Differences in dental and bone maturation in regions with or without hemihypertrophy in two patients with Russell-Silver syndrome.

Hemihypertrophy (hemihyperplasia) is defined as asymmetric body overgrowth of one or more body parts. Hemihypertrophy can be isolated or be part of well-defined syndromes, such as Russell-Silver syndrome (RSS). RSS is characterized by severe intrauterine growth retardation, postnatal short stature, asymmetry of the face, body, and limbs, short and incurved fifth fingers, broad forehead, pointed small triangular shaped face, girdle nose, normal psychomotor development, variations in the pattern of sexual development, and dental and oral abnormalities. The aim of this study was to determine whet

GH and zinc treatments for short stature caused by steroid treatments in a young patient with autoimmune hepatitis (type 1).

An 8-yr-old boy with autoimmune chronic active hepatitis (AiCAH) was treated with steroid hormones. Liver function improved with this treatment, but the patient's growth was severely disturbed. At the age of 10 y, GH treatment was added to the steroid therapy because of his short stature (height: -2.5 SD), but in the following 12 months, he only grew 2 cm. Oral administration of zinc (5 mg/kg/day) was also added to the patient's therapy, but his height has not improved in the 12 months of this combined therapy.

Pseudopapilledema and congenital disc anomalies in growth hormone deficiency.

Optic nerve hypoplasia is a congenital disc anomaly associated with growth hormone deficiency (GHD). Pseudotumor cerebri is an adverse event associated with growth hormone treatment (hGH) and manifested by increased intracranial pressure and papilledema. Pseudopapilledema is a generic ophthalmologic term encompassing several conditions, including congenital disc anomalies. It is benign and can be distinguished from papilledema by physical examination. The objective of this report is to document that congenital disc abnormalities, which can be confused with papilledema, occur in children with G

The effect of growth hormone replacement therapy for up to 12 months on lipoprotein composition and lipoprotein(a) in growth hormone-deficient adults.

The effect of growth hormone replacement therapy in near physiological doses on lipoprotein composition and serum lipoprotein(a) concentrations was investigated in growth hormone-deficient subjects. A randomised, double-blind, placebo-controlled trial of recombinant growth hormone was undertaken for 6 months followed by an open extension for a further 6 months (0.125 IU/kg per week for the first 4 weeks of each 6 month period and thereafter 0.25 IU/kg per week). A total of 18 patients with isolated growth hormone deficiency or hypopituitarism were studied. Lipid concentrations were estimated i

Factors affecting growth and strategies for treatment in children after renal transplantation.

Growth failure is a frequent side effect of long-term high-dose glucocorticoid therapy in children. Pharmacological doses of glucocorticoids interfere at different levels with the integrity of the somatotropic hormone axis. The majority of patients on chronic corticosteroid medication present with overt GH hyposecretion, apparently due to enhancement of hypothalamic somatostatin release. Glucocorticoids also inhibit IGF bioactivity by the induction of IGF inhibitors and stimulate the production of certain IGFBPs. In addition, glucocorticoids inhibit growth directly at the tissue level by suppr

Prolactinoma associated with transient growth hormone deficiency but persistent growth retardation.

A 14.8 year old boy was evaluated for galactorrhea of two months duration and growth deceleration for greater than three years. He was 3.7 standard deviations (SD) below the mean for age in height and euthyroid with uncompromised vision, bilateral galactorrhea, and pubertal arrest. MRI demonstrated a 10 x 8 mm left pituitary mass. Bone age was 11.5 years. Serum prolactin (PRL) decreased by more than 85% after 5 weeks of treatment with bromocriptine (Br). After five months, the prolactinoma (PRLoma) measured 5 x 4 mm. Hypothalamic-pituitary function indicated growth hormone (GH) deficiency and

[Slipped capital femoral epiphysis and biosynthetic growth hormone therapy. A case report].

The case of an adolescent male with short stature and partial growth hormone deficiency who developed a slipped capital femoral epiphysis during the treatment with recombinant growth hormone is reported in this paper. Our patient started GH therapy with recombinant growth hormone at the dose of 15 U/m2/week administered subcutaneously three times a week. After 6 months of GH therapy there was a satisfactory response to the therapy and his growth velocity improved significantly. Unfortunately the patient had pain of the left hip which was exacerbated by walking. The diagnosis of slipped capital

Growth disturbance after osteomyelitis of femoral condyles in infants.

