A pilot study of brain activation in children with trichotillomania during a visual-tactile symptom provocation task: a functional magnetic resonance imaging study.

OBJECTIVES: In order to reveal the etiology and pathophysiology of trichotillomania (TTM), it is necessary to investigate which brain regions are involved in TTM, but limited knowledge exists regarding the neurobiology of TTM and the available functional neuroimaging studies of TTM are little. The purpose of the present study was to investigate the specific brain regions involved in the pathophysiology of TTM with symptom provocation task using functional magnetic resonance imaging (fMRI) for children and adolescents with TTM. METHODS: Pediatric subjects who met the DSM-IV TR criteria for TTM…

The parcellation organization of cerebellum associated with white-matter functional systems.

The cerebellar role in various cognitive functions other than motor coordination has been gradually recognized, while its functional architecture and interaction with white matter functional networks (WM-FNs) remain unclear. The study combined resting-state functional connectivity and K-means clustering methods to obtain nine WM-FNs and seven gray matter functional networks (GM-FNs) by using the test-retest neuroimaging dataset collected from human connectome project. Subsequently, adopting winner-take-all algorithm and two distinct connectivity-based parcellations, we identified two parcellat

Tuberous sclerosis complex and neurofibromatosis type 1: the two most common neurocutaneous diseases.

TSC and NF1 are the most common of the neurocutaneous diseases, and both are autosomal dominant with a high spontaneous mutation rate. For diagnosis, two features are necessary for each disease. Skin findings for each are especially helpful for diagnosis, as is neuroimaging in TSC. For NF1, neuroimaging is not yet reliable for diagnosis. In children, brain symptoms cause most of the morbidity in TSC, and nerve sheath and nervous system tumors as well as learning disabilities cause major morbidity in NF1. Renal disease becomes a serious problem for adults with TSC. The TSC1, TSC2, and NF1 genes

Drawing a line: Differentiating mild from moderate dementia using the functional activities questionnaire.

BACKGROUND: Accurate differentiation between mild and moderate dementia is increasingly important, particularly as amyloid-targeting therapies are restricted to early disease stages. Functional impairment in instrumental activities of daily living is a hallmark of progression beyond mild dementia. The informant-based Functional Activities Questionnaire (FAQ) is widely used, but empirically validated cut-offs distinguishing mild from moderate dementia remain insufficiently defined. METHODS: The optimal cut-off score was derived from the entire National Alzheimer's Coordinating Center (NACC) Un

Association of Left Angular Gyrus-Right Middle Frontal Gyrus Functional Connectivity With Insomnia Severity in Major Depressive Disorder: Evidence From the DIRECT Consortium.

BACKGROUND: Identifying neuroimaging correlates of insomnia severity could provide insights into the underlying biological mechanisms of major depressive disorder (MDD). However, related findings remain inconsistent, and the functional connectivity patterns associated with insomnia severity are unclear. METHODS: This study analyzed resting-state fMRI data from 385 patients with MDD and 336 healthy controls (HCs) sourced from nine sites of the DIRECT Consortium. Patients were stratified into high insomnia (MDDHI; HAMD insomnia subscale ≥ 4, n = 226) and low insomnia (MDDLI; HAMD insomnia subsc

Detecting Awareness in the Intensive Care Unit Using Functional Near-Infrared Spectroscopy.

OBJECTIVE: Signs of awareness after acute severe brain injury are critical for informing goals-of-care decisions, ranging from the continuation of aggressive life-sustaining therapy to comfort-focused end-of-life measures. However, awareness may go undetected in some critically ill patients when assessed using bedside behavioral neurological examination. Here, we use functional near-infrared spectroscopy to identify covert awareness in acute brain injured patients who are unable to follow behavioral commands. METHODS: We conducted a prospective, consecutive cross-sectional study of 32 critica

Cingulate atrophy as a shared structural basis for cognitive and functional brain impairments in GAD, PD, and OCD: Links to shared gene expression and treatment implications.

BACKGROUND: Structural brain deficits associated with generalized anxiety disorder (GAD), panic disorder (PD), and obsessive-compulsive disorder (OCD) have been documented, but their integration within a unified framework remains unexplored. This study investigates, in anxiety and anxiety-related disorders, whether they share neurophysiological bases, whether structural changes (SCs) are linked to common genes, whether shared SCs co-occur with similar functional brain impairments, and whether brain morphometry can serve as biomarkers for diagnosis and treatment prediction. METHODS: Participan

Functional connectivity and structural changes of thalamic subregions in episodic migraine.

BACKGROUND: The thalamus plays a crucial role in transmitting nociceptive information to various cortical regions involving migraine-related allodynia and photophobia. Abnormal structural and functional alterations related to the thalamus have been well established. However, it is unknown whether the brain structure and function of the thalamic subregions are differentially affected in this disorder. In this study, we aimed to clarify this issue by comparing the structure and function of 16 thalamic subregions between patients with episodic migraine (EM) and healthy controls (HCs). METHODS: T

Anton's syndrome following callosal disconnection.

