Single Leg Drop and Hop: Insight Into Multisegment Foot Kinematics, Kinetics and the Role of Visual Focus in Healthy Young Adult Males.

INTRODUCTION: Understanding foot joint loading during different dynamic activities is essential information for guiding exercise progression in rehabilitation. While walking and running biomechanics are well studied, joint-specific kinetic data during a single leg drop and hop task, often used in rehabilitation, are lacking. This study aimed to evaluate (1) the kinetic behavior of the ankle, Chopart, Lisfranc, and MTP-1 joints during a drop-hop task under different visual constraints and (2) to contextualize these findings by comparing them with heel-strike running, to assess the relative load…

The lived experience of migrant Syrian mothers' interaction with the neonatal screening program.

BACKGROUND: The aim of this study was to evaluate the knowledge, attitudes, and practices of migrant Syrian mothers regarding the neonatal screening programs (NSP) using in-depth interviews. METHODS: Women were selected from the Migrant Health Center (MHC), which is the primary health care institution in Istanbul. A translator who spoke Arabic, the native language of participants, used an open-ended, semi-structed interview form to guide face-to-face interviews with participants. Questions were about sociodemographic information, opinions about NSP's heel prick testing, diseases the test scre…

Evaluation of in-shoe plantar pressure and shear during walking for diabetic foot ulcer prevention.

OBJECTIVE: To investigate reliability and changes of in-shoe plantar pressure and shear during walking at three cadences with two insole designs. This was a precursor to the investigation of plantar loading in people with diabetes for potential foot ulcer prevention. METHOD: A sensorised insole system, capable of measuring plantar pressure and shear at the heel, fifth metatarsal head (5MH), first metatarsal head (1MH) and hallux, was tested with ten healthy participants during level walking. Reliability was evaluated, using intra-class correlation coefficient (ICC), while varying the cadences…

Mechanobiological and neuromuscular responses to foot-position variations during front and back squat exercises.

INTRODUCTION: This study investigated the mechanobiological and neuromuscular effects of three foot-position techniques-flat heel (FH), heel-elevated (HE), and forefoot-elevated (FE)-during front and back squat exercises. Variations in foot positioning were expected to influence muscle activation, joint kinematics, ground reaction forces, and postural stability. METHODS: Twelve resistance-trained male athletes (mean age 23.4 ± 3.8 years; training experience 8.1 ± 2.7 years) performed squats at 70% of their one-repetition maximum (1RM) under each foot-position condition. Surface electromyograp…

Causal relationship between stable angina and bone mineral density: A two-sample bidirectional Mendelian randomization study.

This study assesses the causal relationship between bone mineral density (BMD) and stable angina pectoris (SAP) using Mendelian randomizatio … factor, the analysis indicated an increased risk of SAP with higher BMD. Similarly, when only left heel BMD was considered as the exposure factor, a positive correlation with SAP was observed. However, no significant associations were observed between BMD at other sites and SAP. When SAP is the exposure factor and BMD is the outcome factor, left heel …

Umbilical Cord Blood Sampling for Newborn Screening of Pompe Disease and the Detection of a Novel Pathogenic Variant and Pseudodeficiency Variants in an Asian Population.

Pompe disease is an autosomal recessive metabolic disorder caused by acid alpha-glucosidase (GAA) deficiency. The use of umbilical cord blood (UCB) for newborn screening (NBS) of Pompe disease, compared to heel-prick sampling, has not been widely studied. This study compared GAA activity in UCB from term newborns with peripheral or heel-prick blood samples obtained on days 1, 2, and 3 after birth. Enzyme assays were performed using UPLC-MS/MS. Sanger sequencing was conducted in infants with low GAA activity to identify pathogenic variants. Among 4091 UCB samples analyzed over 18 months, the me…

Prescheduled Interleaving of Processing Reduces Interference in Motor-Cognitive Dual Tasks.

Continuous motor tasks like walking have the potential to allow a dynamic allocation of processing resources when interrupted by intermittent cognitive tasks. The degree to which a successful interleaving of processing streams of both tasks is possible may depend on the temporal regularity of events. Fifteen subjects participated in an experiment where we systematically manipulated the regularity of stimulus onsets in a 2-back task relative to the step cycle. We tested three conditions where stimulus onset was always synchronous to a defined event in the stride (right heel strike, left heel st…

The medial plantar artery flap: a series of cases over 14 years.