Growth disturbance after osteomyelitis in a single femoral condyle was seen in seven infants followed by the author from infancy to maturity. Age at onset of infection was between 10 days and 4 weeks. In three cases the medial condyle was affected and in four the lateral. Angular deformity of 20-50 degrees of varus or valgus was present before correction by supracondylar osteotomy. In all seven cases the radiographic appearance before the first osteotomy was similar. Partial closure of the growth plate appeared in four cases at age 6, 7, 10 and 12 years, respectively, which was decisive for lo

Solitary maxillary central incisor and short stature.

Seven patients, three males and four females, each with a single (unpaired) deciduous and permanent maxillary central incisor, were studied. All the males and two the female children were growth hormone deficient. One adult woman and a female infant with a single maxillary central incisor were short in stature but had normal growth hormone responses. No other dental anomalies or pituitary-hypothalamic dysfunctions were found. The dental anomaly was not familial. No eye abnormalitis were present in the patients or their families.

Transitory growth hormone deficiency successfully treated with human growth hormone.

This study was carried out in order to determine whether children with a transitory type of growth hormone deficiency showed an accelerated growth in height velocity on treatment with human growth hormone (HGH). Following careful diagnostic routine procedures 13 extremely short children were diagnosed as having isolated growth hormone deficiency, and were successfully treated with HGH. A true isolated growth hormone deficiency was present in 5 of the children, whereas 8 showed a normal increase in serum growth hormone on repeated growth hormone stimulation tests after their development of pube

The short child: Importance of early detection and timely referrai.

Stunting is a common phenomenon in Malaysian children. Optimising outcomes for children with growth disorders rests on early recognition and prompt referral. In this context, a framework for the clinical approach can help to guide appropriate growth assessment and referral. This review article aims to provide family medicine specialists with such a framework whilst raising awareness about the shortcomings of the existing growth monitoring system in Malaysia. It also invites readers to consider additional measures that could further optimise this system.

How should we investigate children with growth failure?

The early diagnosis of short stature is essential for effective management and treatment. Investigations for children with growth failure are required to distinguish between idiopathic short stature due to physiological variants (familial short stature, and constitutional delays of growth and puberty, or both), primary causes of short stature, such as syndromic and/or genetic defects and skeletal dysplasia, and secondary growth deficits due to endocrine or other chronic disorders such as celiac disease, Crohn's disease, malnutrition, renal, anorexia nervosa or other chronic diseases.

For Debate: Constitutional and Non-Constitutional Delay of Growth and Puberty.

Herewith a definition of constitutional delay of growth and puberty shall be given. Whereas constitutional delay of growth and puberty is a hereditary variant of normal growth and development occurring in both sexes and is irrelevant to the familial height, non-constitutional delay is secondary to a variety of underlying disorders like chronic diseases, malnutrition, persistence of psychologic problems or hormonal anomalies. Constitutional delay occurs in boys and girls and will end in normal final height within the familial height range.

Childhood psychiatric disorders as anomalies in neurodevelopmental trajectories.

Childhood psychiatric disorders are rarely static; rather they change over time and longitudinal studies are ideally suited to capture such dynamic processes. Using longitudinal data, insights can be gained into the nature of the perturbation away from the trajectory of typical development in childhood disorders. Thus, some disorders may reflect a delay in neurodevelopmental trajectories. Our studies in children with attention-deficit/hyperactivity disorder (ADHD) suggest that cortical development is delayed with a rightward shift along the age axis in cortical trajectories, most prominent in

Eating disorders in white and black women.

OBJECTIVE: Epidemiological studies of eating disorders in the United States have focused on white women and girls, and the prevalence of eating disorders in ethnic minority groups is unknown. This study examined the prevalence of anorexia nervosa, bulimia nervosa, and binge eating disorder in a geographically and economically diverse community sample of young white and black women who previously participated in the 10-year National Heart, Lung, and Blood Institute (NHLBI) Growth and Health Study. METHOD: All NHLBI Growth and Health Study participants were recruited for this study. A two-stage
