Beyond endoscopy: the ongoing role of frontal osteoplastic flap surgery-a case series and evidence-based analysis.

INTRODUCTION: Frontal sinus surgery is among the most technically demanding areas in rhinology due to the complex anatomy of the frontal rec … indications were frontal sinus osteomas, mucoceles with orbital involvement, and refractory chronic frontal sinusitis. Supraciliary and bicoronal incisions were used, and in some cases, combined with ESS. No major complications occurred, and outcomes were highly satisfactory both functionally and esthetically. CONCLUSION: Despite advancements in endoscopic techniq …

Odontogenic Sinusitis: Clinical, Radiologic, and Quality-of-Life Characterization in a Prospective Study.

Background: Rhinosinusitis is an increasingly prevalent condition that compromises patients' quality of life and imposes a substantial burden on healthcare systems. Odontogenic sinusitis, a predominantly unilateral form of rhinosinusitis, accounts for up to 40% of unilateral maxillary sinusitis cases. This study aimed to characterize the clinical presentation, radiological extension, and quality-of-life impairment associated with odontogenic sinusitis. Methods: Thirty-eight patients with unilateral sinusitis caused by dental pathology/treatment were recruited. OS diagnosis criteria were establ

[Clinical-radiologic correlation in chronic sinusitis].

INTRODUCTION: Chronic sinusitis is a pathology that has been increasing in incidence in world population. The Mexican Institute of Social Security (IMSS) reports 91 cases per year per 100,000 people. In the last decades, much has been advanced concerning knowledge of the physiology of paranasal sinuses and thus of the pathogenesis of sinusitis; this has led to developing of functional endoscopic sinus surgery. To perform this kind of surgery, it is mandatory to have computerized tomography, and the necessity of clinical-radiologic correlation has been recognized to attain better diagnosis and

The Groote Schuur hospital classification of the orbital complications of sinusitis.

The complications of sinusitis have been well described. The most common classifications used for orbital complications have been that of Chandler et al. (1970) and Moloney et al. (1987). With the ready availability of high-resolution computed tomography (CT) scanners, limitations of these classifications have become apparent. The aims of this study were to determine the relative frequency of the various complications associated with acute sinusitis, to determine which groups of sinuses were most frequently involved and to correlate the orbital signs with a new proposed classification of orbit

[Aerosinusitis: part 1: Fundamentals, pathophysiology and prophylaxis].

The relevance of aerosinusitis stems from the high number of flight passengers and the impaired fitness for work of the flight personnel. The frontal sinus is more frequently affected than the maxillary sinus and the condition generally occurs during descent. Sinonasal diseases and anatomic variations leading to obstruction of paranasal sinus ventilation favor the development of aerosinusitis. This Continuing Medical Education (CME) article is based on selective literature searches of the PubMed database (search terms: "aerosinusitis", "barosinusitis", "barotrauma" AND "sinus", "barotrauma" AN

Classification system of frontal sinus patterns by radiography. Its application to identification of unknown skeletal remains.

Frontal sinuses of Japanese skulls were radiographically examined from the view point of identifying individuals. The system of classification of sinus was proposed, basing on the area size, the bilateral asymmetry, the superiority of side, the outline of upper borders, the partial septa and the supraorbital cells. Frontal sinus patterns could be divided into above 20,000 possible combinations by combining the class numbers in each classification item described above. The frontal sinus pattern of a given person was formulated as a code number which was determined by arranging the class numbers

Arcanobacterium hemolyticum orbital cellulitis: a rare but aggressive disease.

The clinical presentation and course of orbital cellulitis in a young adult resulting from Arcanobacterium hemolyticum frontal sinusitis are presented in detail. This case illustrates the importance of a multidisciplinary approach for refractory and aggressive orbital cellulitis. A high level of suspicion for A. hemolyticum must be maintained in such cases, because it has proven to be a rare but aggressive, potentially occult, and life-threatening pathogen.

CT findings in involvement of the paranasal sinuses by lepromatous leprosy.

The role of nasal infection in the transmission of leprosy has been extensively studied. Leprosy can affect the paranasal sinuses due to mucosal continuity and bacillaemia. This prospective study was performed on 25 untreated patients with lepromatous leprosy. 5 mm contiguous axial and coronal CT sections of paranasal sinuses, on soft tissue and bone windows, were obtained in all patients. Each sinus was examined for mucosal thickening, soft tissue densities and bony outlines. Representative biopsies were taken from ethmoid sinus to confirm the radiological diagnosis in 12 patients with multip

Not the usual sinusitis.

