From 1991 to 1994, 39 ankles of 38 patients underwent surgical open reduction and internal fixation for pilon fractures. These patients included 29 males and 9 females with an average age of 38.6 y/o (range 28 y/o-58 y/o). A dynamic weight-bearing model has been developed in a cadaveric ankle model to assess the contact areas of the talus in varying degrees of bimalleolar ankle fractures. A surgically created transverse fibula osteotomy with up to 6 mm of displacement did not cause a significant change in the contact area. Fifteen tibial nonunions, ranging nine months to 19 years after original injury, were treated with the Brooker intramedullary nail (IMN). The average number of previous failed surgeries was two. Traumatic fracture of the long bones of the lower extremities may frequently occur in spinal cord injured patients. In the past, conservative treatment with pillow or sand bag splinting was advocated by many clinicans. This paper presents two cases in which posterior splinting was used on the affected leg for stabilization of the fracture with good result and well accepted by patients. Also, the use of Mud bed for prevention of pressure sores was illustrated.

The localisation of acute haematogenous pyogenic osteomyelitis following a local injury is well known. Whilst there is often a history of trauma in patients developing bone and joint tuberculosis, its role is obscure. Two patients are reported who developed histologically proven tuberculosis in the vicinity of closed fractures during the healing process. These two patients make a total of four so far reported.