Coverage of the weightbearing heel poses a unique technical challenge to the reconstructive surgeon. In the present study, we share our clinical experience with the use of the medial plantar artery-based flap for coverage of tissue defects around the heel. Eighteen medial plantar artery flaps performed from January 1996 to December 2009 were included. All the procedures were performed by 2 surgeons at Aga Khan University and Hospital (Karachi, Pakistan) and Bahawal Victoria Hospital (Bahawalpur, Pakistan). Of the 18 patients, 16 were male and 2 were female. The indications were traumatic loss …

Reconstruction of neglected Achilles' tendon defect with peroneus brevis tendon allograft: a case report.

This case report describes the use of a peroneus brevis allograft to reconstruct a neglected Achilles' tendon injury in a 75-year-old woman. She had difficulty walking, had stiffness, and was unable to perform a heel raise from a single-leg standing position. Magnetic resonance imaging confirmed a 6.8-cm wide defect 13 months after the initial injury. Surgical repair of the neglected rupture was performed using 4 strands of peroneus brevis allograft to bridge the defect. Early weight bearing and rehabilitation was allowed. At follow-up clinical examination 14 months postoperation, the patient …

Covering small defects on the weight bearing surfaces of the foot: the free temporal fasciocutaneous flap.

Although defects in the weight bearing area of the heel can be covered by local flaps, radiodermatitis is a contraindication to these flaps. Thin free flaps, as grafted fascial or muscles flaps and thin fasciocutaneous flaps, are usually the option of choice in these particular defects. These reconstructions are prone to shearing strains resulting in ulceration, hypertrophic scars and hyperkeratosis. The authors present a retrospective study of the reconstruction of six small heel defects with the fasciocutaneous temporal free flap performed between 1996 and 2001. The mean size of the defect w…

Compensatory advantages of toe walking.

OBJECTIVES: The study's hypothesis is that toe walking requires less peak muscle strength distally about the ankle and knee compared with normal heel-toe walking and thus may have compensatory advantages for patients with upper motor neuron injury and distal muscle weakness. DESIGN: Motion analysis and force platform data were collected in able-bodied subjects during toe walking and normal walking. Sagittal plane joint torques reflecting muscle force requirements and joint powers reflecting nonisometric muscle contraction were compared between the two conditions using paired t tests, applying…

Ankle and subtalar synovitis in a ball-and-socket ankle joint causing posterolateral painful coarse crepitus: a case report.

A 17-year-old girl with bilateral ball-and-socket ankles reported left medial heel pain. Her left heel had gone into a varus position on tiptoeing, and a painful clunk had occurred when returning to normal standing. The clunk persisted after physiotherapy and treatment with an orthosis. Subtalar arthroscopy and peroneal tendoscopy showed mild diffuse synovitis of the ankle joint, especially over the posterior capsule, and a patch of inflamed and fibrotic synovium at the posterolateral corner of the subtalar joint. The clunk subsided immediately after arthroscopic synovectomy and had not recurr…

Effects of ankle-foot orthoses on hemiparetic gait.

OBJECTIVE: Ankle-foot orthoses (AFOs) are widely used to provide optimal ambulation in people with hemiplegia. In this study we evaluated the mechanical effects of metallic and plastic AFOs on severely hemiparetic stroke patients. METHODS: Twelve hemiparetic patients were analysed on a Vicon 370 Motion Analysis System. Spatiotemporal, kinematic and kinetic parameters were measured. RESULTS: The two types of orthoses generally had similar positive effects on hemiplegic gait parameters, increasing cadence, walking speed, single and double step length, ankle dorsiflexion angle at heel strike an…

Congenital idiopathic clubfoot deformities.

Clubfoot is a birth defect that is marked primarily by a deformed talus (ie, ankle) and calcaneous (ie, heel) that give the foot a characteristic "club-like" appearance. In congenital idiopathic clubfoot (ie, talipes equinovarus), the infant's foot points downward (ie, equinus) and turns inward (ie, varus), while the forefoot curls toward the heel (ie, adduction). This congenital disorder has an incidence of 1 in 400 live births, with boys affected twice as often as girls. Unilateral clubfoot is somewhat more common than bilateral clubfoot and may occur as an isolated defect or in association …

Fetal growth retardation in relation to maternal smoking and weight gain in pregnancy.

Two types of fetal growth retardation were recognized in term infants. One type was characterized by an abnormally low ponderal index (defined as birth weight in grams X 100 divided by crown-heel length in cubic centimeters.) The other type of growth-retarded infants had abnormally short crown-heel lengths for fetal age. Both types were observed under all conditions studied. However, mothers who smoked cigarettes during pregnancy were more likely to have infants with short body lengths for dates, whereas mothers who had low weight gain in pregnancy were more likely to have infants with low pon…
