pH-dependent pressure-sensitive colonic capsules for the delivery of aqueous bacterial suspensions.

Microbiome-based therapies hold great promise for treating various diseases, but the efficient delivery of live bacteria to the colon remains a challenge. Furthermore, current oral formulations, such as lyophilized bacterial capsules or tablets, are produced using processes that can decrease bacterial viability. Consequently, high dosages are required to achieve efficacy. Herein, we report the design of pressure-sensitive colonic capsules for the encapsulation and delivery of aqueous suspensions of live bacteria. The capsules consisted of 2 functional thin-films (hydrophobic and enteric) of et

Management of bloating.

Abdominal bloating is a subjective sensation of trapped abdominal gas, producing abdominal pressure, fullness sensation, and, in some patients, associated objective abdominal distension. In this month's edition of the journal, a new validated questionnaire to assess the prevalence and impact of gas-related symptoms is presented by Duracinsky et al., showing that gas-related abdominal symptoms are prevalent in patients with irritable bowel syndrome and have a measurable impact on patients daily life. A parallel study by Gardiner et al. assessing the severity of bloating in functional gastrointe

Electroacupuncture ameliorates corticotrophin-releasing factor-induced jejunal dysmotility in a rat model of stress.

BACKGROUND: Central injection of corticotrophin-releasing factor (CRF) mimics the effect of stress on gastrointestinal (GI) responses, including inhibition of GI motility. This study was designed to explore the effects of electroacupuncture (EA) on disordered jejunal motility in a rat model of stress induced by intracisternal (IC) injection of CRF. METHODS: A stress model was established by IC injection of CRF in Sprague-Dawley rats. GI motility was evaluated by assessing gastric emptying (GE), gastrointestinal transit (GIT) and jejunal motility in vivo. EA was performed at ST36. The function…

[Disaccharide intolerance].

Disaccharide intolerance presents a pathogenic heterogeneous and most complex clinical entity. It usually occurs due to primary or secondary deficit of disaccharide activity, and rarely because of disorders of absorption or monomer metabolism. Symptomatology of disaccharide maldigestion and/or malabsorption depends on the severity of the basic disorder, the level of its overload and the patient's age. In the youngest children, due to a rapid gastrointestinal transit and a low compensatory capacity of the colon, osmotic-fermentative diarrhoea forms the basis of clinical features. Diarrhoeal dis…

Capsule endoscopy: a surgical perspective.

BACKGROUND: Capsule endoscopy (CE) is gaining acceptance as an accurate method of imaging the small bowel. However, it is still being assessed for its use in successfully changing management and improving outcomes. We report the initial experience of CE in Tasmania. METHODS: Findings were collected retrospectively for the first 55 consecutive CE carried out in Tasmania from May 2003 to June 2005. One surgeon had carried out and reported on these tests. Findings included indication, previous tests, abnormalities detected, subsequent change in management and outcomes. RESULTS: The median age w

Proteomic profiling identifies prognostic signature for Krukenberg tumor of gastrointestinal origin.

Krukenberg tumor (KT) primarily originates from the stomach and colorectum, but reliable biomarkers for distinguishing KT from other tumors at the same sites and predicting ovarian metastasis remain lacking. Using pressure cycling technology (PCT) and data-independent acquisition (DIA) mass spectrometry, we analyzed 263 formalin-fixed paraffin-embedded (FFPE) samples, identifying 10,837 proteins. The results revealed distinct proteomic signatures from the primary gastrointestinal lesions of KT. Comparative analyses identified group-specific pathways, particularly mesenchymal-epithelial transit

Transitions of care for adolescents with disorders of gut-brain interaction.

OBJECTIVES: Little is known about the experience of adolescents and young adults (AYA) with disorders of gut-brain interaction (DGBI) who transition from pediatric to adult gastroenterology care. In this two-part study, we used quantitative and qualitative methods to: (1) assess incidence of optimal versus suboptimal transitions of care for AYA with DGBI, (2) characterize health and quality of life effects of the transition, and (3) identify barriers and facilitators for optimal transition of care. METHODS: In Part 1, we conducted a retrospective review of AYA referrals to our adult neurogast

CXCL14+ fibroblasts promote development of IBD-associated fibrosis and cancer by interacting with SPP1+ macrophages via collagen IV secretion.

