Hydatidosis is a rare parasitic disease caused by the Echinococcus tapeworm, which only occasionally affects the musculoskeletal tissues. In this article we describe the case of a patient who underwent a total hip replacement procedure for a pathological fracture of the femur neck. At the next histological examination it was shown to be a consequence of secondary bone hydatidosis. This clinical case is exceptional in that the infection spread to the cotyloid and femoral prosthesis components and, in the following years, caused repeated episodes of joint dislocation.

The data of 55 patients treated with ipriflavone are discussed. The protocol of examination and treatment are introduced. At the 12-month follow-up visit BMI increased with 3-5% in the lumbal spine and in the femur neck. Significant decrease of pain was observed in 55% of the patients. The number of side effects were low. According to the data, ipriflavone treatment was effective and well tolerated in postmenopausal osteoporosis. This treatment can be performed in cases when hormonal replacement therapy is contraindicated.

Femoroacetabular impingement syndrome (FAI) is a pathologic entity which can lead to chronic symptoms of pain, reduced range of motion in flexion and internal rotation, and has been shown to correlate with degenerative arthritis of the hip. History, physical examination, and supportive radiographic findings such as evidence of articular cartilage damage, acetabular labral tearing, and early-onset degenerative changes can help physicians diagnose this entity. We found, in a museum collection of skeletons, nine adult hips with untreated slipped capital femoral epiphyses. All the specimens were from men, five black and two white. Their mean age at death was 44 years. Seven of the femora were retroverted beyond neutral and five had true varus deformities. Osteoarthritis was detected in eight of the hips and the most severe degeneration was seen in the most deformed hips. Radiography revealed that cysts which appeared to occupy the femoral head in fact lay in the metaphyseal bone of the femoral neck.

Focal destructive lesions of the femoral neck occur in some cases of Legg-Perthes disease and may indicate a potential for marked deformity when healing is complete. Moreover, interventional procedures may aggravate tendencies to final deformity. The lesions should not be confused with infectious or neoplastic destruction. The sagging rope sign is the term used to describe the radiographic appearances which sometimes occur after Perthes' disease. It is severe examples of that disease and indicates damage to the growth plate with a marked metaphysial reaction. The same appearance follows severe epiphysitis after forcible reduction of a congenitally dislocated hip, and certain rare epiphysial dysplasias. The origin and significance of the sign are discussed.

Sliding hip screws (SHSs) are commonly used for the operative treatment of extracapsular neck of femur fractures. We report 2 rare cases of intrapelvic migration of an SHS, resultant of non-unions, threatening regional major vessels. Only a handful of such cases have previously been reported in the literature. We provide an up-to-date review of the literature, propose rationale for how a lag screw may migrate, and suggest how this complication can be prevented.

Acute hip pain is a common presenting complaint among patients attending Emergency Departments particularly following a traumatic event. A proximal femoral fracture is the main consideration, which requires urgent confirmation and treatment; however, there are a number of conditions, which could mimic neck of femur fracture and the causes may not be directly related to the hip, so systematic and careful observation of pelvic MRI could shed light on the true culprit of acute hip pain.

Treatment with etidronate alone is unlikely to increase the bone mineral density of the femoral neck. In combination of etidronate and hormone replacement therapy has been shown to markedly increase the bone mineral density of the femur and may decrease the risk of fracture. These findings suggest that etidronate and hormone replacement therapy inhibit bone resorption by different mechanisms of action. Treatment with a combination of etidronate and hormone replacement therapy may therefore be an important option for the management of postmenopausal osteoporosis.

Forty patients with fractured neck of femur were allocated randomly to undergo surgery under general anaesthesia (GA) or subarachnoid anaesthesia (SAB). After operation, the incidence of deep vein thrombosis (DVT), assessed by venography, was found to be 40% in the SAB group, which was significantly lower than the incidence (76.2%) in the GA group. These observations may account for the previously reported effect of SAB, in comparison with GA, in reducing early postoperative mortality in this category of patient.
