Congenital Mallet Finger: A Case Report.

This study describes a rare case of congenital hypoplasia of the terminal extensor tendon in the fifth finger, resulting in a mallet finger-like deformity, and presents the surgical technique used to correct the deformity. The case involves a three-year-old girl with a flexion deformity of the distal interphalangeal joint of the left fifth finger. Initial conservative treatment was unsuccessful, leading to surgical intervention, which involved resecting the hypoplastic portion of the extensor tendon and directly suturing the healthy proximal and distal portions. The surgery was reinforced with

Trigger finger appearing as gradually increasing digital nerve disorder after surgical treatment.

Trigger finger is a common disease, and operative treatments are often applied for it. Digital nerve injury is one of the complications of this surgical treatment, and paresthesia and sensory disturbance occur early after the operation. This paper presents a case of trigger finger appearing gradually as increasing digital nerve disorder after surgical treatment. In the second surgery, scar tissue covered the palmar MP joint where the A1 pulley had existed before, and palmar digital neurovascular tissue of the ulnar side was found on the inside of the scar. The ulnar digital nerve showed swelli

Osteosarcoma of the hand: a rare case for radiographic appearance, location, and age.

OBJECTIVE: This case describes the history, process of correct diagnosis, and treatment of a patient with pain in her left fourth finger, later found to be an osteosarcoma. CLINICAL FEATURES: A 28-year-old woman presented with pain in her left fourth finger, which she also noticed had been "getting bigger." Moderate tenderness and decreased range of motion were noticed over the metacarpophalangeal joint of the involved finger. INTERVENTION AND OUTCOME: Initial radiographs indicated a benign tumor with a pathological fracture, and the patient was referred to a hand surgeon. After debridement

Treatment of comminuted distal radius fractures: an approach based on pathomechanics.

Following dorsally displaced fractures of the distal radius, the classic position of immobilization is with the wrist flexed and in ulnar deviation. This is not the position of function and entails morbidity in the form of finger stiffness, which may require prolonged rehabilitation. We treated 20 consecutive, comminuted, intraarticular distal radial fractures using a new external fixation system with the wrist in a neutral to extended position, thereby promoting metacarpophalangeal joint flexion by relatively relaxing the finger extensor tendons. Supplemental pin fixation was used in eight ca

Skilled throwers use physics to time ball release to the nearest millisecond.

Skilled throwers achieve accuracy in overarm throwing by releasing the ball on the handpath with a timing precision as low as 1 ms. It is generally believed that this remarkable ability results from a precisely timed command from the brain that opens the fingers. Alternatively, precise timing of ball release could result from a backforce from the ball that pushes the fingers open. The objective was to test these hypotheses in skilled throwers. Angular positions of the hand and phalanges of the middle finger were recorded with the search-coil technique. In support of the backforce hypothesis, w

Attempt to Make the Upper-Limb Item of Objective Fugl-Meyer Assessment Using 9-Axis Motion Sensors.

The Fugl-Meyer Assessment (FMA) has been used as a functional assessment of upper-limb function in stroke patients. This study aimed to create a more objective and standardized evaluation based on an FMA of the upper-limb items. A total of 30 first-ever stroke patients (65.3 ± 10.3 years old) and 15 healthy participants (35.4 ± 13.4 years old) admitted to Itami Kousei Neurosurgical Hospital were included. A nine-axis motion sensor was attached to the participants, and the joint angles of 17 upper-limb items (excluding fingers) and 23 FMA upper-limb items (excluding reflexes and fingers) were m

The significance of extensor tendon tethering and dorsal joint capsule tightening after injury to the hand.

This paper examines the clinical problem of extensor tendon tethering and/or dorsal joint capsule tightening secondary to hand injury. One hundred and forty-one patients were examined 13 to 51 months after hand injuries of varying severity. Fifty-six patients (40%) had suffered simple and eighty-five patients (60%) complex injuries. Seventy-four (52%) of the 141 patients had no extensor tendon tethering and/or dorsal joint capsule tightening. In 30 (21%), the extensor tendon tethering and/or dorsal joint capsule tightening was considered to be "obvious" in that it was easily seen on examinatio

An evaluation of the contact forces on the fingers when squeezing a spherical rehabilitation ball.

The rehabilitation squeeze ball is a popular device to help strengthen the hand, fingers and forearm muscles. The distributions of the contact pressure in the interface between the therapy ball and hand/fingers can affect the joint moment of each of the individual fingers, thereby affecting rehabilitation effects. In the current study, we evaluated the contact force distributions on the fingers when gripping a spherical object. Eight female adults [age 29 (9.1) years, mass 64.6 (7.1) kg, height 163.5 (1.9) cm, hand length 17.2 (0.7) cm] participated in the study. Contact force sensors were att

The tension band principle and angular testing of extensor tendon repairs.

Extensor tendons in the finger are flat and not amenable to repair by core and epitendinous sutures. Mattress sutures and Kessler repairs without epitendinous stitching are often used for extensor tendon divisions in the fingers. Except when in full extension, the finger presents a series of curved surfaces (at each joint) to the tendon. It was hypothesized that extensor tendons are subject to the 'tension band' principle and that they might be amenable to repair by dorsal-only epitendinous sutures. A Silfverskiöld dorsal-only repair was compared with mattress and Kessler repairs in vitro on a

Correction of non-traumatic extensor tendon dislocation and ulnar drift at the metacarpophalangeal joint by the modified Dell technique.

