The AO Pediatric Expert Group and the AO Pediatric Classification Group, in cooperation with the AO Investigation and Documentation Group introduce and present the first comprehensive classification of pediatric long bone fractures. The anatomy is related to the 4 long bones and their 3 segments defined as proximal (1), shaft (2) and distal (3). It is further described by the fracture subsegment recorded as epiphyseal (E), metaphyseal (M) and diaphyseal (D), whereby proximal and distal fractures are classified as E or M and shaft fractures are always D. The distinction between metaphyseal and

A 50-year-old male suffered a crash landing while paragliding and sustained a posterior dislocation of the hip with a Pipkin fracture type 4 (fracture of the posterior acetabular wall and Pipkin fracture type 2) and a lesion of the sciatic nerve. After primary treatment in an external hospital, the patient was transferred to this hospital 4 days following the trauma. An operative stabilization of the acetabular fracture and the Pipkin fracture was performed using a trochanter flip osteotomy. Despite a large central defect of the femoral head it was decided to attempt a reconstruction. Followin

INTRODUCTION: Ipsilateral fractures of the shaft of the femur combined with hip dislocations are extremely rare injuries, presenting a difficult diagnostic and therapeutic challenge. Diagnosis of hip dislocation is often delayed, due to the focus on femoral fracture.
CASE PRESENTATION: This article presents a case never described before of a 19-year-old patient who sustained this unusual combination of injuries as a result of a road traffic accident. The patient had an irreducible posterior hip fracture-dislocation associated with an ipsilateral femoral shaft fracture. Closed reduction attemp

A case of severe central fracture-dislocation of the hip migrated deep into the pelvis complicating ileal rupture and ipsilateral comminuted fracture of the proximal femur was described. Plain CT images of the pelvis were distinctive and quite useful for evaluating the severity of the injury. Osteomyelitis of the proximal femur occurred soon after resection of the femoral head and suturing of the peritoneum though laparotomy. Complete closure of the peritoneal laceration and rapid totally diverting colostomy would be recommendable in order to avoid infectious complications in such a severe cen

Background: Restoring the volar lip of the middle phalanx is key to regaining concentric joint gliding after dorsal proximal interphalangeal (PIP) joint fracture-dislocations, but no reliable radiographic assessment currently exists. This study introduces volar tilt as a new radiographic measure. Methods: We evaluated 27 dorsal PIP fracture-dislocations in 25 patients, analysing clinical outcomes, osteoarthritis (OA) and volar tilt on lateral radiographs at injury, after extension block pinning and at 16-year follow-up. Results: Initial volar tilt averaged 22°, improving to 12° after extension

BACKGROUND: It is unclear which patients with fractures and dislocations of the hand necessitate hand therapy referral, which can lead to practice variation.
PURPOSE: The study aimed to evaluate hand therapy referral patterns following non-operative and operative treatments of hand fractures and dislocations and to identify practice variations.
STUDY DESIGN: This multicenter, observational snapshot study was conducted across 12 hospitals in the Netherlands over a 3-month period in 2020.
METHODS: Adult patients admitted to the emergency department with metacarpal and phalangeal fractures or

Ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH) are bone-forming spinal conditions which inherently increase spine rigidity and place patients at a higher risk for thoracolumbar fractures. Due to the long lever-arm associated with their pathology, these fractures are frequently unstable and may significantly displace leading to catastrophic neurologic consequences. Operative and non-operative management are considerations in these fractures. However conservative measures including immobilization and bracing are typically reserved for non-displaced or incomplete

BACKGROUND: Complex elbow fracture dislocations, dislocation with fracture of one or several surrounding bony stabilizers, are difficult to manage and associated with poor outcomes. While many studies have explored treatment strategies but a lack of standardization of patient-reported outcome measures (PROMs) makes cross-study comparison difficult. In this systematic review, we aim to describe what injury patterns, measured outcomes, and associated complications are reported in the complex elbow fracture dislocation literature to provide outcome reporting recommendations that will facilitate i…

OBJECTIVE: Patellar dislocations are a common occurrence in orthopedic practice, often accompanied by osteochondral fractures of the retropatellar cartilage surface, known as flake fractures, in up to 58% of cases. The parachute technique represents a simple and cost-effective surgical option aimed at restoring osteochondral integration and preserving native cartilage.
INDICATIONS: Flake fracture of the patella with osteochondral fragments.
CONTRAINDICATIONS: Patella fracture.
SURGICAL TECHNIQUE: By utilizing transpatellar, absorbable sutures, a stable osteochondral interface is achieved wi

