Protein-losing enteropathy after fontan surgery: insights from Vietnamese clinical experience and the need for global collaboration.

Protein-losing enteropathy remains a formidable and underrecognised complication following Fontan surgery, contributing significantly to morbidity and mortality among affected patients. Despite substantial progress in managing single-ventricle CHDs, protein-losing enteropathy persists as a major clinical challenge, particularly in resource-constrained environments where specialised diagnostic modalities are scarce. This manuscript discusses the prevalence, clinical presentation, diagnostic hurdles, and management of protein-losing enteropathy in post-Fontan patients, drawing from clinical expe

Optimal Strategy for Thromboprophylaxis in Fontan Circulation: A Systematic Review and Meta-analysis with a Focus on Ethnic Differences.

Fontan circulation alters cardiovascular hemodynamics to maintain circulation using a single ventricle, which may consequently increase the risk of thromboembolism. This highlights the need for effective thromboprophylaxis strategies. This study assessed optimal thromboprophylaxis regimens for patients with Fontan circulation through a comprehensive meta-analysis of literature focused on personalized, ethnicity-based approaches. PubMed, Embase, and Cochrane Library databases were searched to identify studies reporting the thromboembolic and bleeding outcomes of patients with Fontan circulation

Optimizing outcomes for patients undergoing superior cavopulmonary connection procedure: what we know.

The Superior cavopulmonary connection (SCPC) is typically the second stage of the single ventricle palliation. There are modifiable factors that have a complex interplay in determining outcomes such as mortality or hospital length of stay. There is no clear consensus on the timing of SCPC as this involves a critical balance between the timing of operation and the length of the high-risk interstage period. Younger age has not been associated with reduced transplant-free survival but has been associated with longer hospital length of stay. Another critical decision is for children with viral res

Mountains and Waves: Fontan Circulation in Different Environmental Conditions.

As surgical options and medical care for patients with univentricular heart physiology continue to improve, leading to increased life expectancy and quality of life, a new population of Fontan patients is growing up with the desire to participate in leisure activities, including aquatic activities, high-altitude stays, and air travel. Due to significant data gaps and insufficient experience, current guidelines do not provide clear recommendations, leading to uncertainty and sometimes restrictive patient management. This review summarizes new insights and the current state of research on this s

Plastic bronchitis: A rare complication following a motor vehicle collision.

Plastic bronchitis, more appropriately termed chyloptysis, is a rare and potentially fatal condition caused by chylous coating of the airways. These cast coating can dislodge and become an obstructive mass in the patient's airway, necessitating rapid intervention. PB is well described to occur following single ventricle physiology heart disease corrective procedures, particularly following Fontan procedures. It is less commonly seen in traumatic settings. We present the youngest known case of a traumatic injury induced plastic bronchitis. A 19-year-old man was involved in a motor vehicle accid…

Cyanosis in a patient after Fontan palliation due to unrecognised hepatic vein to coronary sinus communication.

A 6-year-old male with heterotaxia, abnormal systemic and pulmonary venous drainage, and a history of Fontan completion presented with desaturations and was found by cardiac catheterisation to have a hepatic vein to coronary sinus connection. This was successfully occluded using an Amplatzer Muscular Ventricular Septal Defect Occluder.

Subperiosteal Orbital Hematoma From SVC Occlusion During Cardiac Surgery: To See or Not to See.

We present a case in which the superior vena cava (SVC) cannula was inadvertently clamped for a short while during cardiopulmonary bypass, completely occluding SVC drainage. This resulted in a rarely seen complication - bilateral subperiosteal orbital hematomas causing orbital compartment syndrome. Other instances of intentional SVC occlusion include during the creation of a bidirectional cavo-pulmonary shunt and for emergency control of bleeding during thoracic surgery.

Protein-losing enteropathy managed with percutaneous enlargement of a restrictive atrial septal defect.

Protein-losing enteropathy is one of the most feared complications of the Fontan circulation. The diagnosis of protein-losing enteropathy in this setting should prompt a thorough investigation for the presence of a treatable hemodynamic impairment. In this report, we describe a complete reversal of protein-losing enteropathy following percutaneous enlargement of a restrictive atrial septal defect in a patient with a fenestrated lateral tunnel Fontan and severe mitral stenosis.

The Adult Patient with a Fontan.

The authors summarize the most important anatomic and physiologic substrates of Fontan circulation. Common anatomic substrates include hypoplastic left heart syndrome, tricuspid atresia, double inlet left ventricle, and unbalanced atrioventricular septal defects. After the Fontan operation exercise capacity is limited and the key hemodynamic drivers is limited preload due to a relatively fixed pulmonary vascular resistance. The authors provide contemporary data on survival, morbidity, and need for reintervention. Operative morality is now expected to be less than 1% and 30 year survival approx

Computational modeling of a right-sided Fontan assist device: Effectiveness across patient anatomies and cannulations.