BACKGROUND: The pathogenesis of inflammatory bowel disease (IBD) involves chronic inflammation of the gastrointestinal tract, which predisposes patients to an increased risk of fibrosis and cancer due to prolonged inflammatory stimulation. However, the specific mechanisms underlying secondary gastrointestinal carcinogenesis in individuals with IBD remain poorly elucidated, particularly due to a limited number of studies conducted at single-nuclei level. METHODS: We investigated the dynamics and interactions of cell subtypes during the progression of IBD-associated cancers through comprehensiv

Facilitating discontinuation of intravenous opioids by -concurrent use of sublingual buprenorphine with rapid microdosing -induction: A pain management case study.

We report a case in which sublingual buprenorphine was used to help transition a patient off intravenous (IV) opioid analgesics medications post-multiple abdominal procedures. Intravenous opioids are commonly used in inpatient surgical pain management for patients with severe pain who are unable to take oral medications. Typically, a short course of IV analgesics is used, followed by transition to oral analgesic regimen. However, in patients with poor gastrointestinal absorption, pain control can be challenging. We present this case to highlight how sublingual buprenorphine can be a useful age

Differential sensitivity of gastric and small intestinal muscles to inducible knockdown of anoctamin 1 and the effects on gastrointestinal motility.

KEY POINTS: Electrical pacemaking in gastrointestinal muscles is generated by specialized interstitial cells of Cajal that produce the patterns of contractions required for peristalsis and segmentation in the gut. The calcium-activated chloride conductance anoctamin-1 (Ano1) has been shown to be responsible for the generation of pacemaker activity in GI muscles, but this conclusion is established from studies of juvenile animals in which effects of reduced Ano1 on gastric emptying and motor patterns could not be evaluated. Knocking down Ano1 expression using Cre/LoxP technology caused dramatic

Patients' Transitions and Recovery From Hospital to Everyday Life After Admission for Acute Pancreatitis: A Qualitative Study.

The incidence and readmission rates after acute pancreatitis have increased over the last 20 years. During the time of illness and recovery, patients can experience physical and mental suffering. Despite many studies investigating acute pancreatitis, there is a gap regarding patients' experiences after discharge. This study aimed to explore patients' experiences of recovery from acute pancreatitis during the transition from hospital to home. A qualitative approach was adopted in a Danish surgical gastrointestinal department. Data consisted of 9 semi-structured interviews, which were analyzed w

TRAUMATIC BRAIN INJURY-INDUCED INFLAMMATION AND GASTROINTESTINAL MOTILITY DYSFUNCTION.

Background: Traumatic brain injury (TBI) is a significant cause of morbidity and mortality in the United States, with an annual cost of 60 billion dollars. There is evidence suggesting that in the post-TBI period, the gastrointestinal tract plays a central role in driving organ and immune dysfunction and may be the source of increased circulating proinflammatory mediators. In this study, we examined systemic inflammation and bacterial dysbiosis in patients who sustained a TBI with or without polytrauma. Using a mouse model of TBI, we further show how neuroinflammation after TBI is potentially

Kinetic fingerprinting and food matrix interactions reveal the gastrointestinal behavior of polyphenolics in stir-fried green tea.

Stir-fried green tea polyphenols exhibit strong antioxidant capacity; however, they degrade rapidly during gastrointestinal transit. This study combined antioxidant kinetic profiling, chemometric analysis, simulated digestion, and Caco-2 cell permeability models to identify the principal antioxidant drivers and evaluate the protective effects of the food matrix. Ridge model identified EGCG as the primary determinant of radical-scavenging kinetics, whereas gallic acid contributed more strongly to the thermodynamic antioxidant capacity. Simulated intestinal digestion induced an antioxidant kinet…

Optimizing drug nanoparticles release from amorphous solid dispersions: exceptional performance of poly(acrylic acid) for releasing lumefantrine.

Amorphous solid dispersions (ASDs) have been observed to release their active ingredients in an aqueous medium as drug-rich nanoparticles, leading to high apparent solubility and potential benefits for drug absorption. We have prepared the ASDs of lumefantrine with 10 polymers and tested their release of drug-rich nanoparticles in 0.1 % sodium dodecyl sulfate. Poly(acrylic acid) (PAA) significantly outperforms the other polymers in reaching the highest apparent solubility through a 0.2 μm filter, and its outperformance persists as drug loading increases from 25 % to 50 %, without sudden declin