The aim of the present study was to evaluate long-term functional and radiographic outcome in the distal radioulnar joint (DRUJ) for Galeazzi fracture-dislocation after anatomic reduction and rigid fixation of the radius. Fourteen patients, with an average age of 38 years, presenting with Galeazzi fracture-dislocation treated by open reduction and internal fixation (ORIF) of the radius and closed reduction of the DRUJ were retrospectively evaluated, with a minimum follow-up of 6 years. At final evaluation (mean: 8 years), the DRUJ was objectively and subjectively evaluated for range of motion

INTRODUCTION: Proximal humeral fracture with associated glenohumeral dislocation (PHFD) is a challenging clinical problem. Outcomes of open reduction and internal fixation (ORIF) of these injuries have not been widely reported. The purpose of this analysis is to report our experience with ORIF of PHFD.
METHODS: A retrospective review of our 2 institutions' shoulder surgery databases was conducted to identify all PHFDs that were treated with ORIF from 2008 through 2017. Radiographs were reviewed for fracture healing by 12 weeks postoperatively. All reoperations were recorded. Patient-reported

BACKGROUND: Surgical hip dislocation has been described as an excellent method for reduction and fixation of acetabular fractures. It allows a 360-degree access to the acetabulum and acetabular dome. However, reproducibility of this technique is still a concern. The primary outcome of this study was to investigate the short term functional and radiological outcome with the use of surgical hip dislocation in acetabular fractures. The secondary outcome was to describe different indications and methods of reduction using this technique.
METHODS: This is a retrospective consecutive case series fo

BACKGROUND: Acute sternoclavicular fracture-dislocation is associated with high-energy trauma and is being increasingly recognized in children1. These injuries are associated with compression of mediastinal structures and can be life-threatening1. The management of acute sternoclavicular fracture-dislocation includes closed reduction or open surgical stabilization; however, limited success is reported with closed reduction2,3. To our knowledge, there are no detailed descriptions of open reduction and suture fixation of acute sternoclavicular fracture-dislocation in children.

BACKGROUND: Ulnar metacarpal base fractures can destabilize the carpometacarpal (CMC) joint, prompting surgical stabilization. Studies investigating this injury are limited by small case volumes. Our purpose is to review the surgical techniques, outcomes, and complications of ulnar CMC joint stabilization.
METHODS: A literature search was performed of all articles published on the surgical treatment and outcomes of ulnar CMC fracture dislocations using PubMed and Google Scholar databases between the years 2014 and 2019. Data were pooled and analyzed, assessing surgical techniques and hand out…

AIM: The results of conservative treatment of pediatric dislocated (luxative) condyle fractures are usually unsatisfactory. We therefore decided to present and analyze the results of surgical treatment of these fractures.
PATIENTS AND METHODS: Children with dislocated condyle fractures were treated surgically, with the approach always including opening the temporomandibular joint (TMJ). Postoperatively, patients had regular controls at 1 week, 1 month, 3 months, and 6 months, and then yearly thereafter. At each control visit, facial symmetry, maximal mouth opening, lateral chin deflection upo

Background: Treatment of proximal interphalangeal joint (PIPJ) fracture-dislocations is difficult given the potential long-term complications of the involved finger and entire hand. Several surgical methods have been utilized for management of these injuries, none of which have shown consistently favorable results. The purpose of this systematic review of the literature is to report the post-operative outcomes of multiple treatment modalities for PIPJ fracture-dislocations in various studies. Methods: A literature review of PubMed and EMBASE databases was performed for all articles on PIPJ fra

INTRODUCTION: Flail chest is diagnosed clinically by the presence of paradox movement of a segment of the thoracic wall during spontaneous breathing. Radiographic finding confirming a clinical flail chest are fractures of three or more consecutive ribs or costal cartilages in two or more places. Surgical stabilization is associated with a reduced length of hospital stay, time with mechanical ventilation and risk of respiratory complications.
PRESENTATION OF CASE: A trauma patient had a Computed Tomography (CT) scan showing multiple costa fractures, sternal fracture, manubrium fracture, sterna

The Cunéo and Picot fracture-dislocation is an atypical trimalleolar fracture-dislocation of the ankle with unique anatomopathologic and radiographic features, which has not been reported in English literature. We report a case of a 42-year-old woman that was diagnosed with a trimalleolar fracture-dislocation and treated surgically with an open reduction and osteosynthesis of the lateral and medial malleolus. At one-month follow-up, X-rays showed secondary displacement of the medial malleolus requiring revision surgery. The patient complained of persisting pain, with X-rays showing no signs of