BACKGROUND: Chronic ulnar extensor tendon dislocation at the metacarpophalangeal joint causes permanent extension deficit and ulnar drift. Several soft tissue procedures have been described for realignment of the extensor tendon in post-traumatic reducible instability or in combination with joint replacement in case of altered metacarpophalangeal joint in inflammatory conditions. However, no studies reported correction of both extension lag and ulnar deviation following isolated surgical treatment of ulnar extensor tendon dislocation at the metacarpophalangeal joint. The present study evaluate

Joint laxity and the risk of clubfoot.

BACKGROUND: Clubfoot is a relatively common and potentially disabling birth defect. The etiology of clubfoot (talipes equinovarus [TEV]) is not known. Joint laxity, or hypermobility, is a suspected risk factor for congenital hip dislocation and clubfoot. METHODS: We conducted a case-control study of TEV in five western Washington counties. A total of 239 cases of TEV were identified from hospital and outpatient sources from 1986 to 1994. Controls (n = 356) were identified by random-digit dialing (RDD). Parents were interviewed over the telephone, and medical records were abstracted. In-person

Lengths, girths, and diameters of children's fingers from 3 to 10 years of age.

We obtained data on the lengths, girths, and diameters of the fingers of children from 3 to 10 years of age. A total of 160 children (78 girls, 82 boys) were examined in a cross-sectional investigation. The length of each finger of the right hand of every child was measured, as were the girths of the proximal, middle, and distal phalanges, and of the proximal and distal interphalangeal joint. The average length of the thumb was 49 (35-65) mm. The index and ring fingers both averaged 69 (index, 50-88; ring, 42-96) mm in length, while the middle and little fingers averaged 72 (57-100) and 56 (40

Staged Tendon Repair to Improve Range of Motion in Tamai Zone 4 Replantation: Two Case Reports.

Tamai zone 4 replantation, defined as the replantation at a level proximal to the flexor digitorum superficialis' insertion and distal to where the common digital artery branches into the proper digital artery, has poor functional results because making orthosis and rehabilitation protocols that protect the bone and the flexor and extensor tendons simultaneously difficult. Two cases of Tamai zone 4 replantation are presented: one case of an index finger replantation at the proximal phalanx and a case of ring finger replantation at the proximal interphalangeal joint. Prospective Randomized Clinical Trial Comparing 3-Point Prefabricated Orthosis and Elastic Tape Versus Cast Immobilization for the Nonsurgical Management of Mallet Finger.

PURPOSE: The purpose of this randomized trial was to compare the outcomes of using a 3-point prefabricated orthosis with elastic tape versus cast immobilization for the management of nonsurgical mallet finger. METHODS: This study was conducted in a single center. Individuals with a mallet injury requiring nonsurgical management were randomized to 6 weeks of full-time immobilization with either a 3-point prefabricated orthosis and elastic tape or a cast for distal interphalangeal joint extension. Outcomes were assessed at 12 weeks after the initiation of full-time immobilization and 6 months a

Successful conservative treatment outcomes and clinical characteristics of congenital hypoplasia of the extensor tendon central slip.

UNLABELLED: Congenital hypoplasia of the extensor tendon central slip is a rare entity. This article describes the clinical characteristics in a series of 22 fingers in 16 patients (mean age: 10 months), and the outcomes of conservative treatment. Nine of 22 fingers were classified as slender or hypoplastic. Treatment with bracing was successful in 21 digits, resulting in full active extension of the proximal interphalangeal joint at a mean of 8.5 months after treatment. Bracing was unsuccessful in one digit, in which operative treatment resulted in a successful outcome. Some residual deformit

Surgical Repair with External Fixation of Epiphyseal Fractures of the Proximal Phalanges of Three Fingers: A Case Report.

A 13-year-old girl sustained epiphyseal fractures of the proximal phalanges of the left index, middle, and ring fingers. Though manual reduction of the 3 fingers was possible, it was difficult to maintain the reduction due to severe instability of the middle and ring fingers, and closed reduction with external fixation was performed. At 4 years post-injury, the patient had no impairment of daily activities. The use of external fixation (1) causes no injury to the epiphyseal cartilage, (2) enables accurate reduction and maintenance of reduction, (3) is technically easier than pinning, (4) enabl

Two flaps to resurface the basal flexion-crease of the finger area.

Two flaps are described which have been designed to resurface the skin around the basal flexion crease of the fingers. Their most common use is on the ulnar side of the hand but any finger can be resurfaced. Both flaps are 1 cm in width so the donor sites can be repaired directly without the use of skin grafts. Mobilisation of the fingers is therefore permissible within 24 hours and thus postoperative stiffness avoided. The one-stage cross-finger flap is of particular value in resurfacing and preventing the recurrence of Dupuytren's in the M.P. joint area. The palmar transposition flap based o

Combined usage of hydroxyapatite and cross-finger flap for fingertip reconstruction.

Reconstruction for fingertip defects categorized as Type 3 and Type 4 in Allen's classification is challenging, because surgeons need to reconstruct not only the pulp but also great parts of the distal phalangeal bone. This paper introduces an original technique for the reconstruction of defects of these types. The defects of seven fingers (two small fingers and five index fingers) of seven patients (three males and four females; aged 14-44 years) were repaired. After the fingertip is divided in a fish-mouth fashion to expose the stump of the distal phalangeal bone, a curved block of hydroxyap
