Dolutegravir with enteral feeds in intensive care: Case series with therapeutic drug monitoring.

People with HIV admitted to the intensive care unit (ITU) often receive enteral nutrition via feeding tubes. Enteral feeds containing cations may interfere with dolutegravir (DTG) absorption via chelation and thus temporal spacing is recommended. This can lead to suboptimal caloric intake. We reviewed similar cases at our institution, an electronic health record and data collection, including people with HIV hospitalized in ITU with enterally administered DTG and therapeutic drug monitoring (TDM). Six cases (four males, two females, median age 34 years, median ITU admission 44.5 days, median 4

IL-18 and MIG increase one year after transition to dolutegravir-based dual therapy despite sustained viral suppression.

INTRODUCTION: Modern antiretroviral treatment allowed a significant improvement in the life expectancy of people with HIV (PWH), exhibiting robust and sustained efficacy with scarce toxicities, both in two-drug regimens (2DRs) and three-drug regimens (3DRs). However, a proinflammatory condition is observed even in virologically suppressed PWH. The aim of this study is to assess prospectively the dynamics of various cytokines in PWH who transition from a 3DR to a 2DR. METHODS: A prospective cohort study including 35 virologically suppressed PWH who switched from a 3DR to a dolutegravir-based 2

A prospective assessment of the efficacy and durability of long-acting cabotegravir and rilpivirine in individuals with HIV in Spain (RELATIVITY study).

BACKGROUND: Daily oral antiretroviral therapy (ART) has transformed HIV into a chronic, manageable condition, but it still poses psychological, logistical, and biological challenges. Long-acting injectable (LAI) cabotegravir plus rilpivirine provides an alternative for virologically suppressed individuals, reducing treatment fatigue, stigma, and daily dosing. While clinical trials have demonstrated efficacy and safety, real-world data remain limited. We assessed the effectiveness and safety of LAI cabotegravir plus rilpivirine in Spain. METHODS: RELATIVITY is a 5-year, multicentre, ambispecti

Congenital malformations and preeclampsia associated with integrase inhibitor use in pregnancy: A single-center analysis.

BACKGROUND: Antiretroviral therapy (ART) decreases perinatal HIV transmission, but concerns exist regarding maternal and infant safety. We compared the incidence of congenital malformations and other adverse outcomes in pregnancies exposed to integrase inhibitor (INSTI) versus non-INSTI ART. SETTING: Single-site review of all pregnancies among women living with HIV between 2008 and 2018. METHODS: We used binomial family generalized estimating equations to model the relationship of congenital anomalies and pregnancy outcomes with exposure to INSTI or dolutegravir (DTG) versus non-INSTI ART.

Viral suppression after transition from nonnucleoside reverse transcriptase inhibitor- to dolutegravir-based antiretroviral therapy: A prospective cohort study in Lesotho (DO-REAL study).

OBJECTIVES: Since 2018, the World Health Organization has recommended dolutegravir (DTG)-containing antiretroviral therapy (ART) for most people living with HIV. Country programmes across Africa have subsequently transitioned from other, mostly nonnucleoside reverse transcriptase inhibitor (NNRTI)-based ART to DTG-based ART. This study aims to assess the virological impact of programmatic transitioning to DTG-based ART in Lesotho. METHODS: The prospective Dolutegravir in Real-Life in Lesotho (DO-REAL) cohort enrols people living with HIV initiating or transitioning to DTG-based ART in Lesotho

Dolutegravir Has No Effect on the Pharmacokinetics of Oral Contraceptives With Norgestimate and Ethinyl Estradiol.

BACKGROUND: Dolutegravir (DTG; Tivicay; ViiV Healthcare, Research Triangle Park, NC) is an HIV-1-unboosted integrase inhibitor with no cytochrome P450 or uridine 5'diphosphate-glucuronosyltransferase inhibition or induction. As DTG is administered to HIV-1-infected women receiving oral contraceptives, assessing the potential for drug interactions was warranted. OBJECTIVE: To determine the impact of DTG on the pharmacokinetics (PK) and pharmacodynamics (PD) of a common oral contraceptive, norgestimate/ethinyl estradiol (NGM/EE; Ortho-Cyclen; Ortho-McNeil-Janssen Pharmaceuticals, Inc, Raritan,

Viral load suppression among pregnant and breastfeeding women living with HIV in Ghana: a prospective longitudinal study.

