Ziprasidone-related oculogyric crisis in an adult.

INTRODUCTION: Drug-induced dyskinesias are common side-effects of first-generation antipsychotics (FGAs) but are not usually related to second-generation antipsychotics (SGAs). Oculogyric crisis (OGC) is a disabling acute dystonia that affects extra-ocular muscles usually resulting in an upward deviation of the eyes, which lasts from minutes to hours. CASE REPORT: We describe an adult patient, previously exposed to an FGA, who developed OGC on 80mg/day of ziprasidone. The movement disorder significantly improved after use of 1mg/day of clonazepam without the need to switch to another SGA. DI

Behavioral determinants of drug action. The contributions of Peter B. Dews.

Peter B. Dews played a significant role in shaping the distinctive characteristics and defining the underlying principles of the discipline of behavioral pharmacology. His early and sophisticated use of schedules of reinforcement in the 1950s, incorporated from research in the experimental analysis of behavior and integrated into the discipline of pharmacology, provided tremendous insight, inspiration, and impetus to the newly emerging field of behavioral pharmacology. The experimental findings generated by Dews' research, blending the sophisticated use of behavior and pharmacological principl

Changes in anxiety sensitivity with pharmacotherapy for panic disorder.

Fear of anxiety symptoms (anxiety sensitivity) has been implicated in the etiology and maintenance of panic disorder, and has been shown to improve with cognitive-behavioral treatment. The impact of pharmacotherapy on anxiety sensitivity is less clear. We administered the Anxiety Sensitivity Index (ASI) during a 12-week randomized controlled trial investigating the relative efficacy of paroxetine, paroxetine plus sustained clonazepam, and paroxetine plus brief clonazepam for patients with panic disorder. We found a mean reduction in ASI scores of 9.6 points, which correlated with symptomatic i

Diagnostic significance of Wada procedure in very young children and children with developmental delay.

Localization of vital components of neurological functioning has to be performed before epilepsy surgery can be considered in children with intractable epilepsy. This study reports the experience with the Wada procedure in very young children and/or developmentally delayed children with an a priori considerable chance of failing the procedure. The aim of this study was to indicate the applicability of this procedure in this patient group. The Wada procedure is described in 16 children under 10 years of age and/or have intelligence quotient scores below 50 and/or are critically ill and/or are b

The stiff-person syndrome: an autoimmune disorder affecting neurotransmission of gamma-aminobutyric acid.

The stiff-person syndrome, a rare and disabling disorder, is characterized by muscle rigidity and episodic spasms that involve axial and limb musculature. Continuous contraction of agonist and antagonist muscles caused by involuntary motor-unit firing at rest are the hallmark clinical and electrophysiologic signs of the disease. Except for global muscle stiffness, results of neurologic examination are usually normal. Results of conventional computed tomography and magnetic resonance imaging of the brain are also normal. The cause of the stiff-person syndrome is unknown; however, an autoimmune

The effect of flumazenil on visual event-related potentials of clinically non-encephalopathic patients with cirrhosis.

The effects of flumazenil on the latencies and amplitudes of visual event-related potentials (ERPs), number-connection test (NCT) and visual and auditory reaction times (VRT and ART) were evaluated in ten patients with cirrhosis without clinically overt encephalopathy (HE). Delayed latencies of the ERP component P3a and/or P3b were found in three patients and the time to complete NCT was prolonged in two other patients. Changes in the latencies and amplitudes of the ERP components (N200, P3a and P3b) during 40 min following infusion of flumazenil (1 mg) and placebo were similar. Results of the

Studies of the hepatic mitochondrial and microsomal mixed-function oxidase system during Plasmodium yoelii infection and inducer treatment in Swiss albino mice.

Plasmodium yoelii infection resulted in depression of hepatic mitochondrial and microsomal mixed-function oxidase system indices, e.g. cytochrome P-450, cytochrome b5 and phase II detoxification enzyme glutathione-S-transferase, while heam and haemozoin registered a marked increase in Swiss albino mice. Phenobarbitone (inducer) treatment showed induced levels of hepatic mitochondrial and microsomal cytochrome P-450 and glutathione-S-transferase in normal as well as in infected mice. The induced cytochrome P-450 and glutathione-S-transferase activities were similar in normal and infected mice.

