An analysis of the etiological and predisposing factors related to dentin hypersensitivity.

AIM: To determine the prevalence of different etiological factors of dentin hypersensitivity in patients and to provide information on their association with dentin hypersensitivity. METHODS AND MATERIALS: Twenty-nine patients (17 male, 12 female) suffering from pain of dentin hypersensitivity were recruited to participate in the study. A relevant history was taken and dentin hypersensitivity confirmed by using air-blast and tactile stimuli. RESULTS: All patients were right-handed. The left side of the mouth showed a preponderance of gingival recession, abrasion, abfraction, and erosion whil…

Free gingival grafts to increase keratinized tissue: a retrospective long-term evaluation (10 to 25 years) of outcomes.

BACKGROUND: Gingival augmentation procedures are indicated primarily to increase an insufficient amount of gingiva and sometimes to halt the progression of gingival recession. The aim of this retrospective long-term study was to evaluate changes in the amount of keratinized tissue (KT) and in the position of the gingival margin after free gingival graft procedures over a period of 10 to 25 years. METHODS: One hundred three subjects presenting with 224 sites completely lacking attached gingiva associated with gingival recessions were treated in a private practice. The experimental sites were t…

Innovative i-PRF semisurgical method for gingival augmentation and root coverage in thin periodontal phenotypes: a preliminary study.

OBJECTIVES: The aim of the study was to evaluate the effect of injectable platelet-rich fibrin (i-PRF) on gingival thickness and gingival recession in individuals with thin periodontal phenotypes. METHOD AND MATERIALS: In this prospective study, i-PRF was applied via a semisurgical method to augment 53 tooth regions with thin periodontal phenotypes. In order to ensure that sufficient blood clot formed on the side of the gingiva facing the bone and that i-PRF reached the area, a minimal incision was made with the help of a scalpel in the apical region of the relevant region, and the periosteum…

Sub-epithelial connective tissue graft for root coverage in nonsmokers and smokers: A pilot comparative clinical study.

BACKGROUND: Gingival recession is a common condition and is more prevalent in smokers. It is widely believed that root coverage procedures in smokers result in less desirable outcome compared to nonsmokers', and there are few controlled studies in literature to support this finding. Therefore, the purpose of this study was to evaluate and compare the outcome of root coverage with sub-epithelial connective tissue graft (SCTG) in nonsmokers and smokers. MATERIALS AND METHODS: A sample of twenty subjects, 10 nonsmokers and 10 smokers were selected each with at least 1 Miller's Class I or II rece…

The gingival biotype in a cohort of Chinese subjects with and without history of periodontal disease.

OBJECTIVE: The present study aimed to determine the gingival biotype in Chinese subjects with and without a history of periodontal disease. MATERIAL AND METHODS: Thirty periodontally healthy subjects and 20 subjects with treated chronic periodontitis were recruited. The mid-buccal gingival thickness of upper central and lateral incisors was measured by a customized caliper in all subjects. The crown length and crown width of these teeth were recorded in the healthy group, while gingival recession was measured in the periodontitis group. These outcome measures were compared among the groups an…

Effects of smokeless tobacco on the periodontal, mucosal and caries status of adolescent males.

The use of smokeless tobacco (ST) products, such as snuff and chewing tobacco, was investigated in a school population of 565 males with a mean age of 13.8 years. All students completed a tobacco usage questionnaire and were given intraoral examinations by a team of dentists to determine the possible relationship of ST usage to the presence of gingivitis, gingival recession, mucosal pathology and caries. The overall prevalence of ST usage was 13.3%, which was much higher than the 1.4% prevalence of cigarette smoking. Pairwise chi square analysis revealed that there was no relationship between …

A new technique for root coverage using buccal fat pad--a short case report.

Gingival recession presents a multifactorial etiology that may be associated with anatomical, physiologic, or pathologic factors. Several surgical techniques are described to manage the gingival recession. The buccal fat pad is a specialized organized fat tissue that maintains its volume and structure over a long period of time. It can also serve as a well-vascularized, readily obtainable local flap for reconstructive purposes as root coverage for severe recession defects, increasing the dimensions of keratinized mucosa. The aim of this case report is to establish a new technique for root cove…

Treatment of Multiple Adjacent Class I and Class II Gingival Recessions by Modified Microsurgical Tunnel Technique and Modified Coronally Advanced Flap Using Connective Tissue Graft: A Randomized Mono-center Clinical Trial.

BACKGROUND: Complete and uneventful recession coverage should be the aim of gingival recession treatment. Systematic reviews have said that coronally advanced flap with connective tissue graft (CTG) is the gold standard for gingival recession treatment. Minimally invasive procedures with optical magnification allow minimal tissue manipulation and precise adaptation of wound edges helping in faster and uneventful healing, thus bringing about a satisfactory clinical and patient outcome. Thus, the following study compares the clinical- and patient-related outcomes of modified microsurgical tunnel…

Surgical coverage of exposed root surfaces: a review of available techniques and applications.

Exposed root surfaces often create esthetic problems with less-than- satisfactory restorative solutions. Gingival recession may result in thermal and tactile sensitivity that is unresponsive to treatment with desensitizing agents. Surgical procedures for the coverage of exposed root surfaces have provided successful and highly predictable results even in deep, wide areas of recession. This article reviews the available procedures and discusses the indications for periodontal plastic surgery. Clinical cases are presented that demonstrate solutions to commonly encountered clinical problems that …

Increased gingival dimensions. A significant factor for successful outcome of root coverage procedures? A 2-year prospective clinical study.

The aim of this study was to evaluate whether an increased thickness of the gingiva through the use of a free connective tissue graft, in conjunction with a coronally advanced flap procedure, may positively influence the treatment outcome with respect to (i) root coverage and (ii) long-term stability of the position of the soft tissue margin following treatment of recession type defects. 67 consecutive patients having a total of 103 buccally located recession type defects of at least 3 mm were included in the study. After an initial phase of prophylaxis including instructions in a tooth brushi

Significance of the width of keratinized mucosa on the health status of the supporting tissue around implants supporting overdentures.

Our objective was to determine the association between keratinized mucosa width and the health status of the supporting tissue around implants supporting overdentures. Sixty-six functioning dental implants were examined. Periodontal parameters measured included gingival index, plaque index, bleeding on probing, probing depth, mucosal recession, periodontal attachment level, radiographic bone level, and width of keratinized mucosa. A negative correlation was found between keratinized mucosa width and mucosal recession and periodontal attachment level. When data were dichotomized by keratinized

Gingiva thickness in guided tissue regeneration and associated recession at facial furcation defects.

Consistently successful regenerative therapy for furcation defects using membrane techniques remains a challenge for clinicians. The purpose of this study was to determine if the thickness of tissue used to cover the membrane influences postsurgery recession. Thirty-seven (37) moderate to advanced adult periodontitis patients presenting with at least one mandibular or maxillary molar class 1 or 2 facial furcation involvement participated in the study. Mid-facial presurgery recession was recorded from the cemento-enamel junction to the free gingival margin at a reproducible point. Mid-facial ti

A novel single-stage procedure for increasing the width of attached gingiva and eliminating the aberrant frenal attachment.

Common treatment for buccal gingival recession caused by an aberrant frenal attachment includes elimination of the frenum and treatment of the gingival recession by soft tissue graft to increase the width of the attached gingiva that in turn results in root coverage. Keratinised gingival, if present in adequate amount, maintains the gingival health by protecting the marginal gingiva. This not only considers the desires of the patient but also explores the potential regenerative capacity of the tissues. This report describes a novel single-stage procedure for increasing the width of the attache…