BACKGROUND: The post-delivery period is a critical window for sustaining maternal viral suppression (VS) to prevent vertical transmission (VT) of HIV during breastfeeding. Despite Ghana’s national scale-up of dolutegravir-based antiretroviral therapy, limited data exist on viral load suppression and infant HIV outcomes in the year post-delivery. We aimed to determine the proportion of women who achieved and maintained a suppressed viral load (VL) (≤ 1000 copies/mL) and an undetectable VL (< 20 copies/mL) at 12 months post-delivery and examine associations between maternal characteristics and V

Bictegravir/emtricitabine/tenofovir alafenamide in patients with genotypic NRTI resistance.

BACKGROUND: Bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF) is approved for treatment of HIV without known resistance to its components. Several studies have demonstrated efficacy of B/F/TAF in patients with nucleoside reverse transcriptase inhibitor (NRTI) resistance-associated mutations (RAMs), mainly identified by proviral DNA testing, but data on the efficacy of B/F/TAF in patients with NRTI RAMs identified in viraemic plasma are limited. METHODS: We used a retrospective analysis of patients receiving B/F/TAF identified by searching electronic health records with eligibility con

Brief communication: virological outcomes and dolutegravir resistance mutations in HIV-infected patients: a multicenter retrospective cohort study in Mozambique.

The global HIV epidemic remains a major public health challenge, with DTG playing a key role in ART regimens due to its efficacy and tolerability. This study evaluated virological outcomes and resistance mutations in patients on DTG in Mozambique through a retrospective cohort study in seven DREAM centers. Data from 29,601 patients (98.1% on DTG) revealed a virological suppression rate of 95% (27,622/29,051). Factors positively associated with suppression included age > 50, longer ART duration, and being female. Of 17 resistance tests, 8 showed major mutations, including G118R and E138K. Resul

Elevated liver enzymes resulting from an interaction between Raltegravir and Panax ginseng: a case report and brief review.

BACKGROUND: In the last decade, some evidence has arisen supporting the usefulness of Asian ginseng (Panax ginseng, fam. Araliaceae) as a complementary remedy in patients receiving antiretroviral therapy. However, its role in current therapeutics remains unclear. METHODS: The patient was admitted for an acute elevation of liver enzymes, marked jaundice, and significant weight loss after taking ginseng-based tablets starting approximately 39 days prior. His past medical history (PMH) was also significant for HIV+, long-term hepatitis C, an episode of mitochondrial toxicity, and several comorbi

Diet quality and nutritional status in people living with HIV on dolutegravir therapy.

While antiretroviral therapy (ART) has improved life expectancy for people living with HIV (PLWHIV), it has also brought new health challenges, including poor diet quality and excess weight. This cross-sectional study evaluated the diet quality and nutritional status of PLWHIV on dolutegravir (DTG)-based therapy in a Brazilian specialized care service. Adults (≥18 years) who initiated DTG between February 2017 and March 2020 were included. Data on sociodemographic, clinical, anthropometric, and dietary aspects were collected through interviews and medical records. Among the 148 participants (9

Associations between change in BMI and the risk of hypertension and dyslipidaemia in people receiving integrase strand-transfer inhibitors, tenofovir alafenamide, or both compared with other contemporary antiretroviral regimens: a multicentre, prospective observational study from the RESPOND consortium cohorts.

BACKGROUND: Integrase strand-transfer inhibitors (INSTIs) and tenofovir alafenamide have been associated with weight gain in several clinical trials and observational cohorts. However, whether weight gain associated with INSTIs and tenofovir alafenamide confers a higher risk of weight-related clinical events is unclear. We aimed to assess whether changes in BMI differentially increase hypertension or dyslipidaemia risk in people with HIV receiving INSTIs, tenofovir alafenamide, or both versus other contemporary regimens. METHODS: This multicentre, prospective observational study analysed pros

Cabotegravir-loaded polymeric micelles: a promising strategy for improved solubility and therapeutic outcomes.

OBJECTIVE: To design and optimize a d-α-tocopheryl polyethylene glycol 1000 succinate (TPGS)-stabilized polymeric micellar system for cabotegravir (CAB), strategically addressing its dissolution-rate and permeability constraints to potentiate oral bioavailability and maximize antiretroviral therapeutic outcomes. SIGNIFICANCE: CAB, a potent HIV-1 integrase strand transfer inhibitor, suffers from dissolution-rate limitations and P-glycoprotein efflux, restricting oral utility. Incorporation of TPGS enables superior solubilization, enhances intestinal translocation, and circumvents efflux transp