A risk-benefit assessment of flumazenil in the management of benzodiazepine overdose.

The worldwide expansion in the use of benzodiazepines has led to their frequent, and often inappropriate, use and to increase in their involvement in self-induced poisoning and iatrogenic overdosing. Flumazenil is a specific and competitive antagonist at the central benzodiazepine receptor, reversing all effects of benzodiazepine agonists without tranquillising or anticonvulsant actions. Incremental intravenous bolus injections of flumazenil 0.1 to 0.3 mg are the most effective and well tolerated in the diagnosis and treatment of pure benzodiazepine overdose; additional boluses or an infusion

Effects of Exogenous SA/GABA Combined with ZnSO4 Treatment on the Physiological Metabolism and Flavonoid Biosynthesis in Finger Millet (Eleusine coracana L.) Sprouts.

Finger millet (Eleusine coracana L.) is rich in bioactive compounds, including flavonoids. Following exogenous substance regulation, its sprouts can achieve efficient flavonoid enrichment. This study investigates the regulatory effects of exogenous salicylic acid (SA) and γ-aminobutyric acid (GABA) on the physiological metabolism, oxidative stress response, and flavonoid biosynthesis of finger millet sprouts subjected to 5 mM zinc sulfate (ZnSO4) stress. Compared to treatment solely with ZnSO4, the application of both 50 μM salicylic acid (SA) and 1 mM gamma-aminobutyric acid (GABA) markedly e

The influence of Yishen Ningxin formula on the behavioral and neurotransmitter changes in a zebrafish model of generalized anxiety.

This study evaluated the anxiolytic effects of Yishen Ningxin formula (YSNX) in caffeine-induced zebrafish models and explored underlying mechanisms. Using 7-day post-fertilization AB zebrafish, anxiety-like models were established with 100μmol/L caffeine exposure. Groups included normal control, caffeine-only, and YSNX-treated cohorts (200/550/1600μg/mL). Behavioral assessments included thigmotaxis, vertical preference, and locomotor activity using automated tracking, with GABA/5-HT levels measured by ELISA. Compared with normal controls, caffeine-exposed zebrafish showed increased total move

Alleviating effect of vagus nerve cutting in Salmonella-induced gut infections and anxiety-like behavior via enhancing microbiota-derived GABA.

The vagus nerve, a pivotal link within the gut-brain axis, plays a critical role in maintaining homeostasis and mediating communication between the gastrointestinal tract and the brain. It has been reported that gastrointestinal infection by Salmonella typhimurium (S. typhimurium) triggers gut inflammation and manifests as anxiety-like behaviors, yet the mechanistic involvement of the vagus nerve remains to be elucidated. In this study, we demonstrated that unilateral cervical vagotomy markedly attenuated anxiety-like behaviors induced by S. typhimurium SL1344 infection in C57BL/6 mice, as evi

Ketoprofen, lysine and gabapentin co-crystal magnifies synergistic efficacy and tolerability of the constituent drugs: Pre-clinical evidences towards an innovative therapeutic approach for neuroinflammatory pain.

Chronic pain is an enormous public health concern, and its treatment is still an unmet medical need. Starting from data highlighting the promising effects of some nonsteroidal anti-inflammatory drugs in combination with gabapentin in pain treatment, we sought to combine ketoprofen lysine salt (KLS) and gabapentin to obtain an effective multimodal therapeutic approach for chronic pain. Using relevant in vitro models, we first demonstrated that KLS and gabapentin have supra-additive effects in modulating key pathways in neuropathic pain and gastric mucosal damage. To leverage these supra-additiv

Presynaptic CaMKIIα modulates dopamine D3 receptor activation in striatonigral terminals of the rat brain in a Ca²⁺ dependent manner.