Anosognosia for cortical blindness, also called Anton's syndrome, is a rare neurological disorder usually following bilateral lesions to occipital cortices. Neuropsychological, morphological and functional neuroimaging (SPECT and fMRI) findings are reported in a patient who incurred Anton's syndrome after an ischaemic lesion confined to the left occipital lobe involving the corpus callosum. The present case study suggests that Anton's syndrome may also follow from lesions disconnecting the occipital cortices.

Amyotrophic lateral sclerosis and primary lateral sclerosis: evidence-based diagnostic evaluation of the upper motor neuron.

Magnetic resonance imaging and MR spectroscopy are important tools in the diagnostic evaluation of patients with suspected motor neuron disease. Further investigation is needed to determine and to compare the utility of various neuroimaging markers for diagnosis and disease progression [112]. Newer MR tools, such as diffusion tensor imaging, magnetization transfer imaging, and functional MR imaging, have substantial promise as scientific and clinical tools in this ongoing endeavor.

Regional cerebral blood flow and oxygen metabolism in a patient with Korsakoff syndrome.

We report a functional neuroimaging study of a patient clinically diagnosed with Korsakoff syndrome. Positron emission tomography (PET) with the 15O inhalation method showed decreased regional cerebral blood flow (rCBF) and decreased regional cerebral metabolic ratio for oxygen (rCMRO2) in the bilateral fronto-temporal areas and in the left thalamus. These results suggest that dysfunction of the frontal-thalamic neural network plays a role in the disturbance of Korsakoff syndrome.

Structural imaging in premanifest and manifest Huntington disease.

Huntington disease (HD) neuropathology has a devastating effect on brain structure and consequently brain function; neuroimaging provides a means to assess these effects in gene carriers. In this chapter we first outline the unique utility of structural imaging in understanding HD and discuss some of the acquisition and analysis techniques currently available. We review the existing literature to summarize what we know so far about structural brain changes across the spectrum of disease from premanifest through to manifest disease. We then consider how these neuroimaging findings relate to pat

Simultaneous monitoring of the human brain, spinal cord, and cauda equina activity for movement control: An fNIRS approach.

Brain‒spinal cord‒cauda equina interactions are essential for controlling lower body movement. However, current monitoring approaches for spinal and caudal activity are limited to use without body movement and to processing via batches of data. Here, we present a novel optical method based on functional near-infrared spectroscopy that enables simultaneous tracking of human brain-spinal cord-cauda equina hemodynamics during body movement. We first developed a support frame for positioning optical emitters and receivers along the spinal canal to maximize spatial resolution and identify the optim

A Connectomic Atlas of the Human Cerebrum-Chapter 7: The Lateral Parietal Lobe.

In this supplement, we build on work previously published under the Human Connectome Project. Specifically, we seek to show a comprehensive anatomic atlas of the human cerebrum demonstrating all 180 distinct regions comprising the cerebral cortex. The location, functional connectivity, and structural connectivity of these regions are outlined, and where possible a discussion is included of the functional significance of these areas. In part 7, we specifically address regions relevant to the lateral parietal lobe.

Mechanisms of Antidepressant Response to Electroconvulsive Therapy Studied With Perfusion Magnetic Resonance Imaging.

BACKGROUND: Converging evidence suggests that electroconvulsive therapy (ECT) induces neuroplasticity in patients with severe depression, though how this relates to antidepressant response is less clear. Arterial spin-labeled functional magnetic resonance imaging tracks absolute changes in cerebral blood flow (CBF) linked with brain function and offers a potentially powerful tool when observing neurofunctional plasticity with functional magnetic resonance imaging. METHODS: Using arterial spin-labeled functional magnetic resonance imaging, we measured global and regional CBF associated with cl

Brain activity in struggling readers before intervention relates to future reading gains.

Neural markers for reading-related changes in response to intervention could inform intervention plans by serving as a potential index of the malleability of the reading network in struggling readers. Of particular interest is the role of brain activation outside the reading network, especially in executive control networks important for reading comprehension. However, it is unclear whether any intervention-related executive control changes in the brain are specific to reading tasks or reflect more domain general changes. Brain changes associated with reading gains over time were compared for

Abnormal functional connectivity of thalamic sub-regions contributes to fatigue in multiple sclerosis.

OBJECTIVE: To investigate sub-regional thalamic resting-state (RS) functional connectivity (FC) abnormalities in multiple sclerosis (MS) and their correlation with fatigue and its subcomponents (physical, cognitive, and psychosocial). METHODS: From 122 MS patients and 94 healthy controls, 5 thalamic sub-regions (frontal, motor, postcentral, occipital, temporal) were parcellated based on their cortico-thalamic structural connectivity and used for a seed-based RS FC analysis. Abnormalities of thalamic RS FC in MS patients and their correlation with Modified Fatigue Impact Scale (MFIS) were asse
