Impact of a Clinical Decision Support System on Guideline Adherence of Surveillance Recommendations for Colonoscopy After Polypectomy.

Background: Surveillance colonoscopy is required in patients with polyps due to an elevated colorectal cancer (CRC) risk; however, studies suggest substantial overuse and underuse of surveillance colonoscopy. The goal of this study was to characterize guideline adherence of surveillance recommendations after implementation of an electronic medical record (EMR)-based Colonoscopy Pathology Reporting and Clinical Decision Support System (CoRS). Methods: We performed a retrospective cohort study of patients who underwent colonoscopy with polypectomy at a safety-net healthcare system before (n=1,82

The high-risk liver allograft recipient. Should allocation policy consider outcome?

The Boston Center for Liver Transplantation has accumulated one of the larger series of liver allograft recipients. This review has provided an opportunity to examine recent pronouncements by Medicare regarding patient selection and survival and to question whether the current allocation scheme best utilizes a scarce supply of donor liver allografts. Patients with primary biliary cirrhosis, sclerosing cholangitis, and metabolic derangements have enjoyed excellent survival: in aggregate, 78.9% at 1 year. In contrast, patients suffering from acute hepatic failure, patients requiring life support

Policy choices in dementia care-An exploratory analysis of the Alberta continuing care system (ACCS) using system dynamics.

BACKGROUND: The increase in the incidence of dementia in the aging population and the decrease in the availability of informal caregivers put pressure on continuing care systems to care for a growing number of people with disabilities. Policy changes in the continuing care system need to address this shift in the population structure. One of the most effective tools for assessing policies in complex systems is system dynamics. Nevertheless, this method is underused in continuing care capacity planning. METHODS: A system dynamics model of the Alberta Continuing Care System was developed using

Palliative care and support for persons with HIV/AIDS in 7 African countries: implementation experience and future priorities.

To combat morbidity and mortality from the worldwide epidemic of the human immunodeficiency virus (HIV), the United States Congress implemented a President's Emergency Plan for AIDS Relief (PEPFAR) in 30 resource-limited countries to integrate combination antiretroviral therapy (ART) for both prevention and cure. Over 35% of eligible persons have been successfully treated. Initial legislation cited palliative care as an essential aspect of this plan but overall health strengthening became critical to sustainability of programming and funding priorities shifted to assure staffing for care deliv

Effectiveness of collaborative care depression treatment in Veterans' Affairs primary care.

OBJECTIVE: To compare collaborative care for treatment of depression in primary care with consult-liaison (CL) care. In collaborative care, a mental health team provided a treatment plan to the primary care provider, telephoned patients to support adherence to the plan, reviewed treatment results, and suggested modifications to the provider. In CL care, study clinicians informed the primary care provider of the diagnosis and facilitated referrals to psychiatry residents practicing in the primary care clinic. DESIGN: Patients were randomly assigned to treatment model by clinic firm. SETTING:

Oral implants in dependent elderly persons: blessing or burden?

BACKGROUND: Implant-supported (partial) dentures may raise problems in patients who have become dependent on others for daily oral health care. Dental hygienists and general dental practitioners, as well as care providers, volunteer aiders and even health care insurance companies, should anticipate the growing demand for specific oral health care for patients provided with implant-supported (partial) dentures. OBJECTIVE: To report three cases of dependent patients and to present recommendations to prevent or resolve implant-related oral problems. MATERIALS AND METHODS: The three case reports

[Rethinking aging, autonomy and disability in France: A University-driven, territory-based, and prevention-oriented transformation of the health system - Position of the French Conference of Deans of Medical Faculties].

Population ageing, the increasing burden of chronic diseases, and the growing prevalence of disability are major challenges for the French health system. These trends expose the limitations of a model that remains largely organized around disease and dependency, with fragmented coordination between healthcare, social care, and community services. This fragmentation results in disrupted care pathways, delayed identification of functional decline, territorial inequalities, and an increasing burden on patients, caregivers, and health professionals. This position paper presents the vision of the F

Models of care across settings supporting ageing in place: a narrative review.

Older people's preference is to age in place. With an ageing population, the demand for services that are effective in supporting older people to live at home independently has increased dramatically. This narrative review provides an overview of recent evidence of models of care in the aged and community care and health care sectors that contribute to supporting older people (≥ 65 years) to age in place (ie, delay or avoid entry into residential long term care). Overall, there is limited evidence for the identified models of care about the outcome of ageing in place, but there is evidence of

Interactive Web-Based Learning: Translating Health Policy Into Improved Diabetes Care.

