Cystic hygroma, also known as lymphangioma, are unusual congenital malformations of the lymphatic system. Cystic lymphangioma is a rare lesion in the breast of children. Only a few cases have been documented in literature. We report a rare case of a 5-year-old boy presented with a gradually enlarging, painless breast mass which was identified sonographically and histologically as a breast lymphangioma and treated by surgical excision. We investigated the relationship between sex hormone binding globulin (SHBG) and pubertal gynecomastia in 21 adolescents evaluated longitudinally. Thirteen patients were given tamoxifen treatment after grading according to the Nydick classification (group 1). Group 2 consisted of eight patients followed without treatment. Levels of serum testosterone (T), estradiol (E2), and estrogen receptor (ER) in the tissue were detected by radioimmunoassay and immunohistochemical method (ABC technique) in 46 patients with gynecomastia. The results showed that rise of E2 and E2/T is a whole-body etiologic factor of hypertrophy of male breast, and the positive rate of ER is an important local etiologic factor of gynecomastia. E2 and ER have aosynergism effect on onset and development of gynecomastia and cause the ductal cell hyperplasia of male breast. It should be alerted whether it can cause male breast cancer.

Severe gynecomastia with excessive skin is difficult to treat by only periareolar excision or suction-assisted lipectomy or both. In these patients, total mastectomy and free nipple grafting may be the best option. Placement of the nipple, however, has been arbitrary.
