Center in Northern Brazil.

BACKGROUND/AIM: Facial trauma is a major public health challenge due to its aesthetic, functional, psychological, and socioeconomic impacts. In Northern Brazil, particularly in the Amazon region, subsistence activities such as plant extraction, animal handling, and river transportation are essential to daily life and may contribute to an increased risk of facial injuries. These environmental and cultural factors are often underrepresented in the literature. This study aimed to investigate the epidemiological profile of facial fractures associated with etiologies specific to the Amazon region. …

Characterization of granulocyte colony-stimulating factor receptor expressed on human lymphocytes.

Maxillofacial injuries in general occur quite commonly following trauma and these injuries if not properly managed can negatively influence both the psychosocial and functional activities of the patient. This is as a result of the centrality of the facial region as a key factor in human identity, aesthetics, and general well-being. Fractures involving the facial skeleton may be isolated or complex. High velocity trauma is usually seen in urban and semi-urban areas while low velocity trauma is the common setting in rural areas. The pattern of faciomaxillary fractures vary with geographical area…

Delayed presentation of traumatic facial nerve (CN VII) paralysis.

Facial nerve paralysis (Cranial Nerve VII, CN VII) can be a disfiguring disorder with profound impact upon the patient. The etiology of facial nerve paralysis may be congenital, iatrogenic, or result from neoplasm, infection, trauma, or toxic exposure. In the emergency department, the most common cause of unilateral facial paralysis is Bell's palsy, also known as idiopathic facial paralysis (IFP). We report a case of delayed presentation of unilateral facial nerve paralysis 3 days after sustaining a traumatic head injury. Re-evaluation and imaging of this patient revealed a full facial paralys
