Imaging Findings of Metabolic Bone Disease.

Metabolic bone diseases are a diverse group of diseases that result in abnormalities of (a) bone mass, (b) structure mineral homeostasis, (c) bone turnover, or (d) growth. Osteoporosis, the most common metabolic bone disease, results in generalized loss of bone mass and deterioration in the bone microarchitecture. Impaired chondrocyte development and failure to mineralize growth plate cartilage in rickets lead to widened growth plates and frayed metaphyses at sites of greatest growth. Osteomalacia is the result of impaired mineralization of newly formed osteoid, which leads to characteristic L

Reossification in Gorham's disease of the hand and wrist with unusual CT and MR imaging features.

Gorham's disease (GD) rarely occurs in the hand and wrist. Only nine cases of GD in the hand and wrist have been reported in the literature. The imaging technique used in all nine cases was mainly radiography. The natural history of GD is unpredictable. Spontaneous regression has been reported in a few cases. There is no consensus about the most efficient treatment of GD. Surgical resection and reconstruction with bone grafts and/or prostheses are used sparingly as bone grafts tend to be resorbed in most cases. We report a case of GD that involved the right hand and wrist in a 26-year-old male

[Reconstruction of through and through gunshot injuries of the hand with a double skin paddle reverse-flow forearm radial flap. Two case reports].

The authors report two cases of gunshot wounds to the hand treated with a double skin paddle reverse-flow forearm flap. The two cases, male and female, were due to a self-inflicted rifle injury. The wounds were severe and extensive with full-thickness palmar and dorsal skin, tendon, muscle, bone, vessel and nerve losses. Treatment was performed in a single stage with skeletal stabilization, bone grafting and nerve and tendon reconstruction. The double palmar and dorsal skin defect was reconstructed with a double skin paddle reverse-flow forearm flap which was routed through the hand wound. The

Osteoid osteomas of the hands and feet: a series of 71 cases.

Osteoid osteomas typically arise in the long bones of extremities. Patients often report pain relieved by NSAIDS, and radiographic findings are often sufficient for diagnosis. However, when involving the hands/feet, these lesions may go unrecognized or misdiagnosed radiographically due to their small size and prominent reactive changes. The clinicopathologic features of this entity involving the hands and feet are not well-described. Our institutional and consultation archives were searched for all cases of pathologically confirmed osteoid osteomas arising in the hands and feet. Clinical data

[X-ray densitometry and biochemical parameters of bone tissue metabolism in patients with vibration disease].

To reveal systemic and local osteoporosis, the authors studied biochemical markers of bone metabolism in vibration disease patients. The vibration disease patients appeared to have the most frequent and marked osteoporosis in peripheral bones--hands (in 90% of cases) and forearms (in 66.7%). Prevalence of systemic osteoporosis and osteopenia reached 11.7 and 48.3% in the select respectively. According to biochemical markers, bone reconstruction state was characterized by moderately intensified bone resorption and diminished bone formation.

[The sphenoid bone: anatomy].

In this paper, the anatomy of the sphenoid bone and surrounding and associated structures is described. The sphenoid bone is centrally located in the skull base. The anatomy of the bony canals, foramina and fissures that run through the sphenoid bone or run along its borders must be known. The osseous anatomy can be studied using CT. On the other hand, MR is mandatory for visualization of the vascular and nervous structures in and around the sphenoid bone.

Hand workload and the metacarpal cortical index. a study of middle-aged teachers and dentists.

We studied load-related predictors (handedness, occupation and workload history) of cortical bone mass as estimated by the metacarpal cortical index (MCI). After being randomly selected from trade-union registers, 295 female dentists and 248 teachers, aged 45-63 years (mean 54 years), participated in the study. The MCI was defined from the second metacarpal of both hands by analogue radiographs. Data regarding anthropometric measures, handedness, occupational exposure, number of pregnancies, hormonal therapy and contraceptive use, the presence of chronic diseases, dietary factors, smoking and

Coccidioidomycosis fungal infection in the hand mimicking a metacarpal enchondroma.

Coccidioidomycosis, an infection caused by the fungus Coccidioides immitis, rarely affects the hand, but we report an unusual case which mimicked the radiological appearance of an enchondroma in the metacarpal. Curettage and bone grafting in combination with long-term antifungal therapy are necessary for successful treatment of coccidioidomycosis of the hand.

Central and ulnar cleft hands: a review of concurrent deformities in a series of 47 patients and their pathogenesis.

Two main types of cleft hands have been described. The ulnar cleft hand deformity is very rare and is characterized by two constant features: a deep cleft radial to the little finger and hypoplasia of the ulnar digits. The pathogenesis of ulnar clefts is unknown. The second type is the central cleft hand deformity, which is characterized by a soft tissue/bone defect in the hand centrally. Patients with central clefts also have several concurrent deformities in the remaining digits. This paper reviews the clinical features of three cases with ulnar cleft hands and 44 cases of central cleft hand

Musculoskeletal involvement in systemic sclerosis.

Systemic sclerosis (SSc) is a multi-organ, systemic connective tissue disease, which affects the lungs, heart, gastrointestinal tract, kidneys, skin, and musculoskeletal system. Musculoskeletal involvement is observed in 40-90% of patients with SSc. During the disease, any structure of the musculoskeletal system, such as bones, joints, tendon sheaths, tendons, and muscles, may be affected. The most common symptoms include joint pain, arthritis, tendinitis leading to tendon rupture, acro-osteolysis, calcinosis, myalgia, and myositis. Osteo-articular complications and changes in the soft tissues

A Large Osteoid Osteoma of Trapezium: A Regenerative Approach and a Review of Literature.

