Management of Rectovaginal Fistulae.

A rectovaginal fistula is an epithelialized connection between the rectum and the vagina. These fistulae are stratified based on location, complexity, sphincter damage, and the status of the surrounding tissue. Multiple surgical treatments options are available and should be individualized based on the specific circumstances. Even in the most experienced hands recurrences are common. Recurrent fistulae often require the precise placement of well-vascularized transpositional flaps. Diversion is frequently required to control the symptoms of vaginal soilage and urinary tract infections, but it d…

[Carotid-cavernous fistulas].

BACKGROUND: Carotid cavernous fistulas (CCFs) are rare but clinically significant vascular anomalies characterized by abnormal connections between the cavernous sinus and arteries. This overview presents a comprehensive analysis of anatomy, classification, clinical presentation, diagnosis, imaging, and therapy of CCFs. The cavernous sinus, a central venous structure in the brain, is of critical importance for understanding CCFs due to its proximity to key structures such as the internal carotid artery and cranial nerves. CLASSIFICATION: CCFs are classified into direct and dural types, with di…

[Esophagotracheal and esophagobronchial fistulas].

Esophagobronchial and esophagotracheal fistulas are rare but complex diseases with a heterogeneous spectrum of underlying etiologies. Common causes are locally advanced tumors of the esophagus and larynx, traumatic perforation from the esophageal or tracheal side as well as postoperative fistulas. The management of esophagotracheal and esophagobronchial fistulas always involves different health care providers and in most cases patients require a multidisciplinary treatment on the intensive care unit. The therapeutic concept primarily depends on the underlying cause, localization and size of th…

[Cryptoglandular anal fistulas].

Cryptoglandular anal fistulas (AF) cause recurrent anal abscesses and patients risk multiple surgeries due to low healing rates of sphincter-saving procedures. Knowledge of anal anatomy and imaging with MRI or endoanal sonography is crucial to classify AF as simple or complex depending on risk of anal incontinence after fistulotomy as summarised in this review. Fistulotomy has healing rates of > 90%, risks incontinence, and the procedure is reserved for simple fistulas. Complex AF are treated with a draining seton and then with sphincter-saving procedures which have long-term healing rates of …

Female Pelvic Fistulae.

Female pelvic fistulae are a pathologic connection between the urinary or gastrointestinal tract and the genital tract. Although this condition has been present for millennia, successful surgical treatments have only been described for the past few hundred years. In developed nations, the most common cause of genitourinary fistulae is benign gynecologic surgery, but worldwide it is obstetric trauma. Fistulae management is rooted in surgical intervention with the highest probability of success associated with the first repair.

Coronary artery fistulae.

Coronary artery fistulae (CAF) are rare congenital or acquired in which a connection forms between one of the coronary arteries and a heart chamber or with other vessels. This paper describes three cases of CAF along with their initial presentation, imaging findings and management. The first case is a rare form of CAF in which the left circumflex coronary artery fistula empty into left ventricle. We discuss the different types of CAF along with their prevalence and the different imaging tools that could be utilized to identify CAF. There is no unifying consensus on treatment strategy for sympt…

Management of enteroatmospheric fistulae.

A small-bowel enteroatmospheric fistula (EAF) is an especially challenging complication for patients with open abdomens (OAs) and their surgeons. Manipulation of the bowel during treatment (e.g. dressing changes) is one of the risk factors for developing these openings between the atmosphere and the gastrointestinal tract. Unlike enterocutaneous fistulae, EAFs have neither overlying soft tissue nor a real fistula tract, which reduces the likelihood of their spontaneous closure. Surgical closure is necessary but not always easy to do in the OA environment. Negative pressure wound therapy (NPWT)…

Coronary artery fistulae.

Coronary artery fistulae and coronary cameral fistulae are rare anomalies that are discovered incidentally in patients undergoing coronary angiography. This article reviews the classification, management, and complications of these fistulae, and discusses a variety of presentations.

Uretro-arterial fistulas.

Ureteral arterial fistula (UAF) is an uncommon condition but one that has been increasing with over 100 cases reported. The presentation is gross hematuria in a patient with predisposing factors, such as prior pelvic surgery, past radiation therapy, and chronic ureteral stenting. When not correctly diagnosed, the associated mortality is significant. Diagnosis is best accomplished by careful angiography with frequent use of provocative maneuvers. The current best therapeutic option is sealing off the UAF by deploying a stent graft in the artery.

[Cryptoglandular anal fistulas].

Cryptoglandular anal fistulae are the most frequently occurring form of perianal sepsis. Characteristically they have an endoanal primary opening, a fistula track and an abscess and/or an external purulent opening. Antibiotic therapy is not of use in initial management except in special cases. Treatment of an abscess, if present, is required urgently and when possible, consists of its incision under local anaesthesia. Treating the fistula track occurs afterwards and aims to dry up the purulent discharge and avoid recurrence of the abscess by means of surgical fistulotomy. These techniques are …

[Uro- and enterovaginal fistulas].

BACKGROUND: Gynecological fistulas that cause faecal or urinary incontinence, represent a considerable global health problem that usually reflects inadequate help at birth. The problem has a different profile in the more industrialised countries. The aim of the present study was to characterise gynaecological fistulas in a Norwegian setting. MATERIAL AND METHODS: Data (medical history, type of fistula, treatment and outcome) were recorded from women assessed and treated for uro- and enterogenital fistulas in 1995-2005 at the Woman's clinic Haukeland University Hospital. RESULTS: Of 82 women,…

Anal abscesses and fistulas.

Anal abscesses and fistulas are a common part of surgical practice. Most abscesses simply need to be drained and most fistulas can be safely laid open. Excessive probing should not be attempted when draining abscesses as this may lead to iatrogenic fistulas. A small percentage of fistulas are complex and very challenging to manage. Management involves an accurate diagnosis and a balance between eradication of the fistula and maintenance of continence. A decision should be made, based on clinical evaluation and anal ultrasound (if available), whether the fistula can be laid open. If it cannot b…

[Treatment of pancreaticopleural fistulas].

Pancreaticopleural fistulas are rare but serious complications of chronic or recurrent inflammatory pancreatic disease. Massive unilateral or bilateral pleural effusion may occur in association with rupture of pancreatic duct or pseudocyst into the pleural cavity. In the past decade we have treated 10 patients with pancreaticopleural fistulas. Eight of them had a previous history of inflammatory pancreatic disease. Diagnosis was made by finding a markedly elevated amylase level (10 pts) in the pleural fluid. US and CT examination enabled the establishment of the diagnosis of pancreatic origin,…