An encephalocele is a protrusion of the cranial contents beyond the normal confines of the skull. It is a rare cause of seizure in adults. A 38-year-old woman presented with a first-onset seizure. Brain CT was interpreted as right frontal sinus opacification suggestive of sinusitis. The patient was discharged home with an amoxicillin prescription. A few days later, she was re-admitted with another seizure. Careful evaluation of the brain CT and MRI revealed a right frontal sinus posterior wall defect and possible brain encephalocele. The patient had complained of chronic nasal discharge for ye

The technique of endoscopic surgery and diagnosis of frontal recess and sinus disease under local anesthesia.

Operations are performed with the patient in a semi-reclining position under local anesthesia. Usually a rigid endoscope (70 degrees) is use … riorly, and it is necessary that this area be made as smooth as possible. Most cases of suppurative frontal sinusitis are curable just by opening nasofrontal-duct via endonasal approach using an endoscope.

Osteoma of the skull base and sinuses.

Osteomata of the frontal and ethmoid sinuses have traditionally been surgically removed via external approaches. However, endoscopic techniques have increasingly been used for the surgical management of selected cases. Advances in visualization and instrumentation, as well as the excellent access provided by the Draf type 3 procedure, expanded the reach of endoscopes. We describe current limits of endoscopic approaches in the removal of osteomata from the frontal sinus and our algorithms for their management. We believe that the vast majority of frontal sinus osteomata can be managed endoscopi

Computer-aided superimposition of the frontal sinus via 3D reconstruction for comparative forensic identification.

The anatomical uniqueness of the frontal sinus morphology has been widely used for comparative forensic identification using various techniques, mostly including 2D X-rays or one fixed slice of an axial computed tomography (CT) scan image. However, computer-aided 3D automatic graphical comparison techniques can provide accurate comparisons between two 3D models that allow users to comply with even the strictest deviation standards, avoiding error-prone identification of frontal sinuses with similar morphologies. The study proposes the use of a computer-aided comparative paradigm based on the 3

Minimally invasive treatment of sino-nasal aspergillosis in dogs.

BACKGROUND: Sino-nasal aspergillosis is a common nasal disease in dogs. Recommended treatment protocols typically involve trephination of the frontal sinuses or the use of an antifungal solution instilled into the frontal sinus under anesthesia, both of which have associated morbidity and complications. OBJECTIVES: To assess a minimally-invasive topical treatment protocol for sino-nasal aspergillosis in dogs. ANIMALS: Twelve client-owned dogs diagnosed with sino-nasal aspergillosis that completed recommended treatment. METHODS: Medical records were retrospectively reviewed to identify dogs

Pott's Puffy Tumor: A Potentially Deadly Complication of Sinusitis.

Pott's puffy tumor (PPT) is a rare but potentially deadly complication of bacterial sinusitis that consists of subperiosteal abscess and osteomyelitis of the frontal bone. It is best diagnosed with computerized tomography (CT) with intravenous contrast or magnetic resonance imaging and treated with early broad-spectrum antibiotics and surgical intervention. We describe a case of PPT in a 7-year-old patient and present the CT images.

Osteoporosis and cranial asymmetry in a mountain sheep (Ovis canadensis).

The skull of a mountain sheep (Ovis canadensis cremnobates) exhibiting osteoporosis was recovered from San Diego County, California. This specimen also exhibited lateral asymmetry of the cranium and rostrum, malocclusion of molars, and mandibular asymmetry. Investigators are cautioned about potentially confusing lesions associated with chronic frontal sinusitis with lesions of osteoporosis in mountain sheep. To our knowledge this is the first report of osteoporosis in this species.

Pott's puffy tumor.

Pott's Puffy tumor is a rare clinical entity characterized by subperiosteal abscess associated with osteomyelitis. It is usually seen as a complication of frontal sinusitis or trauma predominantly in the adolescent age group. Pott's Puffy tumor can be associated with cortical vein thrombosis, epidural abscess, subdural empyema, and brain abscess. The cause of vein thrombosis is explained by venous derange of the frontal sinus, which occurs through diplopic veins, which communicate with the dural venous plexus; septic thrombi can potentially evolve from foci within the frontal sinus and propaga…