Osteoid osteoma is a benign bone tumor that usually grows in the long bones of the body and arises from osteoblasts and some components of osteoclasts. It represents the third most frequent type of benign bone tumors, accounting for 11% to 14% of the tumors. The entity usually involves the proximal femur and tibia. It has also been reported in the hand, especially the scaphoid, capitate, and proximal phalanx. The most common symptom is pain, usually during the night, relieved by the use of salicylates and nonsteroidal anti-inflammatory drugs. To date, only 5 cases involving the trapezium have

The prevalence of pseudoepiphyses in the metacarpals of the growing hand.

Normally the metacarpals have an epiphysis at one end--distally for the second to fifth and proximally for the first. Pseudoepiphyses are notches or clefts that occur at the non-epiphyseal ends of bones where an epiphyseal plate would be expected and are common incidental findings in the metacarpals of the growing hand. We aimed to identify the prevalence of pseudoepiphyses on serial radiographs of 610 healthy asymptomatic children. Pseudoepiphyses in the form of notches or clefts were common, identified most often in the second metacarpal (15.25%), fifth metacarpal (7.21%), and third metacarp

Solitary phalangeal brown tumour in primary hyperparathyroidism: Report of a rare presentation.

Parathyroid adenoma is the most common cause of primary hyperparathyroidism. Severe cases of primary hyperparathyroidism manifest as osteitis fibrosa cystica generalisata, characterized by generalized bone loss with increased bone resorption, including both subperiosteal and endosteal surfaces. The most common sites for formation of fibrotic cystic lesions (brown tumors) are in the long bones and jaw which present as swelling, pathological fracture, and/or bone pain, usually involving multiple sites. Here, we describe an unusual presentation of a solitary brown tumor in a young male who initia

Effect of implant design on the cyclic loading properties of mini suture anchors in carpal bones.

Mini suture anchors are commonly used in hand surgery involving attachment of soft tissue to bone. There are few data on the biomechanical behavior of these implants under physiologic conditions. Commercially available mini anchors were inserted into the carpal bones of fresh-frozen cadaver wrists. Anchors were divided into threaded and pronged types according to design. The anchors were cyclically loaded. Pronged anchors (1.3 and 2.1 mm) failed at a range of 199 to 5,155 cycles. Threaded anchors (2.5 and 2.8 mm) consistently completed 40,000 cycles without failure. Before failure pronged anch

Correcting and lengthening of metatarsal deformity with circular fixator by distraction osteotomy: a case of longitudinal epiphyseal bracket.

The condition known as longitudinal epiphyseal bracket or delta phalanx is a rare congenital anomaly that affects the tubular bones of hand or foot. Metatarsal epiphyseal bracket restrains longitudinal growth, causing progressive deformity and resulting in a short, broad metatarsal and a medially deviated metatarsophalangeal joint and hallux magnus. Although there are some case series describing metatarsal lengthening with the unilateral fixator in the literature, we could not find any case that combines both metatarsal lengthening and deformity correction with the circular external fixator. Horse-related injuries in a thoroughbred stabling area in Japan.

To investigate the demographic details and patterns of injuries related to horse handling, we reviewed 637 horse-related injuries in 581 stable- or stud-workers in a representative area of thoroughbred stabling in Japan. We found that (1) injuries occurred most frequently in a group of a relatively young workers, with a seasonal variation; (2) the principal mechanism of injury was kicks, which accounted for 39.2% of all injuries, including 11 serious and one lethal visceral injuries; (3) the upper half of the body was more frequently involved than the lower half; and (4) the peripheral bones (

The usefulness of quantitative evaluation of three-phase scintigraphy in the diagnosis of post-traumatic reflex sympathetic dystrophy.

Quantitative analysis of three-phase bone scintigrams was done in 70 patients with reflex sympathetic dystrophy (RSD) and in 30 patients who did not have RSD after injury to the hand or wrist. Regions of interest were selected and the uptake ratios (affected/unaffected) were calculated. Significant differences between affected and unaffected patients were seen in the metacarpal area in phase 2 of the scintigrams as well as in the metacarpophalangeal joints and metacarpal bones in phase 3. Combination of these images had the greatest diagnostic value. Sensitivity and specificity of 80% were ach

Enlarged hands and feet - Not always acromegaly.

Pachydermoperiostosis maybe mistaken for acromegaly as it can present with progressive enlargement of hands and feet. We describe a 32 year old male with enlargement of hands and feet and extensive keloid formation. Family history was positive for similar complaints. X ray imaging showed normal heel pad thickness with acroosteolysis and subperiosteal new bone formation in hands and feet. IGF-1 was normal and glucose suppressed GH values were normal.

Cinegammography and 3-D emission tomoscintigraphy for evaluation of revascularized mandibular bone grafts: a preliminary report.

The following reports a study conducted to investigate a scintigraphic monitoring procedure for bone free flap in mandibular reconstruction. This procedure is based on the one hand, on vascular and bone cinegammography, and on the other hand, on vascular and bone 3-D tomoscintigraphy. We used a prospective cohort study design. All patients who underwent free flap mandibular reconstruction from April 1993 to April 1998 in the Salpêtrière University Hospital, France (20 cases) have been evaluated in relation to this scintigraphic procedure. Vascular and bone scintigraphy were performed within th

[Matev's digit lengthening technic. Apropos of 20 cases].

The authors reviewed 20 lengthening procedures according to Matev performed between 1982 and 1985 at SOS Main Strasbourg. They developed a special device which has proved to be simple and easy to use. Indications are limited to a few congenital malformations and traumatic mutilations. In children a free vascularized epiphysis transfer has been used to allow normal growth. Results are analysed and short-comings are outlined: lengthening is limited (47.8%); frequency of complications mainly trivial account for 25%; treatment period is lengthy (2.8 m in children and 4.2 m in adults).
