Onychomycosis in Adana, Turkey: a 5-year study.

BACKGROUND: Onychomycosis results from invasion of the nail plate by dermatophytes, yeasts or mould species of fungi. This report documents the etiological agents of onychomycosis in Adana, Turkey. METHODS: Results of mycological tests of nail clippings or subungal scrapings collected in the period between 1998 and 2002 were analyzed. Both the direct microscopy and the cultures of the nail material were performed to identify the causative agent. RESULTS: Out of a total of 388 cases examined, finger (12.4%) and toenail (87.6%) involvement, 155 (39.9%), were mycologically proven cases of onych

Palmoplantar lichen planus presenting with vesicle-like papules.

Palmoplantar lichen planus is a rare, localized variant of the disease that may create difficulty in diagnosis if it is present as an isolated finding. Although several morphological patterns may be seen, plaques or small papules with compact hyperkeratosis are usually observed. We present a 25-year-old woman with a one-month history of slightly pruritic, red papules on her palms and soles. Dermatologic examination revealed numerous, asymptomatic, unscaly, red papules on her palms and soles, some of them resembling vesicles and white reticulate plaques on both sides of the buccal mucosa. The d

Lichen planus with lesions on the palms and/or soles: prevalence and clinicopathological study of 36 patients.

According to the literature, palmoplantar lesions in lichen planus (LP) are uncommon, and do not usually have the classically described clinical morphology. The aim of this study was to evaluate the prevalence of palmoplantar lesions in LP and to describe the characteristic clinicopathological appearances of LP affecting palms and soles. Palmoplantar LP with accompanying skin involvement accounted for 26% of our cases. The presence of very pruriginous erythematous scaly and/or hyperkeratotic plaques, with well-defined edges, located on the internal plantar arch, without involvement of the fing

Itraconazole pulse therapy for onychomycosis and dermatomycoses: an overview.

BACKGROUND: Itraconazole is a broad-spectrum antifungal agent that has been used to treat dermatomycosis and onychomycosis using continuous therapy. More recently the drug has been used as pulse dosing. OBJECTIVE: Our purpose was to review the studies in which itraconazole pulse therapy (PT) has been administered in the management of dermatomycoses. RESULTS: For tinea pedis and manuum, the recommended dosage is itraconazole 200 mg twice daily for 1 week (n = 220). A clinical response and mycologic cure rate of 90% +/- 4% and 76% +/- 6%, respectively, has been obtained. For tinea corporis/cru

Evaluation of 6 weeks treatment of terbinafine in tinea unguium in a double-blind trial comparing 6 and 12 weeks therapy. The Lagos V Study Group.

Terbinafine (Lamisil) has been registered throughout the world for the treatment of finger and toenail onychomycosis. The recommended duration of treatment of toenail onychomycosis based on phase III studies is 12 weeks. This study was designed to determine: (i) if patients in whom the proximal part of the toenails was not affected respond as well after 6 weeks treatment as after 12 weeks treatment; (ii) to identify factors which may allow selection of patients for shorter treatment duration; and (iii) confirm that 6 weeks therapy is sufficient in fingernail mycosis. Infantile acropustulosis in six immigrant children.

Infantile acropustulosis is a recurrent, pruriginous, vesiculopustular eruption of the palms and soles first described in 1979. We report six cases of infantile acropustulosis in recently emigrated children treated for scabies. Clinical follow-up was obtained by questionnaire addressed to patients' families and general practitioners. Our study suggests infantile acropustulosis is frequent in immigrant infants and could be a non-specific hypersensitivity reaction to Sarcoptes scabiei.

Papular-purpuric 'gloves and socks' syndrome: not only parvovirus B19.

We previously described an acute dermatosis characterized by pruritic erythematous and slightly papular lesions on the hands and feet in a 'gloves and socks' distribution associated with oral aphthoid lesions and fever (papular-purpuric 'gloves and socks' syndrome = PPGSS). We strongly suspected a viral origin, but serologic tests for a large panel of viruses remained negative. Subsequently, 2 cases of PPGSS with serologic evidence of a parvovirus B19 infection have been reported in the literature. Since then we observed 5 additional patients with a PPGSS. Parvovirus B19 infection could be con

Thyroid function in patients with pustulosis palmoplantaris.

