Learning systems in biosignal analysis.

In biosignal analysis, the utility of artificial neural networks (ANN) in classifying electromyographic (EMG) data trained with the momentum back propagation algorithm has recently been demonstrated. In the current study, the self-organizing feature map algorithm, the genetics-based machine learning (GBML) paradigm, and the K-means nearest neighbour clustering algorithm are applied on the same set of data. The aim of this exercise is to show how these three paradigms can be used in practice, given that their diagnostic performance is problem- and parameter-dependent. A total of 720 macro EMG r

Beyond hypoxic-ischemic encephalopathy: genetic insights and precision diagnosis in neonatal encephalopathies.

Neonatal encephalopathies are a heterogeneous group of early-onset neurological disorders. While hypoxic-ischemic encephalopathy (HIE) has long been considered the predominant cause, a growing number of genetic, metabolic, immune-mediated, and toxic etiologies may clinically mimic HIE, posing diagnostic challenges. Timely recognition of these alternative causes is essential to initiate targeted therapies and provide appropriate family counseling. This article reviews the main clinical presentations and pathogenetic mechanisms of neonatal encephalopathies and introduces a structured diagnostic

Approach to the patient: investigation of postmenopausal androgen excess.

Androgen excess in postmenopausal women presents a challenging clinical conundrum. A crucial component in management involves identification of the underlying etiology with a focus on detection of potentially malignant pathology or underlying genetic syndromes. A basic understanding of androgen physiology in women and the associated alterations during the menopausal transition is required to accurately risk stratify this cohort and streamline investigations. Over the course of this article, we will propose an approach to investigation and management of postmenopausal androgen excess that focus

[Osteochondritis dissecans of the knee revisited. Recent advances and practical treatment algorithm].

Osteochondritis dissecans (OCD) frequently affects the knee joint, causing instability or separation of an osteochondral fragment, which often leads to pain, mechanical symptoms, and, if left untreated, early-onset osteoarthritis. The pathogenesis of OCD is multifactorial, involving a number of anatomical, mechanical, vascular, genetic, and biological factors. Imaging plays a critical role in the diagnosis, decision-making, and follow-up of OCD lesions. Management can be surgical or non-surgical, depending on patient age and lesion characteristics. This article aims to synthesize the recent li

Optimal allocation and operation of sewer monitoring sites for wastewater-based disease surveillance: A methodological proposal.

Wastewater-based epidemiology (WBE) is drawing increasing attention as a promising tool for an early warning of emerging infectious diseases such as COVID-19. This study demonstrated the utility of a spatial bisection method (SBM) and a global optimization algorithm (i.e., genetic algorithm, GA), to support better designing and operating a WBE program for disease surveillance and source identification. The performances of SBM and GA were compared in determining the optimal locations of sewer monitoring manholes to minimize the difference among the effective spatial monitoring scales of the sel

Ecological niche modeling of potential West Nile virus vector mosquito species in Iowa.

Ecological niche modeling (ENM) algorithms, Maximum Entropy Species Distribution Modeling (Maxent) and Genetic Algorithm for Rule-set Prediction (GARP), were used to develop models in Iowa for three species of mosquito - two significant, extant West Nile virus (WNV) vectors (Culex pipiens L and Culex tarsalis Coquillett (Diptera: Culicidae)), and the nuisance mosquito, Aedes vexans Meigen (Diptera: Culicidae), a potential WNV bridge vector. Occurrence data for the three mosquito species from a state-wide arbovirus surveillance program were used in combination with climatic and landscape layers

Accurate haplotype inference for multiple linked single-nucleotide polymorphisms using sibship data.

Sibships are commonly used in genetic dissection of complex diseases, particularly for late-onset diseases. Haplotype-based association studies have been advocated as powerful tools for fine mapping and positional cloning of complex disease genes. Existing methods for haplotype inference using data from relatives were originally developed for pedigree data. In this study, we proposed a new statistical method for haplotype inference for multiple tightly linked single-nucleotide polymorphisms (SNPs), which is tailored for extensively accumulated sibship data. This new method was implemented via

Paroxysmal Movement Disorders.

Paroxysmal movement disorders (PxMDs) are a clinical and genetically heterogeneous group of movement disorders characterized by episodic involuntary movements (dystonia, dyskinesia, chorea and/or ataxia). Historically, PxMDs were classified clinically (triggers and characteristics of the movements) and this directed single-gene testing. With the advent of next-generation sequencing (NGS), how we classify and investigate PxMDs has been transformed. Next-generation sequencing has enabled new gene discovery (RHOBTB2, TBC1D24), expansion of phenotypes in known PxMDs genes and a better understandin

The VUS Challenge in Cystic Kidney Disease: A Case-Based Review.

