Assessing the feasibility of solar-powered digital health infrastructure in rural and peri-urban Kenya: a mixed-methods study.

BACKGROUND: Health systems in low- and middle-income countries (LMICs) face persistent infrastructure constraints, particularly unreliable electricity and limited digital connectivity, that hinder the effective implementation of digital health interventions. These challenges are especially pronounced in rural and peri-urban settings, where facilities rely on unstable power sources and fragmented information systems. Although digital health technologies can strengthen health systems and improve care delivery, their effectiveness depends on reliable infrastructure. Integrating renewable energy w

Barriers and facilitators to implementing a national guideline on foot assessment in people with diabetes in primary care: a qualitative study with insights to advance guideline practice, Singapore.

BACKGROUND: Diabetes-related foot ulcers precede 85% of lower extremity amputations, with Singapore among countries with the highest rates. To improve early detection and management, Singapore’s Agency for Care Effectiveness (ACE), Ministry of Health, introduced a national ACE clinical guideline (ACG) on diabetes-related foot assessment in 2019, with an updated edition released in 2024. This ACG is intended to guide routine diabetes-related foot screening (DFS), risk stratification and referral decisions across public and private primary care settings, and serves as a national reference for ev

Effect of the 'digital plus' approach to upscaling early childhood development services in rural China: study protocol for a cluster randomised controlled trial.

INTRODUCTION: The high prevalence of developmental delays among young children poses significant barriers to long-term social mobility in low-income and middle-income countries. Digital interventions aimed at early childhood development (ECD) have shown promise in improving developmental outcomes, yet scalable and effective strategies remain under-explored. This study aims to evaluate the effectiveness of a digitally delivered parenting intervention to improve early child development in rural China. METHOD AND ANALYSIS: We design a parallel cluster-randomised controlled trial to evaluate a 12

Harnessing community-based one health interventions implementation beyond Mpox outbreak management in Africa: insights and benefits.

BACKGROUND: Little is documented on key community-based One Health (OH) approach implementation, pro-activeness and effectiveness of interactions and strategies against Mpox outbreak public health emergency in international concern (PHEIC) in various African countries in order to stamp out the persisting Mpox outbreak threat and burden. Prioritizing critical community-based interventions and lessons learned from previous COVID-19, Mpox, Ebola, COVID-19, Rift Valley Fever and Marburg virus outbreaks revealed critical shortcomings in funding, surveillance, and community engagement that plague pu

Spatial analysis of unimproved drinking water source in East Africa: Using Demographic and Health Survey (DHS) data from 2012-2023.

BACKGROUND: According to the WHO/UNICEF Joint Monitoring Programme, unimproved drinking water sources include unprotected wells and springs, surface water (e.g., rivers, lakes), vendor-provided water, bottled water (without improved alternatives), and tanker truck-provided water. In East Africa, 68.7% of water at very high risk comes from such sources. Using unimproved drinking water sources can lead to serious health risks, including waterborne diseases such as cholera, dysentery, typhoid fever, and hepatitis. Therefore, this study aimed to map the spatial distribution of unimproved drinking

Sustainability of opioid agonist therapy programmes in Belarus, the Republic of Moldova, Tajikistan and Ukraine in the context of transition from Global Fund support during 2020-2023.

BACKGROUND: Most national programmes of opioid agonist therapy (OAT) in Eastern Europe and Central Asia are at a critical juncture for building their sustainability due to decreasing support from the Global Fund and other international HIV funders. Therefore, it is timely to identify the status, trends, opportunities and risk factors of OAT preparedness in the face of donor transition. METHODS: The study assessed the OAT sustainability progress in 4 countries: Belarus, the Republic of Moldova, Tajikistan and Ukraine. The study used a comparative country case study design with qualitative meth

New progress in HBV control and the cascade of health care for people living with HBV in China: evidence from the fourth national serological survey, 2020.

BACKGROUND: Since 1992, when recombinant hepatitis B vaccine was introduced in China, government health officials have used nationally representative serological surveys to monitor progress in prevention and control of hepatitis B. In 2020, we conducted the fourth seroepidemiological survey, which for the first time included medical evaluation of the clinical status of HBsAg positive subjects over the age of 15 and their medical management. We report survey results in comparison with the three previous surveys. METHODS: Consistent with previous national surveys, the 2020 survey used a stratif

Out-of-pocket expenditures and financial risks associated with treatment of vaccine-preventable diseases in Ethiopia: A cross-sectional costing analysis.

BACKGROUND: Vaccine-preventable diseases (VPDs) remain major causes of morbidity and mortality in low- and middle-income countries (LMICs). Universal access to vaccination, besides improved health outcomes, would substantially reduce VPD-related out-of-pocket (OOP) expenditures and associated financial risks. This paper aims to estimate the extent of OOP expenditures and the magnitude of the associated catastrophic health expenditures (CHEs) for selected VPDs in Ethiopia. METHODS AND FINDINGS: We conducted a cross-sectional costing analysis, from the household (patient) perspective, of care-s

The integration of the global HIV/AIDS response into universal health coverage: desirable, perhaps possible, but far from easy.

BACKGROUND: The international community's health focus is shifting from achieving disease-specific targets towards aiming for universal health coverage. Integrating the global HIV/AIDS response into universal health coverage may be inevitable to secure its achievements in the long run, and for expanding these achievements beyond addressing a single disease. However, this integration comes at a time when international financial support for the global HIV/AIDS response is declining, while political support for universal health coverage is not translated into financial support. To assess the risk

World Health Organization's Innovative Direct Disbursement Mechanism for Payment of Grassroots Immunization Personnel and Operations in Nigeria: 2004-2015.