Fifty consecutive patients (45 women and 5 men) with pustulosis palmoplantaris were evaluated for thyroid disease. Five women had previously had thyroid disorders for which they were receiving thyroxine substitution therapy. Of the other 40 women three had a raised serum concentration of thyroid-stimulating hormone and nine had high concentrations of antibodies against thyroid "microsomal antigen." During a 4-year follow-up hypothyroidism developed in two women with initially raised thyroid-stimulating hormone, and laboratory test results for one woman indicated subclinical hyperthyroidism. Fo

Blistering diseases in children. How to recognize and treat the most common.

The most common blistering diseases encountered in children are impetigo, burns, acute dermatitis, friction blisters, viral blisters, insect bite reactions, and linear IgA dermatosis. Diagnosis should be based on specific clinical manifestations and, when necessary, results of a Tzanck test or skin biopsy. A search for an intact blister is always warranted when erosions, oozing, or crusts are noted.

The prevalence of accentuated palmoplantar markings and keratosis pilaris in atopic dermatitis, autosomal dominant ichthyosis and control dermatological patients.

The prevalence of keratosis pilaris and accentuated palmoplantar marking was evaluated in 61 patients with atopic dermatitis, 35 patients with dominant ichthyosis vulgaris and 247 other dermatological cases taken as controls. Our data showed that (1) these features are of no diagnostic significance for atopic dermatitis and (2) they are significantly more frequent in patients with ichthyosis vulgaris without associated eczema than in those with atopic dermatitis. Consequently, they should be considered as part of the phenotype of ichthyosis vulgaris rather than attributed to a concomitant atop

[Anterior thoracic and intervertebral erosive joint diseases associated with palmoplantar pustulosis].

The authors report three cases of palmo-plantar pustulosis associated with articular signs: erosive arthritis of the right first sternocostal joint in 2 cases (without hypertrophy of the clavicle or the sternum) and atlanto-occipital arthropathy with marked neck stiffness in another case. The HLA phenotype of one case was: A2 - A9 - B14 - X - DR3 - DR4. A surgical sterno-costal biopsy revealed non-specific inflammatory lesions in 2 cases. In one of these cases, a Corynebacterium sp. was isolated. The clinical course was favourable in response to local antibiotic therapy in one case (follow-up

Immunohistological studies on tonsils with recurrent tonsillitis and focus-tonsil with pustulosis palmaris et plantaris.

By use of monoclonal antibodies and anti-immunoglobulin antibodies, the distributions of 15 adult tonsils with recurrent inflammation and palmoplantar pustulosis as focus-tonsil were investigated. The avidin-biotin-peroxidase complex (ABC) method was utilized throughout the experiment. Monoclonal antibodies of the Leu and OKT series revealed the location of T and B cell subsets characteristic in the tonsils with chronic inflammation and the focus tonsils with pustulosis palmaris et plantaris (PPP). In vitro sensitization of tonsil tissues was performed with crude antigen from tonsillar epithel

Clinical features of 22 cases with "inter-sterno-costo-clavicular ossification". A new rheumatic syndrome.

We present 22 cases with inter-sterno-costoclavicular ossification. Clinical and pathological findings show that abnormal ossification observed in this situation is due to non-suppurative chronic inflammation of the soft tissues around the sterno-costo-clavicular region such as the costo-clavicular ligament. We have classified X-ray findings into three stages according to the extent of the ossification; localized, generalized, and hyperostotic, and show that the disease progressed in this sequence. A considerable number of the patients showed abnormal X-ray findings in the spine or the sacro-i

Fibro-osseous pseudotumor of the digit: Presentation as an enlarging erythematous cutaneous nodule.

Fibro-osseous pseudotumor of the digit is an unusual ossifying fibro-proliferative lesion. The current case is a 54-year-old woman who presented with an enlarging reddish nodule on the distal aspect of her left big toe. Excision biopsy was performed and the histopathology featured a dermal-centered lesion composed of osteoblast-rimmed bony trabculae lying amidst fibrovascular stroma. This entity resembles and may be within the spectrum of myositis ossificans, the prototype post-traumatic reactive fibro-osseous proliferation, although the latter is typically more proximally located and features