CaMKIIα is expressed at high density in the nucleus accumbens where it binds to postsynaptic D3 receptors inhibiting their effects. In striatonigral projections, activation of presynaptic D3 receptors potentiates D1 receptor-induced stimulation of cAMP production and GABA release. In this study we examined whether the presynaptic effects of D3 receptor stimulation in the substantia nigra reticulata (SNr) are modulated by Ca²⁺ activation of CaMKIIα. In SNr synaptosomes two procedures that increase cytoplasmic Ca²⁺, ionomycin and K⁺-depolarization, blocked the additional stimulation of cAMP accu

Role of nicotinic receptors and acetylcholine in mucous cell metaplasia, hyperplasia, and airway mucus formation in vitro and in vivo.

BACKGROUND: Airway mucus hypersecretion is a key pathophysiologic feature in a number of lung diseases. Cigarette smoke/nicotine and allergens are strong stimulators of airway mucus; however, the mechanism of mucus modulation is unclear. OBJECTIVES: We sought to characterize the pathway by which cigarette smoke/nicotine regulates airway mucus and identify agents that decrease airway mucus. METHODS: IL-13 and γ-aminobutyric acid type A receptors (GABA(A)Rs) are implicated in airway mucus. We examined the role of IL-13 and GABA(A)Rs in nicotine-induced mucus formation in normal human bronchial

Cortical GABA, striatal dopamine and midbrain serotonin as the key players in compulsive and anxiety disorders--results from in vivo imaging studies.

Various factors are discussed in the pathophysiology of anxiety disorders, including dysfunctions of the (DA)ergic, serotonin (5-HT)ergic and GABAergic system. We assessed the contribution of the individual synaptic constituents by subjecting all available in vivo imaging studies on patients with anxiety disorders to a retrospective analysis. On a total of 504 patients with obsessive-compulsive disorder (OCD), generalized anxiety disorder (GAD), panic disorder (PD), phobia, or posttraumatic stress-disorder (PTSD) and 593 controls, investigations of VMAT2, DAT, SERT, D1, D2, 5-HTIA, 5-HT2A, GAB

Reduced Ventral Tegmental Area GABA neuron output contributes to hyperactivity in the activity-based anorexia model in female mice.

Anorexia nervosa (AN) has the highest mortality among psychiatric disorders, with hyperactivity being one of the most persistent and deleterious symptoms. Increased dopamine transmission is linked to AN and hyperactivity, though the underlying mechanisms remain unclear. Local GABAergic neurons powerfully regulate the dopamine system but their involvement in AN is unknown. Using the activity-based anorexia (ABA) mouse model, we found that GABAergic transmission onto ventral tegmental area dopamine (VTADA) neurons modulates hyperactivity in female mice. Indeed, female mice exposed to the ABA mod

Dissecting the molecular basis of human interneuron migration in forebrain assembloids from Timothy syndrome.

Defects in interneuron migration can disrupt the assembly of cortical circuits and lead to neuropsychiatric disease. Using forebrain assembloids derived by integration of cortical and ventral forebrain organoids, we have previously discovered a cortical interneuron migration defect in Timothy syndrome (TS), a severe neurodevelopmental disease caused by a mutation in the L-type calcium channel (LTCC) Cav1.2. Here, we find that acute pharmacological modulation of Cav1.2 can regulate the saltation length, but not the frequency, of interneuron migration in TS. Interestingly, the defect in saltatio

Glia: A major player in glutamate-GABA dysregulation-mediated neurodegeneration.

The imbalance between glutamate and γ-aminobutyric acid (GABA) results in the loss of synaptic strength leading to neurodegeneration. The dogma on the field considered neurons as the main players in this excitation-inhibition (E/I) balance. However, current strategies focusing only on neurons have failed to completely understand this condition, bringing up the importance of glia as an alternative modulator for neuroinflammation as glia alter the activity of neurons and is a source of both neurotrophic and neurotoxic factors. This review's primary goal is to illustrate the role of glia over E/I