BACKGROUND: Following the 1988 World Health Assembly resolution to eradicate polio, the government of Nigeria, with support from partners, has been implementing several rounds of supplementary immunization activities (SIAs) each year. In addition to the technical requirements, the success of the polio eradication initiative depends on timely provision of adequate financial resources. Disbursement of funds for SIAs and payment of allowances to numerous vaccination personnel at the grassroots level are enormous operational challenges in a country the size of Nigeria. Upon donors' request for a t

Stakeholder Perspectives on Digital Health Technologies in Poststroke Self-Management and Rehabilitation: Systematic Review Based on the NASSS Framework.

BACKGROUND: Digital health technologies (DHTs) offer opportunities to support poststroke self-management and rehabilitation, but their implementation may be hindered by mismatches between technological design, stakeholder needs, and care contexts. Synthesizing stakeholder perspectives can clarify implementation priorities and gaps in the evidence. OBJECTIVE: This study aimed to synthesize stakeholders' perceptions, experiences, and recommendations regarding DHT implementation in poststroke self-management and rehabilitation and to map the findings to the NASSS (Nonadoption, Abandonment, Scale

Health research versus the virus: strengthening systems, saving lives.

For this Commentary, we selected papers from those in this journal's Thematic Series on health research systems' pandemic response. The calling notice for papers suggested possible use of a WHO framework for analysing health research systems (HRSs). Whilst it was not widely used in the reported studies, it did provide the basis for the two main, overlapping, topics for analysis in this Commentary. These, in turn, informed the selection criteria for papers. First, we selected papers that described the contributions made towards meeting the needs for pandemic-related research in at least one are

Global Prosthetic Provision Rate After Major Lower Extremity Amputation for Dysvascular Populations From Four Continents: A Scoping Review.

IMPORTANCE: Lower extremity amputation (LEA) is a worldwide public health problem leading to large disability burdens and health care costs. After LEA, prostheses improve functional mobility and quality-of-life. However, health care costs are high and prosthetic provision worldwide is unknown. OBJECTIVE: The objective was to map the worldwide evidence for prosthetic provision rates after major dysvascular LEA to identify evidence gaps. DESIGN: This scoping review followed best practices and PRISMA-Sc guidelines. Of 609 abstracts screened and 28 full texts reviewed, 18 articles were included.

Exploratory multi-omics analysis reveals host-microbe interactions associated with disease severity in psoriatic skin.

BACKGROUND: Psoriasis (Pso) is a chronic inflammatory skin disease that poses both physical and psychological challenges. Dysbiosis of the skin microbiome has been implicated in Pso, yet a comprehensive multi-omics analysis of host-microbe interactions is still lacking. To bridge this gap, we conducted an exploratory study by adopting the integrated approach that combines whole metagenomic shotgun sequencing with skin transcriptomics. METHODS: This was a cross-sectional study, adult patients with plaque-type Psoriasis (Pso) and healthy volunteers were included. Skin microbiota samples and bio

Lessons learned from the polio eradication initiative in the Democratic Republic of Congo and Ethiopia: analysis of implementation barriers and strategies.

BACKGROUND: Since its inception in 1988, the Global Polio Eradication Initiative (GPEI) has partnered with 200 countries to vaccinate over 2.5 billion children against poliomyelitis. The polio eradication approach has adapted to emerging challenges and diverse contexts. Knowledge assets gained from these experiences can inform implementation of future health programs, but only if efforts are made to systematically map barriers, identify strategies to overcome them, identify unintended consequences, and compare experiences across country contexts. METHODS: A sequential explanatory mixed method

[Historical context of parasitological studies in tropical area, malaria as an example].

Review paper deals with the historical analysis of intellectual activity of some famous parasitologists working in tropics on malaria in the XIX/XX century: A. Laveran (1845-1922), R. Ross (1857-1932), P. Manson (1844-1922), B. Grassi (1854-1925), and other Nobelprize researchers: P. H. Müller (1899-1965) and J. W. von Jauregg (1857-1940). Those chief investigations are interpreted from the point of view of the so-called anthropology of knowledge based on L. Fleck's (1896-1961) philosophy and sociology of science. Parasitological investigations, especially in the field of malaria, were underta

Quality Assurance in Laparoscopic Gynecological Training: A Narrative Review of Standards with a Multicenter Expert Perspective on Pathways Toward Effective Education and Care.

Background: Quality assurance (QA) is a cornerstone of safe and effective postgraduate training in gynecological laparoscopic surgery, where clinical outcomes depend on procedural complexity, equipment sophistication, and the coordinated competence of an entire surgical team. This article is a narrative review of the conceptual, professional, and regulatory foundations of QA in healthcare education, complemented by a structured synthesis of the collective teaching and clinical experience of the author group. It is not an empirical study, and the sections that follow present a conceptual mappin

The Brandenburg reformed medical curriculum: study locally, work locally.

The Brandenburg Medical School "Theodor Fontane" (MHB) was founded in 2014 by municipal and non-profit institutions in Bernau, Brandenburg an der Havel and Neuruppin to train more physicians for the non-metropolitan region of Brandenburg. Since the 2015 summer term, 48 medical students have been enrolled each year, accepted through the university's own selection process in which the score on the German school-leaving exam (Abitur) and time spent on the waiting list play subordinate roles. Tuition fees can be partially financed through scholarship agreements with regional hospitals if the appli