The use of mechanical circulatory support for failing Fontan patients is an area of growing interest, as the increased life expectancy of these patients continues to be accompanied by numerous end-organ complications. In vitro work has shown positive results using the CentriMag device for right-sided Fontan support, however the generalizability across various patient anatomies and cannulations is unknown. Computational simulations are first validated against in vitro modeling, then used to assess generalizability and further explore hemodynamic metrics including relative pressure changes, hepa

Improved survival after heart transplant for failed Fontan patients with preserved ventricular function.

BACKGROUND: Patients with a failing Fontan continue to have decreased survival after heart transplant (HT), particularly those with preserved ventricular function (PVF) compared with impaired ventricular function (IVF). In this study we evaluated the effect of institutional changes on post-HT outcomes. METHODS: Data were retrospectively collected for all Fontan patients who underwent HT. Mode of failure was defined by the last echocardiogram before HT, with mild or no dysfunction considered PVF and moderate or severe considered IVF. Outcomes were compared between early era (EE, 1995 to 2008)

Obstructive jaundice secondary to Morgagni hernia in an infant with Fontan circulation.

The vast majority of congenital diaphragmatic cases present with severe respiratory distress in the immediate neonatal period. Obstructive jaundice secondary to CDH is a rare presenting symptom, previously unreported with Morgagni type hernias which comprise 2% of all CDHs. Although the safety of minimally invasive repair for CDH has been shown previously, there has been little data on its safety for patients with complex congenital cardiac disease. In this case report, we present an 18-month old infant with Fontan circulation presenting with jaundice secondary to Morgagni hernia who ultimatel

Experiences with fenestration closure in patients after the Fontan operation.

BACKGROUND AND OBJECTIVES: [corrected] In this study, decision and timing of fenestration closure are discussed. We review our experiences with transcatheter occlusion of fenestration or residual baffle leak using Amplatzer occluder. PATIENTS AND METHODS: We have retrospectively reviewed the cath lab records of 26 patients who have undergone closure of fenestration (or another baffle leak) in period of 2002 to July 2006. Patients with the previous fenestrated Fontan procedure (intracardiac lateral tunnel, extracardiac conduit) with the presence of clinically important cyanosis, arterial desat…

Impact of arrhythmia circuit cryoablation during Fontan conversion for refractory atrial tachycardia.

Refractory atrial arrhythmias in late postoperative Fontan patients are usually associated with residual hemodynamic abnormalities and result in significant morbidity and mortality. Surgical revision of the Fontan anastomosis may improve hemodynamics without eliminating tachycardia. This study sought to assess the impact of surgical cryoablation of the arrhythmia circuit at the time of Fontan conversion on the clinical recurrence of tachycardia. Sixteen consecutive atriopulmonary Fontan patients with refractory atrial arrhythmias underwent surgical conversion to lateral tunnel total cavopulmon

Computer simulation of circulation in patient with total cavo-pulmonary connection: inter-relationship of cardiac and vascular pressure, flow, resistance and capacitance.

The aim is to develop a computer model representative of the circulation in a patient with a uni-ventricular heart surgically palliated by a total cavo-pulmonary connection (TCPC). The effects of known hazardous exposures on this type of circulation are investigated. A model of the cardiovascular system is built using standard components such as transmission lines, restrictors and capacitances. The chamber of the heart consists of a volume connected to checkvalves, and an oscillating source flow connected to the volume represents the pumping of the heart. The following are simulated: exposure

Patients With Hypoplastic Left Heart Syndrome Have a Shorter Superior Vena Cava.

BACKGROUND: The primary treatment for hypoplastic left heart syndrome (HLHS) is the Fontan pathway, which entails performing the Glenn procedure. We hypothesized that the superior vena cava in patients with HLHS was short. As the length of the superior vena cava influences the Glenn procedure, we compared its length between patients with HLHS and those with other congenital heart diseases. METHODS: Patients with HLHS or its variant, patients with ventricular septal defects (VSD), and patients with pulmonary atresia with intact ventricular septum (PA/IVS)-including critical pulmonary stenosis-

Problems in the modified Fontan operation for univentricular heart of the right ventricular type.

Thirteen patients (ages 4 to 16 years) with univentricular heart of right ventricular type, nine with double-inlet right ventricle (DIRV), and four with mitral atresia who underwent a modified Fontan operation were reviewed. Among those with DIRV, right isomerism with a common atrioventricular (AV) valve was found in eight and situs inversus in one; among those with mitral atresia, AV discordance was found in two and concordance in two. Intra-atrial routing using a baffle with atriopulmonary anastomosis was the main procedure (11 patients). There were two operative deaths (one DIRV, one mitral

Stable Cytokine Network during Hypoxia and Exercise in Patients with Fontan Circulation.

BACKGROUND: Patients with Fontan circulation are often advised to avoid hypoxic exposure due to presumed cardiopulmonary vulnerability. Low-grade inflammation has also been reported in this population and may be influenced by hypoxia and/or exercise. Based on the potential interaction between hypoxia and submaximal exercise in modulating inflammatory signaling, we hypothesized that this combination could exacerbate subclinical inflammation. METHODS: Eighteen clinically stable patients with Fontan circulation (age 25 ± 6 years, 9 female, NYHA I) underwent a submaximal cycling step test under n