Genetic testing in nephrology is becoming increasingly important to diagnose patients and to provide appropriate care. This is especially true for autosomal dominant polycystic kidney disease (ADPKD) because this is a common cause of kidney failure and genetically complex. In addition to the major genes, PKD1 and PKD2 , there are at least six minor loci, and phenotypic, and in some cases, genetic overlap with other cystic disorders. Targeted next-generation sequencing, a low-cost, high-throughput technique, has made routine genetic testing viable in nephrology clinics. Appropriate pre- and pos

Automated Embryo Sorting in Zebrafish Facilities: Performance Benchmarking Against Manual Processing.

Zebrafish (Danio rerio) are widely used in biomedical research due to their genetic similarity to humans and rapid development. Efficient management of large embryo populations is essential for experimental reproducibility and high-throughput screening, yet manual counting and sorting methods are labor-intensive, time-consuming, and pose ergonomic risks including musculoskeletal disorders. We evaluated a commercially available AI-powered automated embryo sorting system against manual methods across 117,956 embryos to assess efficiency, accuracy, and reliability in operational facility conditio

Newborn screening follow-up for very long-chain acyl-CoA dehydrogenase deficiency in Colorado: Working towards a standardized protocol.

Very long chain acyl-CoA dehydrogenase deficiency (VLCADD) is an autosomal recessive fatty acid β-oxidation disorder that has been identified by newborn screening (NBS) in most states since the early 2000s. Despite over 20 years of experience, there are aspects of VLCADD NBS that remain challenging. We conducted a retrospective chart review of abnormal NBS for VLCADD in Colorado between 2017 and 2023. We analyzed confirmatory plasma acylcarnitine profiles (P-ACP), genetic sequencing of ACADVL, Collaborative Laboratory Integrated Reports (CLIR) scores, patient enzyme analysis of VLCAD, and cell

Hemochromatosis: Ferroptosis, ROS, Gut Microbiome, and Clinical Challenges with Alcohol as Confounding Variable.

Hemochromatosis represents clinically one of the most important genetic storage diseases of the liver caused by iron overload, which is to be differentiated from hepatic iron overload due to excessive iron release from erythrocytes in patients with genetic hemolytic disorders. This disorder is under recent mechanistic discussion regarding ferroptosis, reactive oxygen species (ROS), the gut microbiome, and alcohol abuse as a risk factor, which are all topics of this review article. Triggered by released intracellular free iron from ferritin via the autophagic process of ferritinophagy, ferropto

A comprehensive review of scapulothoracic abnormal motion (STAM): evaluation, classification, and treatment strategies.

The term "dyskinesia" has often been used interchangeably with "winging," leading to ambiguity in the literature. To address this, the broader term "scapulothoracic abnormal motion (STAM)" was introduced to describe any abnormal position or movement of the scapula on the chest, resulting in pain and dysfunction. Scapulothoracic abnormal motion has a wide range of causes, including musculoskeletal imbalances such as pectoralis minor hyperactivity, neurological impairments such as long thoracic nerve palsy, and genetic conditions like facioscapulohumeral muscular dystrophy (FSHD). This review ai

New QSPR model for prediction of corrosion inhibition using conceptual density functional theory.

The relationship between structure and corrosion inhibition of a series of twenty-eight quinoline and pyridine derivatives has been established through the investigation of quantum descriptors calculated with PBE/6-311 +  + G** method. A quantitative structure-property relationship (QSPR) model was obtained by examining these descriptors using a genetic algorithm approximation method based on a multiple linear regression analysis. The results indicate that the efficiency of corrosion inhibitors is strongly associated with hardness (η), minimal electrostatic potential (ESPmin), and volume (V) d

Congenital myopathies and muscular dystrophies.

The congenital muscular dystrophies (CMD) and myopathies (CM) are a diverse group of diseases that share features such as early onset of symptoms (in the first year of life), genetic causes, and high risks for restrictive lung disease and orthopedic deformities. Understanding for disease mechanism is available and a fairly well-structured genotype-phenotype correlation for all the CMDs and CMs is now available. To best illustrate the clinical spectrum and diagnostic algorithm for these diseases, this article presents 5 cases, including Ullrich congenital muscular dystrophy, nemaline myopathy,

A machine learning framework for classifying dementia risk in mild cognitive impairment: evidence from a Korean genome-wide association study cohort.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder that presents challenges for early detection and intervention. Mild cognitive impairment (MCI), a critical precursor of AD, progresses to dementia in a substantial proportion of individuals annually. Genetic factors, particularly single nucleotide polymorphisms (SNPs), play a key role in the pathogenesis of AD, as identified by genome-wide association studies (GWAS). Therefore, we aimed to develop and evaluate predictive models for classifying patients with MCI into high- and low-risk groups for dementia using SNP