In August 2014, the U.S. DHHS's Office of Disease Prevention and Health Promotion released the National Action Plan for Adverse Drug Event Prevention, highlighting prevention of diabetes agent-related hypoglycemia as a key area for improvement. In support of the Action Plan, the Office of Disease Prevention and Health Promotion then developed a web-based interactive module, or eLearning lesson, based on formative research and stakeholder feedback to educate healthcare professionals on strategies to prevent adverse drug events from diabetes agents. The training incorporates health literacy prin

Transforming Clinical Education While Addressing System Gaps: Student Leadership Experiences (SLEs).

Health and social care systems are increasingly characterized by complexity, resource constraints, and evolving service demands, requiring educational models that prepare learners to navigate and lead within dynamic environments. Student Leadership Experiences (SLEs) are proposed as a theory-informed educational model that extends beyond traditional workplace-based learning by intentionally integrating system responsiveness, collaborative leadership, and interprofessional engagement. SLEs are structured around identified system or practice gaps and position students not only as participants in

Pharos: a mobile GIS application for network assessment and ground-truth mapping to support mHealth study design in low-resource settings.

Mobile health (mHealth) is a promising tool for improving healthcare access, particularly in low-resource settings. However, limited mobile accessibility and poor connectivity remain significant barriers to implementing mHealth interventions in these regions. To address these challenges and support the development and scalability of mHealth studies, we developed Pharos, a mobile GIS application designed to assess network coverage and facilitate ground-truth mapping. Pharos autonomously measures spatiotemporal variations in mobile network signal strength and enables precise mapping of critical

A clinical intervention for children exposed to alcohol in utero.

This study describes a statewide system of delivering special services to children affected by prenatal alcohol exposure and investigates the usefulness of this system as part of a statewide fetal alcohol syndrome surveillance system. The Alaska Department of Health and Social Services administered seven clinics in four of the five largest cities in Alaska during 1992-1995. Among the 74 children selected to be evaluated for fetal alcohol syndrome, 70 had one or more central nervous system abnormalities. Four children were assigned the diagnosis of fetal alcohol syndrome. Recommendations made t

Evaluation of a health trainer-led service for people with knee, hip and back pain.

AIMS: Chronic knee, hip and back pain is extremely prevalent. Management guidelines emphasise maintaining physical activity and healthy weight to reduce pain and improve physical and mental wellbeing. Unfortunately, few people receive support to make lifestyle changes. We evaluated whether a health trainer-led 'joint pain advice' (JPA) service delivering person-centred lifestyle coaching was feasible, acceptable and effective for people with knee, hip and back pain. METHODS: Feasibility of delivering a JPA service was assessed by documenting whether the health trainers could deliver JPA and i

Migration to the US among rural Puerto Ricans who inject drugs: influential factors, sources of support, and challenges for harm reduction interventions.

BACKGROUND: While PWID of Puerto Rican origin have been migrating to the US for decades, the range of factors influencing their migration to the US and the resources they draw on to do so are not well understood. This is particularly true for rural Puerto Rican PWID, and the present study is the first empirical research to document migration patterns among this population. The specificities of their migration raise important challenges that need to be documented in order to implement more effective harm reduction policies at home (Puerto Rico) and abroad (US). METHODS: This paper draws from d

An innovative approach to assess comprehensive care: Temple University's patient wellness report.

PURPOSE: The transition from a paper-based to an all-electronic patient health record took a major step forward in 2018, with the implementation of an electronic health record that supports the School's patient-centered comprehensive care model and facilitates outcomes assessment. The Patient Wellness Report (PWR) summarizes findings of the patient assessment, and it does so automatically by locating data already entered in axiUm forms. This study aimed to describe the PWR implementation procedures and to examine outcomes and characteristics among patients with completed treatment plans during

From data entry to digital transformation: Allied health perspectives on standardised electronic medical records data.

BACKGROUND: Electronic medical records (EMRs) currently rely on standardised data fields to support secondary data use for clinical care, performance monitoring, and system-level reporting. However, utilisation of standardised data capture and reporting within allied health remains underdeveloped in practice. Greater understanding of how allied health clinicians and managers perceive the purpose, value, and impact of standardised data collection is needed to support advancement in data maturity and transformation of utilisation. OBJECTIVE: To explore the perceived benefits, challenges and fut
